PODIATRISTS IN THE COMMUNITY
6 TX Podiatrists Named "Best Doctors in Collin County" by D Magazine

The following podiatrists were named "Best Doctors in Collin County" for 2015: (alphabetically) Drs. Jeremiah Graff, Horst Knapp, Steven Miller, Kevin Myer, Mojgan Tavakoli, and Robert Taylor.

(L-R) Drs. Jeremiah Graff, Horst Knapp, Steven Miller, Kevin Myer, Mojgan Tavakoli, and Robert Taylor.

Best Doctors in Collin County is a peer-review voting process. This year, D Magazine mailed a letter to 8,938 local doctors from our online directory, inviting them to vote using a ballot on its website. They could vote for up to three doctors in 38 categories. The nomination form asked them to vote bearing in mind the following question: Which Dallas doctors would you trust with the care of a loved one? This year, 5,971 votes were cast by 542 doctors.

CHIROPODISTS AND FOOTWARE
Canadian Chiropodist Discusses Children's Shoe Sizing

Ian McLean, co-owner of the Simcoe Foot Clinic in Alliston, Ontario, and the president of the Canadian Federation of Podiatric Medicine, says “A very young child will have a hard time telling you if a shoe is rubbing or if it’s too tight,” says McLean. The solution? See if the shoe has a removable insole (many kids’ shoes have this feature). “Take out the insole and place it on the floor,” suggests McLean.

Ian McLean

“Have your child stand on it to check if the shoe will fit properly.” Make sure that there is enough room all around the end, and that your child’s toes aren’t hanging over the front edge. On average, McLean says a young child’s foot tends to grow one shoe size per year. So, even after you’ve bought new footwear, measure their feet every once in a while to make sure they aren’t in need of new pairs.

PODIATRISTS OVERCOMING ADVERSITY
UK Podiatrist Has Double Mastectomy to Avoid Breast Cancer

A young woman who opted to have a double mastectomy at the age of 26 has documented her journey. Charley Wood, from Timperley, Greater Manchester, had both her breasts removed this summer after learning she had inherited the BRCA1 gene. Miss Wood, who works as a podiatrist, lost her mother Lorraine to ovarian cancer in 2006 - and discovered she had inherited the gene last year after a series of tests.

Charley Wood

Having been told she had an 85 per cent chance of developing breast cancer, she decided to undergo a preventive mastectomy to reduce her risk of the disease.

ICD-10 UPDATE
CMS Prepares for Impact of Possible Federal Shutdown on ICD-10 Compliance

Just a few days before the Oct. 1 implementation of ICD-10, the CMS is preparing for another deadline—the possible government shutdown. Medicare and Medicaid payments could be affected if some Republican members of Congress who want to defund Planned Parenthood win the battle against the White House and Democrats.

But Dr. Patrick Conway, the CMS' principal deputy administrator and chief medical officer, said claims would still go through because there would likely be a continuing resolution to let necessary programs function. “(We) would continue to pay claims. And we would execute the ICD-10 transition,” Conway said.

RESPONSES/COMMENTS (OBITUARIES)

RESPONSES/COMMENTS (CLINICAL) - PART 1A
RE: Standard of Care for Bunion Hardware (R. Alex Dellinger, DPM)
From: Burton Katzen, DPM

I have read the recent posts regarding "standard of care for bunion hardware", and the two main concerns seem to be the type and cost of the hardware. If we assume the standard of care is screw fixation, being a Medicare patient myself, I certainly do not want a below-standard-of-care procedure performed on me. Lucky for me and my patients, there is an answer for both concerns - minimally invasive surgery with no fixation at all, which can be performed in an office or outpatient ASC setting. As for standard of care, there are literally thousands of procedures performed on a daily basis all over the world. There are over forty years of detailed documentation, beginning with  Dr. Austin's original work, that show that fixation is not necessary for many types of bunions with osteotomies, and therefore should certainly also be considered as an accepted standard of care. (Do we open and fixate every metatarsal fracture that walks into our office?) Statistics to consider after 35 years of performing MIS:

Pre- and Post-op photos of bunion

Hardware cost - $0.00 - Hardware failure or pin tract infections - 0%
Travel time to hospital - 0 hours - Percentage of time being bumped or delayed - 0%
Cost of plaster casting supplies - $0.00 - Pain, swelling, and disability - lessened
Avascular necrosis and non-unions - 0% to date
Keloid or hypertrophied scar formation to date - 0% to date
Post-operative tourniquet complications - 0% (not used)
Surgery time - varies between 10 - 20 minutes
Appearance of foot at first redressing (usually 7-10 days) (See above)

RESPONSES/COMMENTS (MEDICAL/LEGAL))
From: Spencer Dubov, DPM, Robert Scott Steinberg, DPM

The first area to check is the Wyoming Podiatric Practice Act for scope, including H&Ps. With that in hand, you have a substantial basis to rectify your problem.

Spencer Dubov, DPM, Commack, NY

Is rearfoot within the WY scope of practice? What is the rational behind the refusal to grant rearfoot privileges to a DPM? Has the APMA and state society, been contacted?

Robert S. Steinberg, DPM, Schaumburg, IL

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Return to Practice After 3 Years
From: John Aaron, DPM

If the podiatrist returning to practice after 3 years of retirement is interested in biomechanics, I suggest Human Locomotion: The Conservative Management of Gait-Related Disorders by Thomas Michaud. It is one of the texts used at the NYCPM Biomechanics Department. I have found it to be an excellent reference.
