Charcot Foot is the Most Humbling of All Deformities: AZ Podiatrist

Charcot foot can affect any upper or lower limb joint (forefront, mid-tarsal, ankle joint) but the hallmark deformity associated with this condition is mid-foot (or Lisfranc joint) collapse, described as a 'rocker-bottom' foot. The radiographic hallmarks are bone destruction, fragmentation, bony remodeling and joint destruction. Dr. Robert Frykberg, Chief of Podiatry Section at Carl T Hayden VA Medical Centre, USA, calls it "the most humbling of all deformities, and a difficult and challenging diagnostic dilemma."

Dr. Robert Frykberg

Dr. Frykberg sums up the problem of Charcot foot succinctly: 'It is early diagnosis, early offloading, catching it early which minimizes the problems associated with it. If we wait, if it is left undiagnosed and the patient keeps walking, then it is very limb-threatening as ulcers get infected. So everything is dependent upon a high degree of clinical suspicion, early diagnosis, and early off-loading."

APMA STATE COMPONENTS IN THE NEWS

MPMA Donates Shoes to Kids in Need

The Michigan Podiatric Medical Association, along with a matching donation from the Southeast Division of the MPMA, purchased 250 pairs of athletic shoes to donate to children for the new school year. The shoe donation was enhanced by a generous contribution from the podiatrists at NorthPointe Foot & Ankle in Berkley who gave new socks for each pair of shoes.

Dr. Jodie Sengstock finds the right pair of shoes for a child in need.

MPMA partnered with four non-profit agencies to distribute the shoes: Lighthouse of Oakland County in Pontiac; Coalition on Temporary Shelter (COTS) in Detroit; Turning Point in Mt. Clemens; and Lutheran Child & Family Services in Farmington Hills. The agencies support programs for families in need. Dr. Jodie Sengstock of Feet First Podiatry in West Bloomfield, and Dr. Jeffrey Frederick of NorthPointe Foot and Ankle in Berkley - both MPMA members - fit over 50 children in the PATH program of Lighthouse of Oakland County.

DC Podiatrist Discusses Running Shoe Buying Considerations

According to Stephen Pribut, a doctor of podiatric medicine and past president of the American Academy of Podiatric Sports Medicine, runners should generally go one size larger than their standard shoe size. A runner may actually have different size feet. Time of day also matters when shoe shopping. Pribut suggests going shoe shopping at the end of the day when feet have expanded, or around the time of day you normally run.

Dr. Stephen Pribut

Confused about manufacturer shoe categories? You're in good company. "The industry just made up a bunch of words to describe their shoe lines," said Pribut. "Everyone has their own meaning for the terms."

INTERNATIONAL PODIATRISTS IN THE NEWS

"Bad Feet Saves My Future" - French Podiatrist

Bastien Gonzalez certainly knows his pieds. He started up his own podiatrist studio when he was only 23, in the glamourous Place des Vosges in Paris’s Marais district. He was working on up to 20 people a day, six days a week, with an appointments list stretching to one month ahead. “I called it my happy depression,” he laughs. “Happy because I had amazing artistic clientele, and good money by the time I was 24, but I saw my life being the same for the next 45 years.”

Bastien Gonzalez

Gonzalez loves a challenge; a few weeks ago he came back, exhilarated, after working on athletes at the U.S. Open Championships in New York. “By the way, they all have horrible feet,” he confides, visibly shocked. “You have no idea. I thought high heels and pointy shoes create bad feet, but [the effect from] sport is phenomenal.” "Bad feet are what saves my future,” he jokes, before adding, on a more serious note: “With 26 bones, 103 ligaments, 51 muscles, and three arteries, feet are the most complicated part of the body. So you need to take good care of them.”

OUTSIDE INTERESTS

GA Podiatrist Is Renowned Concert Photographer

As told to CL photo editor Joeff Davis by concert photographer — and podiatrist — Dr. Perry Julien: "This photograph was taken during Ludacris' set at Music Midtown. He performed Saturday afternoon. I shot every band at Music Midtown. In addition to shooting for Creative Loafing, for some acts I was also shooting for the festival, so that gave me unprecedented access. Normally, I get to shoot the first three songs from the pit [the area in front of the stage where photographers normally shoot from], but during Ludacris' set, I was able to photograph the show onstage from the side of the stage," said Julien.

Music Midtown (Photo: Dr. Perry Julien)

"From this vantage point, you naturally become part of the show. To be able to experience 50,000 people focused towards you is truly indescribable. In this situation, it is very different from being in the pit, because in the pit you're facing the stage and when you are on the stage, you are facing the crowd."

REMEMBERING GREAT PODIATRISTS

Late OH Podiatrist's Daughter Finds Success with Support of Big Family

Her life changed forever at the age of 10. So young, yet old enough to understand, Sandra Yu realized her life would never be the same when her father, Gerard, passed away from a heart attack at 51 years old. Dr. Gerard Yu left behind his wife, Barbara, and five children who at the time were between the ages of 10 and 19.

Soccer star Sandra Yu (photo: Michael Chritton)

Seven years later, Sandra and her family remember Gerard and honor him any way they can. Sandra has developed into a top student with a 4.25 cumulative grade-point average and an All-American high school soccer player at Walsh Jesuit. She has an athletic scholarship waiting for her at the University of Notre Dame.

RE: Retirement Options (Joe Borden, DPM)
From: Hal Ornstein, DPM

Having entered my fifties a little over two years ago, I often reflect on my road ahead and how I can make the most impact for myself and humanity in Act II. When you look at the many options we have upon retirement in what we can call the “second half”, you should look at what is coined in the excellent book, Halftime by Bob Buford as taking your life from success to significance.

As podiatrists, we have had a blessed life and in most cases, a fine quality of life. This affords many of us the opportunity to approach midlife and retirement as a chance to begin a better half of life. We have so many resources, life experiences, lessons learned, and a well-stocked tool kit that can help so many outside our offices. In our first half, our focus is on earning, learning, and yearning for more. The second half can transform...
