Walking in Inappropriate Shoes is Damaging to Feet: NJ Podiatrist

"Those who walk 5,000 to 8,000 steps (a day) in inappropriate shoes are doing more damage than good," says Dr. Ritchard Rosen, a podiatrist in Englewood, New Jersey, and chief of podiatric surgery, Holyname Hospital, Teaneck, New Jersey. "When ambulating, a patient will have a certain degree of pronation (inward rolling of the foot) and stretch of the plantar fascia," a band of ligament-like tissue that connects the heel to the ball of the foot.

"If, however, the patient has a high arched or flat foot, sandals will not afford any longitudinal arch support, and a potential stress to the fascia, known as plantar fasciitis, will occur." A classic sign of this is heel pain upon awakening in the morning, or from extended walking.

"High-heeled shoes will cause potentially many problems if worn for any prolonged walking," says Dr. Rosen. "High-heeled shoes will cause shortening of the Achilles tendon and increase the stress to the ball of the foot. Abnormal forces to the ball may cause bursitis, synovitis (inflammation of joint fluid) and possible nerve irritation." Even lower-heeled dress shoes, including men’s professional footwear, can lead to arch strain, tendonitis and plantar fasciitis, says Dr. Rosen.

NM Podiatrist Picked as E.D. of Texas Longhorn Breeders Association

Less than a year after changing leaders, the Fort Worth-based Texas Longhorn Breeders Association of America is looking for another top hand. Larry Barker, a longtime employee, took over as executive director in November after his predecessor up and resigned amid a financial controversy. He expected to stay on a few weeks or months to help with the transition that happens when a leader is hired or promoted. But some board members wanted him to turn over the office keys pronto, prompting a walkout by six board members, including Chairman Ben Gravett of Virginia.

And Fritz Moeller of Socorro, NM, a breeder and retired podiatrist, suddenly found himself the new chairman."Larry was with us 18 years, and that's not the way to treat anybody," Gravett said, explaining his spontaneous departure. "He didn't deserve it, flat and simple." Moeller said board members felt it was an "astute" business practice to have Barker leave immediately. From their career experience, they believe that "if you leave that employee in place [he] can cause a lot of damage," Moeller said.

QUERIES (CLINICAL)

Query: Recurrent Glomus Tumor

We have a patient who underwent excision of a painful lesion which proved to be a glomus tumor. It recurred rather quickly and she had a second surgery, after which the tumor recurred again. She has been receiving steroid injections about 1-2 times per year. These have been helpful at controlling the pain, but are there any other treatment options besides amputation as recommended in the literature? I have searched for alternatives, but mostly I see surgical resection as 'curative.'

RE: Treatment For Verrucae (Kevin Lam, DPM)

I have used a product called "Wart-Peel." It's a compounded formulation of salicylic acid and 5-flourouracil. It is made by NuCara Pharmacy in Naperville, IL. I have had pretty good success with this product.

In evaluating the case and photo presented, there are a couple of questions that need to be asked. How long after the patient's pedicure did the problem begin? In any infection whether it be viral, bacterial or fungal there is a time period between exposure to the virus or micro-organisms and the development of the disease. It’s important to ask this because patients are human beings and they may try to invent connections and create blame. They do this to all kinds of health care professionals, including podiatrists. The pedicure might not have had anything do with the development of the problem. Another big question that should be asked is what do the pathology slides reveal? Are you basing the diagnosis on the written report?

People reading path slides are also human beings and sometimes make errors and are sometimes quick to see a requisition slip that says "foot specimen submitted by podiatrist and quickly read into it "verrucca." I am not saying that this definitely happened in this case, however the photo depicted does not appear to be typical of widely spreading verruccae. Then again it might not be a clear photo or the electronic transfer process might have obfuscated important details. It would be very helpful if you could upload a clear picture of the biopsy slides and then let the pathology experts who read PM News weigh in on whether we are dealing with verrucae or not. A good lab might already have these slides on their computer ready to e-mail to you. This could be a good educational experience for all of us.
