PRACTICE MANAGEMENT TIP OF THE DAY |
8 Tips to Improve Patient Satisfaction
It’s more important than ever to be able to track and monitor patient satisfaction and take steps to make improvements to meet the changing expectations of patients. To effectively gauge what patients think and identify areas for improvement, here are best practices to follow.
8. Follow up
Make sure that as you implement changes and settle into new ways of doing things that you follow up to see what patients think. Surveys should not be static. If you’ve made changes, then change your post-visit survey or send out a single survey specifically about those changes. Following up shows patients you are committed to improving their experience and continues to engage them in the process. These types of ongoing interactions and engagement are a key way to build loyalty at a time when patients are more likely than ever to go elsewhere when they are dissatisfied.

CODINGLINE CORNER |
Query: Cuneiform Repair
A patient sustained a right foot, medial cuneiform fracture. It was a transverse fracture that extended into the navicular cuneiform joint and tarsometatarsal joint. I fixated the medial cuneiform and explored the navicular cuneiform joint and 1st metatarsocuneiform joint to make sure the cartilage was not damaged and needed a fusion. Do I just bill an open reduction and internal fixation of the cuneiform, or can I bill capsulotomy of the joints described above? What CPT code and ICD-10 code would I use?
California Codingline Subscriber
Response: So, the patient had a transverse fracture of the medial cuneiform. It was treated by open reduction and internal fixation (ORIF). The appropriate CPT code to bill is CPT 28465 which is defined as the following: Open treatment of tarsal bone fracture (except talus and calcaneus), includes internal fixation, when performed, each. The fracture extended into both the navicular-cuneiform and the 1st metatarsal-cuneiform joints. Obviously, it was best to explore the adjacent joints to ensure that there was no cartilage damage present. If it was present, fusions would need to be performed.
Based upon the fact that there was no evidence of further damage to either the navicular-cuneiform joint or the 1st metatarsal-medial cuneiform joint necessitating further surgery to be performed (i.e. joint arthrodesis), the capsulotomies that were performed within the 2 adjacent joints would be considered incidental to the primary procedure that was performed, the ORIF of the medial cuneiform fracture, and would not be separately reimbursable.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 |
From: Brian E. Homer, DPM
I agree with Dr. Bardfeld. Growing up in suburban Detroit, I learned about the power of unions at a young age. Fast forward to 1991 when I started my PSR-24 at The Cambridge Hospital in Massachusetts. One day, I was standing in line at the bank depositing my $200 weekly paycheck. Alongside me was a 1st year medical (MD) intern. He saw the amount of my check and asked if it was for CME reimbursement. I informed him that it was my weekly paycheck, then he showed me his which was approximately triple the amount of mine. He then suggested that I complain to the House Officers Union Rep.
I first spoke with our residency director who had no knowledge that a House Officers Union even existed. After contacting the Union Rep, we learned that podiatry was the only specialty in the hospital that was excluded from the Union. She immediately appealed to...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 |
From: Michael Zapf, DPM, MPH
I have been pondering the various viewpoints about podiatrists abandoning nail and callus care. Maybe it is about time for our schools to create a two year program that, without a residency, would allow a graduate to trim nails and calluses. Maybe we could call the graduates, oh, I don't know... how about chiropodists?

RE: Reimbursement for Custom Orthotics (Alan Mauser, DPM)
From: Pete Harvey, DPM
Here’s just one more thought concerning orthotic fees. The orthodontists don’t charge for those little wires. They charge for knowing and applying HOW the wires work!
