Sentara Northern Virginia Medical Center (SNVMC) launched the Sentara Foot & Ankle Center. The Ribbon Cutting Ceremony was held on September 23, 2019. The physicians who support the Sentara Foot & Ankle Center perform a multitude of procedures, from ankle replacements and pediatric flat foot reconstructions to trailblazing minimally-invasive Subchondroplasty® procedures.

Query: Prior Authorization on a Replacement AFO
 
I have a patient who received a custom-made ankle foot orthosis (AFO) approximately three years ago. Her condition and foot deformity have worsened and now the AFO is no longer fitting. She needs a new, custom AFO. However, I know there is a five year look back from Medicare unless there are mitigating circumstances such as this.
 
How do I get a prior authorization for the new brace? Is there a form I need to fill out somewhere? The patient has traditional Medicare insurance. When I call Jursidiciton B Medicare DME, they are telling me two different things. The first representative said I needed to fill out a “Redetermination Form” to get the approval first. The second representative said I needed to dispense the product first, bill, get a rejection, and then send in the “redetermination form.”

Response: My advice is to submit the claim and be prepared to appeal. Your documentation should include the very elements described in your narrative. Your documentation seems to be sufficient. This will be your basis for your appeal.
 
If possible, have your patient sign an ABN which is specific regarding Medicare’s reasonable useful lifetime of 5 years.

I participated in my very first malpractice jury trial as the lead counsel almost 30 years ago. I was able to garner a nice verdict for my client. The defense attorney was a partner in a fairly well known malpractice defense firm in the San Francisco Bay Area.
 
Remember that if you are even a newly-minted podiatrist, you know more about the foot and foot function than just about all other physicians! I believe when it comes to saving a foot from amputation that our service is truly priceless.
