PODIATRISTS IN THE COMMUNITY
OK Podiatrist Cites ICD-10 as Reason for Early Retirement

Dr. Frank Aaron, owner of the Woodward Foot Clinic is retiring on Oct. 1 after 40 years of committed service helping people take care of the very foundation of their bodies, their feet. “I've done just about everything one can do as a podiatrist, and I've enjoyed my work very much," he said. “The only things that I have disliked over the years are government and insurance intrusion. It makes things more difficult for both the patients and doctors who care for them.
Dr. Frank Aaron (Photo: Richelle Baker)

"I could very well have continued practicing until I was 75 and that was my original plan, but the government is changing disease classifications from ICD-9 to ICD-10 which will be initiated Oct. 1st. All of the old disease classification codes will be done away with on Sept. 30th. There will be great chaos in the medical community, doctors won't be able to see as many people as they have been because it will take more time to document. All so that the government can have more information on everybody. It's the reason why I've decided to retire now,” Aaron said.

PRACTICE MANAGEMENT TIP OF OF THE DAY
Seven Ways to Beat Telemedicine at Its Own Game - Part 1

Have you ever wondered if payers are helping primary care by offering complimentary access to care through 24/7 telemedicine physicians, payer-owned and operated health centers, or drug store clinics? Or are these efforts undermining the physician/patient relationship that is the supposed key to patient engagement (and thus bending the healthcare cost curve)?

Here's what I suggest physicians do to address this competition:

1. Accept that these alternatives are not going away.

Especially since current primary-care shortages are being forecasted in many areas. Complain and protest as we might, it's a Don Quixotic fight that distracts from the core strategy of out-servicing our new competition.

QUERIES (EMR)
Query: Greenway PrimeSUITE for EHR

My hospital is using Greenway PrimeSUITE for EHR. I find this system cumbersome to create templates. There are podiatry templates that I can import, but they are incomplete and not intuitive. I was wondering  if anyone has used this system and if there are any help/tutorials that I could use.

QUERIES (NON-CLINICAL)
Query: Recommendations for Software

We are a 3-doctor practice and are switching the billing from outside to here in the office. We are looking to find a software system that can handle our podiatry-specific needs and would welcome any suggestions that you may have. We are using Amazing Charts which is an HL-7 based system that we are generally very happy with. We will, of course, be utilizing electronic submission, and still need to figure out the process of sending statements out as well.

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Don Peacock, DPM

I am a proponent of MIS surgery and I feel the debate on fixation is off-topic when discussing the advantages of MIS foot surgery. I have performed the Katzen modified MIS Wilson and was personally taught this surgery by Dr. Burton Katzen. His technique is incredible and the results are great in the appropriate patient.

That being said, some patients require fixation and some do not. The decision to use fixation lies mostly in what...

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: Ed Cohen, DPM

There are some podiatrists doing great surgery on bunions using either limited fixation or non-fixation. The trend in medicine is toward minimally invasive surgery. This movement in podiatry began 40 years ago and has really seen some state-of-the-art advancements which give better functional and cosmetic results.

Advantages of minimally invasive foot surgery are that it is a safer technique when working with the higher risk and geriatric patients who are not good candidates for...

RESPONSES/COMMENTS (NON-CLINICAL)
From: Richard Adams, DPM

In my opinion, the Mindray Z6 is the best model for all uses in a clinical or surgical setting. It exhibits precision with guided injections, but also performs admirably for diagnostic ultrasonography and vascular analysis. It is quite cost-effective, with the price point being many thousands of dollars below its competition.

Complete disclosure necessitates that I note my consulting arrangement with Mindray. However, I was using the Mindray Z6 long before any financial arrangement was ever considered.
