The Eatery 309 in Quakertown is set to open Monday. The building, which formerly housed Wawa, will feature three separate businesses: Burrito Boy, Pita Girl, and YoYum Frozen Yogurt. "I got the idea at the airport," says owner Dr. Austin Sedicum, who has operated his own podiatry practice a few buildings down from The Eatery 309 for almost 15 years.

AZ Podiatrists Lecture on The Future of Technology at SAWC 
 
The Southern Arizona Limb Salvage Alliance's Drs. Nicholas Giovinco and David G. Armstrong gave a series of three lectures at the Symposium for the Advancement of Wound Care (SAWC) in Las Vegas this weekend focusing on our collective future where, according to Armstrong, "Consumer electronics will merge with medical devices and even us."

Giovinco serves as Director of SALSA's 3-D Replicator Lab, where they are working on next-generation printed medical, surgical, and consumer devices. In addition to discussing advances in wound care diagnostics,  Armstrong predicts a day in the near future when 3-D printing will be the next Napster-- but for physical things. Instead of downloading an MP3, we will be downloading a file and printing out things.

Without a conscious effort to change the current system, we are creating foot and ankle orthopedists without the MD degree rather than regional specialists of the foot and ankle. That is not the best model for this profession. Whether we are doing this by intent or passive neglect, this is happening.

UNDERSTANDING THE AFFORDABLE CARE ACT
Medical Groups Fear ACA Grace Period Will Lead to Unpaid Claims
 
Physicians are calling for real-time patient information regarding insurance coverage eligibility status from health plans purchased on new exchange marketplaces, citing a potential loophole that could leave doctors holding the bill. Doctors, practice administrators, and hospitals have continued to express concerns to the Centers for Medicare & Medicaid Services about claims for services going unpaid when patients stop paying premiums but still retain insurance coverage. Beneficiaries delinquent on their premiums are given a 90-day grace period before their coverage is dropped. During the final two months of that grace period, health plans are instructed to designate claims submitted for physician services as pending, but the services ultimately could go unpaid.
 
The health plan would be responsible for paying claims during the first 30 days after a patient enters the non-payment grace period. If the enrollee continues to be delinquent in paying premiums, practices and hospitals would be left to collect payment directly from the patient when claims during the final 60 days of the grace period are rejected. MGMA-ACMPE, an organization for medical practice managers, has urged CMS to pay claims during the entire grace period.

The Patient Protection and Affordable Care Act and Centers for Medicare & Medicaid guidelines and rules resulted in a record number of investigations and enforcement actions in 2011. My office has been using Blue Orchid Marketing for over 2 years. We have been very satisfied with the results of patients returning to the office. We have used several campaigns over this period for our diabetics and heel pain patients, and just patients not returning to the office in over 2 years.   
 
The recall messages are diagnosis-specific with compelling copy, and are done regularly as not all patients may have any foot concerns the first time they get one of the recall letters. The campaigns run simultaneously and we can track how many people come back for any specific diagnosis. It is all automatic, so this ensures the best success.

If you are just selling the list, that requires patient permission. If you are transferring the complete patient records to a new provider who will be taking over the care of patients in your practice, that is another matter. So the question becomes, are you transferring the patient records for treatment purposes, or are you just selling the list?
 
Under HIPAA, sharing patient records with another provider treating the same patients is one of those disclosures that does not require patient permission. So if you mean to say you wish to transfer the patient records to another provider who will be taking over the care of the patients, then this is allowed and you do not need patient permission under the HIPAA regulations. At that point, each patient should be notified of the new location of the records, and the name and address of the provider who now has possession/control of those records. As noted by the editor, there may also be more stringent local regulations that are applicable.

Unfortunately, simply perforating the disk case with a drill bit a number of times is not sufficient. This is because it is still possible (though unlikely) that data could be retrieved from the disks. I DO NOT recommend this method for destruction of protected health information (PHI).  
 
A few years ago, I 'tested' the sledgehammer method of destroying a hard disk. I hit the disk many times with a heavy sledgehammer. The spindle of the disk broke, and the circuit board was thoroughly fragmented. I then opened the case and found that the platters were still in good shape. I am 6'1" and in fairly good shape, and hit the drives hard.
 
Based upon my personal experience with testing this method, I strongly recommend that this method not be trusted to destroy PHI. Please either shred the disks or use software that meets the published standards.
