IPED Sponsors Student Suture Workshop at Scholl College
 
The Institute of Podiatric Excellence and Development (IPED) recently held a suture workshop for the students at Scholl College of Podiatric Medicine. This was one of several workshops and lectures that are offered to the students involved in IPED’s first student chapter. President and third year student Monte Schmalhaus said, “This was a great opportunity for students to learn proper suture techniques before going out on externships.”

NY Podiatrist Receives HSBC Community Service Award
 
On September 20, 2018, HSBC hosted its 2nd Annual HSBC Cares Awards Dinner and Ceremony at Gotham Hall in New York City, New York with 1,000 invited guests. HSBC Cares is an award program that recognizes everyday heroes who make a difference in their community. HSBC honored three of their customers and one HSBC employee who make a significant impact in the areas of Education, Environment, and Diversity. Winners were recognized with crystal awards.

Dr. Kathleen Reilly Fallon, a Manhattan-based podiatrist, received the HSBC Cares 2018 Customer of the Year Award in Education. She received the honor as her charity - Heavenly Productions Foundation, a 501c-3 not-for-profit helps children in need with backpacks, school supplies, classroom supplies, and library books annually. To date, Heavenly Productions Foundation has donated over 10,000 backpacks and school supplies to students in need. HSBC also presented a check for $10,000 donated in Dr. Reilly Fallon’s name to the PENCIL, an organization that creastes innovative and impactful models of ongoing collaboration between business and public schools.

Lawrence Lavery, DPM, Professor in plastic surgery at the University of Texas Southwestern Medical Center, is a member of the International Working Group on the Diabetic Foot's (IWGDF) infection writing group.

Doctors and Tax Fraud Schemes - Part 1
 
Here is a list of the IRS’s own “2018 Dirty Dozen” list of most prevalent tax scams. 
 
1. Phishing: Taxpayers should be alert to potential fake emails or websites looking to steal personal information. The IRS will never initiate contact with taxpayers via email about a bill or tax refund. Don’t click on one claiming to be from the IRS. 
 
2. Phone scams: Phone calls from criminals impersonating IRS agents remain an ongoing threat to taxpayers. The IRS has seen a surge of these phone scams in recent years as con artists (falsely) threaten taxpayers with police arrest, deportation, and license revocation, among other things.

Query: Wound debridement along with at-risk foot care
 
I have a patient who comes in regularly for at-risk diabetic foot care (diabetic with vascular issues). She has calluses sub 1st metatarsal and 5th metatarsal, bilaterally, along with dystrophic toenails. At last visit, she presented with an ulceration along the lateral side of her 5th toe from rubbing. The extensor tendon and PIPJ joint capsule was visible. I performed a debridement of the ulcer, skin, including tendon. I billed the following:
 
CPT 11043 (L97.523)
CPT 11721 – Q8 – 59 (E11.51,B35.1)
CPT 11056 – Q8 – 59 (E11.51,L84)
 
The at-risk codes were paid, but the CPT 11043 was denied. My billers are being told by Medicare, “if you add a certain modifier, to unbundle it, it will get paid.” But they won’t tell what the modifier is.

Response: There are several modifiers that come to mind that you can add:
The -51 modifier: indicating multiple procedures &/or
The -59 modifier: indicating a distinct procedural service
 
Either or both of those modifiers should allow the debridement to be reimbursed as well.

Dr. Robert Shafer
 
Dr. Robert Shafer passed away at his home in Painted Post on Sept. 20, 2018, at the age of 90. He graduated from the Ohio College of Chiropody in Cleveland in 1954 and opened his podiatry practice in Sayre, PA in 1956 and provided care for thousands of patients until his retirement 35 years later. He was a 50-year member of the American Podiatric Medical Association.

Query: Removing False Patient Reviews
 
What can we do about online reviews that are patently false and malicious? While most reviews are positive, there is one patient review on Google and now other sites attacking my character, surgical skills, and expertise, submitted by a person WHO HAS NEVER BEEN SEEN OR TREATED BY ME. She claims her surgery was in 2007 and she is still posting reviews nearly 12 years later. Realizing “hell hath no fury like a woman scorned,” she has the wrong guy/gal. Can an attorney do something?

There are neurologists and podiatrists in my own geographic area who do not prescribe any of the varied vitamin compounds for diabetic neuropathy. I respect their right to their opinions.
 
The way I handle these compounds in my practice is that I give the patient a one-month supply of one of the compounds (and there are many) and tell him or her to try it for a month. If the patient feels that it helps them, they will come back every month for a new supply. If it does not help them, they won't buy it again. Some patients will want to run it past their neurologists, endocrinologists, or primary care doctors first; and in some cases, those doctors will give them a green light and in some cases they are told not to try it. I don't lose any sleep over either decision.

I 100% concur with Lancing Malusky, DPM. I use Metanx as my frontline in treatment of neuropathy. I actually prefer Metanx’s sister formulation of Deplin. It’s labeled for depression, but it’s the same stuff – just pure 15mg L-methyl folate. I typically prescribe it as 1/2 tablet qd to 1/2 bid. I then supplement with Jarrow-Brand methylated B12 to prevent anemia. Aside from being contra-indicated in history of cancer or history of seizure, patients can take it with any of their meds. If anything, they are able to take lower doses of their other neuropathy meds.
 
Anecdotally, I noticed that their Raynaud’s symptoms also resolves. Therefore, I use it with good success as a front liner in treating my Raynaud's patients, without the side-effects of calcium channel blockers. One can use it on children as well as adults. You can start and stop on a dime and use it seasonally. I would love to know why the neurologist thinks it’s so bad.
