Most Women Wear Wrong Sized Shoes: TX Podiatrist

"By the time most people enter adulthood, they're wearing shoes that are one-half to a full size too small," says Leslie Campbell, DPM, a podiatrist and spokesperson for the American Podiatric Medical Association. This can lead to everything from bunions to hammertoes, which are not only unsightly but can be painful.

In addition to properly sizing your feet and helping you find the right shoes — which can include heels, as long as they don't twist from side to side and bend only in the toe box — podiatrists have solutions for more cosmetic concerns, such as discolored corns (a problem for women of color) and varicose veins around the ankles. Incidentally, a visit to the podiatrist is covered by most insurance companies.

Des Moines University has a new educational resource for students and faculty, courtesy of Brad Bakotic, DPM, DO and his company. The Bako Pathology Clinical Skills Lab and Classroom is the result of a $100,000 gift to DMU from Bako Pathology Services in Alpharetta, Georgia, where Bakotic is chief executive offer.

Cash Stolen From CT Podiatrist’s Office

Police say they are investigating a larceny at a local podiatrist’s office. Police said that on Sept. 26 an employee of Associated Podiatrists of Fairfield called to report that an unspecified amount of cash had been taken from the office. Police said the cash was first noticed missing on Sept. 11. The employee said that other miscellaneous items had disappeared from the office in recent weeks as well. Police said the owner is seeking restitution and/or an arrest.

MEDICARE NEWS

Care-Coordination Initiative Announced

The CMS will pay primary-care doctors to better coordinate care under a pilot program that would require other public and private insurers to make a similar investment. Primary-care doctors will receive an average of $20 per month for each Medicare fee-for-service enrollee to coordinate care in five to seven communities where most other insurers also agree to take part.

Care coordination payments or other support from public or private insurers “will be expected,” as part of the program, according to the solicitation. Insurers must also agree to shared savings, disclosure of cost and medical use data, and should be willing to coordinate quality and other measures. Medical practices that participate in the program and meet quality measures will be eligible to keep a share of savings.

MEDICAID NEWS

Medicaid Claims Audits to Begin in January

Physician and hospital organizations praised several changes federal health officials made to an earlier proposed version of a new Medicaid claims audit program authorized by the health system reform law. The Centers for Medicare & Medicaid Services on Sept. 14 released a final rule detailing implementation of the Medicaid Recovery Audit Contractor program, based on a similar Medicare program in operation nationwide. The Medicare RACs have come under fire from physicians for what doctors have termed aggressive auditing tactics, prompting some critics to label them "bounty hunters."

Some aspects of the Medicaid audit program will mirror the Medicare approach. States will contract with the Medicaid RACs, which will search for fraud, waste, and abuse in the program by reviewing past claims that already have been paid. Auditors will be compensated based on a percentage of funds they recover that were paid inappropriately to doctors, hospitals, and others. The final rule also directs states to pay reviewers for uncovering underpayments that must be reimbursed to those filing the claims.

The days of splurging on week-long training retreats that cover a range of topics are long gone. Today, the focus is on inexpensive, customized training that centers on specific and immediate needs. Adopt these ideas:

  •  Arrange job swaps, work shadowing and in-house mentoring.
  •  Ask suppliers to run training sessions on the use of their materials or equipment.
  •  Set up an in-house library of business books and training tools.

RE: Compromised NPI Number

If you are concerned about a compromised NPI number, don't. Go to nppes.cms.hhs.gov/ plug in a doctor's name, and not only can you get the NPI number, but you can also get the Medicare number, Medicaid number, and in some instances the DEA# and other information. All the information is on the World Wide Web available.

For a fee, one can go to KNOWX.COM and get the DEA information as well. There are many well-informed websites out there. Additionally for your patient, the patient only needs to look at Rx bottles. The doctor's DEA and state registration # are printed for convenience. The information is available without a lot of work or even little knowledge of computers and the Internet. So, DON'T WORRY. There are some things that you can control but there are many more you cannot.

Dr. Rogers brings up a point that has become a boiling point in my office too:  social media, personal texting, personal emailing, Facebook, Twitter, etc. All distract and take away from the work to be done in the office. I had to put policies in recently, as not only my medical staff but also my doctors have been abusing the systems of texting, Facebook, etc. 

It is distracting at work when they become "FRIENDS" on Facebook, and the doctor/staff relationship becomes a problem.  How does this become a problem?  They start sharing personal information, sharing weekend adventures and photos, information that  they would otherwise not share. Schools here have disallowed teachers and students from being "friends" for the same reason. It became a problem of polarizing our office. I started with no Facebook during working hours. Well, that didn't work. Now it's NO Social media or personal text at all in the office. This new policy was one of the reasons to have to dismiss a couple of doctors and a couple of their "FRIENDS" for violations. It is a very slippery slope that should not be allowed.
