PODIATRISTS AND FOOTWEAR |
NY Podiatrist Provides Cold Weather Footwear Tips
According to Brooklyn, NY-based podiatric surgeon Dr. Allyssa Knowles of The Brooklyn Hospital Center, there are some easy steps to follow to ensure positive foot health as the cold weather looms. Here, she shares five easy-to-follow tips to comfortably take your feet through the fall and winter seasons.
|
Dr. Allyssa Knowles |
Wear the appropriate type footwear when heading outdoors, warns Dr. Knowles. Since many women may ignore ice and snow, stepping out in heels can lead to conditions such as frostbite. "Boots are the way to go," said Dr. Knowles. For sports enthusiasts, such as runners, Dr. Knowles warns to be mindful of changing into performance footwear that offers moisture-wicking properties due to wet weather conditions. A visit to a pro running store offers plenty of choices.

Falls are Number One Cause Of Fatal And Non-Fatal Injuries Among Adults Over 65
Older Americans need to go for regular eye exams, get rid of their throw rugs, and retire their high heels. These are just some of the pieces of advice experts on aging and government health officials are doling out in tandem with a new report that shows falls are the number one cause of fatal and non-fatal injuries among adults over 65.
In 2014, older Americans fell 29 million times, leading to seven million injuries, according to the report out today in the U.S. Centers for Disease Control and Prevention’s Morbidity and Mortality Weekly Report. About 2.8 million cases were treated in emergency departments, and approximately 800,000 seniors went on to be hospitalized for fall-related issues. More than 27,000 falls led to death.

QUERIES (CLINICAL) |
Query: Anchor for Jones Tenosuspension
It has been some time since I had a patient who needed a Jones tenosuspension. Instead of creating a sling through a transosseous tunnel or using a trephine plug with a button, I would like to use an anchor. I am looking for a recommendation for the easiest and most secure anchor for this application.

QUERIES (NON-CLINICAL) |
Query: To Block or Unblock Caller ID on Your Cell Phone
In this modern day technology, cell phone use is ubiquitous. When I'm not at the office, I return patient phone calls with my cell phone caller ID not showing. It is expected that most people will not answer a "NO CALLER ID" when they receive a call. Our group does not utilize pagers as they are antiquated.
I've spoken to a number of my colleagues who state that there is no need to block my number and that it is okay for patients to have my personal cell phone number. I find this too invasive. It's a task to always having to call your answering service to connect with the patient, not to mention the cost involved in each call they need to put through. Any ideas or am I just being too paranoid about freedom of access to me?

CODINGLINE CORNER |
Query: Medicare DME Orders
Medicare DME dispensing rules require an "order" by the physician, but what does that entail? Is that part of the physician's note or a separate note such as a written prescription? Please advise.
Amy Meehan, Billing Manager, Potomac, MD
Response: The physician order is the prescription. In the case of DME, there would be no difference. The order would need to include patient identifying information, item name/description, diagnosis, and the expected length this device will be needed (e.g., lifetime for an AFO vs. 3 months for crutches).
The physician-supplier does not need to produce a unique prescription for their own patient for DME or supplies. That order can be included within the physician's medical record. You should check with your DMAC to see about any other requirements. Make sure that your records include what you are prescribing, the medical necessity, why a custom device is required vs. an OTC device, the level of disability/impairment, symptoms, etc.

RESPONSES/COMMENTS (EMR) |
From: Michael L. Brody, DPM
A second back-up is not a bad idea. Here are some things to look at: If the company closes their doors will you have access to that second backup? Or is it 'their' account? How often is the back-up done? Is the back-up encrypted? Can you get access to the decryption key? Can you get the back-up file if you need it?
My recommendations: 1) Try the service and then try to 'get' a copy of the back-up. 2) Try to de-crypt it, and then have an IT professional look at the back-up to see if the data can be accessed. 3) Contact some competing EHR vendors and see if they have the ability to 'import' your back-up into their EHR so that you have access to their data.
A back-up that you cannot use does you no good. If you can get your back-up in a form that you can use, then this is an 'extra' protection against the company closing their doors. Alternately, you can ask for a copy of your back-up directly from your current vendor at least once a month and avoid the charge from the third-party vendor. If you do this, you still need to make sure that you have any necessary decryption keys and can use the back-up

RESPONSES/COMMENTS (CLINICAL) |
From: Keith L. Gurnick, DPM
A 7 year old with crossover toes (could also be called under-riding, or varus rotated toes) will not improve or resolve on its own without treatment. This condition is most likely congenital, present at birth, and was either undetected, under-appreciated, or if the parent(s) had asked their pediatrician they were told a) don't worry about it or b) it will resolve on its own once the child begins walking or c) wait until the child is older and see if it gets better or d) if it doesn't hurt, leave it alone.
All of these answers are not only wrong, but show a total lack of concern for the patient for the future or an understanding of how the foot should...
