FL Podiatrist Discusses Best Shoe to Wear When Flying: FL Podiatrist
"Sitting for long periods of time on a plane can encourage stasis of the blood in our legs, leading to higher potential for things like blood clots," says Julia Schottenstein, DPM, a podiatrist at The Schottenstein Center, noting that this can quickly become dangerous. "Knee-high boots can lock up the calf muscle even more," she notes.

Instead, the podiatrist says it is best to choose shoes that won't limit leg mobility while on an airplane. "It is also advised to move the ankle, knee, and hip while flying to help with circulation along with wearing compression socks to aid in this as well," she adds.

NBA Star Likely Spends Five-Figures on His Feet: UK Podiatrist
LeBron James is one of the greatest basketball players of all time, but even the greats aren’t invincible. He has a history of suffering ankle injuries during his illustrious career. Marion Yau, a London-based podiatrist, thinks that LeBron spends upwards of $84,000 on foot care.

In an interview with The Sun, Yau claimed, “For his feet generally, I would say that every month LeBron will be likely spending around $7,000 on podiatric care, orthotics, and custom sneakers. LeBron also has a complicated left foot and would need scalpel treatment to get the calluses removed and prevent ulcerations. Now there’s rehabilitation for his right foot on top of that. That’s an enormous bill – adding up to $84,000 a year.”

For the debridement of nails CPT 11720 and CPT 11721 with a systemic condition, do we:
1. Put the diagnosis of the systemic condition first, then list B35.1 (or L60.0)?
2. Use it only by itself?
3. Or list it as a secondary diagnosis after B35.1 (or L60.0)?

Response: With respect to “at-risk” routine foot care, specifically the debridement of nails using CPT 11720 (Debridement of nail(s) by any method(s); 1 to 5) and CPT 11721 (Debridement of nail(s) by any method(s); 6 or more), National Government Services aka NGS, the Medicare Administrative Contractor for the state of New York has an LCD specifically for Routine Foot Care and Debridement of Nails, L33636. In addition to the LCD, NGS also has an associated article for Billing and Coding: Routine Foot Care and Debridement of Nails, A57759. It is certainly advantageous and important to access, download, and review these two publications.
If a patient qualifies for “at-risk” routine foot care based upon the presence of a covered systemic disease and class findings, the primary diagnosis would be the systemic disease diagnosis and, in the example given in the above post, the secondary diagnosis in all likelihood would be B35.1 (Onychomycosis/Tinea Pedis).
Where the patient has evidence of neuropathy, but no vascular impairment, for which class findings modifiers are not required, ICD-10 CM code B35.1, L60.2 or L60.3 respectively, must be reported as primary, with the diagnosis representing the patient’s neuropathy reported as the secondary ICD-10-CM code. These ICD-10-CM codes are listed within article A57759 under “Group 4 Codes.”

RE: The Passing of James H. Canavan, DPM
James H. Canavan, DPM passed to Eternal Life on September 10, 2023 at age 97 years. James was a U.S. Navy Corpsman with the Marines in World War II. He served on the West Allis Fire and Police Commission for 15 years, and helped organize one of the first Paramedic’s in Wisconsin for the West Allis Fire Department; and it is still known today as ‘Med-1’.

Query: Reimbursement for Custom Orthotics
What do people get paid for custom orthotics? I charge $400 for a pair of custom orthotics. I have done so for the past 35 years. Insurance in my state reimburses me $69.97 per orthotic. Essentially, I'm getting less than $140 to make, evaluate, dispense, and possibly adjust the product... all for a profit of about $30. I am curious as to what others get paid. I have always heard that orthotics are profitable for practitioners in other states, but never for me - unless it's paid in cash.
Orthotics are one of the mainstays of our practice as podiatrists. I feel obligated to provide a service to my patients, even though I received little benefit in terms of reimbursement. It is very difficult to recommend a custom orthotic device when I can simply dispense an over-the-counter product that usually does work well and is much more cost-effective. Don't get me wrong, custom orthotics are an integral part of my practice, just like surgery which also does not pay well. We are all surgeons and that is what we love to do.

RE: It's Time for Certified Podiatric Assistants

Podiatrists have fought for parity for years and, in doing so, we have incorporated advanced training with podiatric residencies and are now rightfully recognized as true foot and ankle surgical specialists. It is true; however, some of us have lost sight of the need for care of such mundane problems such as corns, calluses, and the need for palliative at-risk foot care.
Consider, also, that the provision of medical care is morphing into large, managed groups and that many of us have become employees. Our ability to manage our future as independent providers of podiatric services is diminishing.
I suggest that we consider a path to have certified podiatric assistants in our practices. The dentists have done it. They have assistants who provide prophylactic care under their supervision. If we don’t do this, and do it soon, we will lose our status as general foot specialists and only be recognized as foot and ankle surgeons. We will be at the mercy of the behemoth managed care organizations, and they will readily delegate at-risk foot care to others.

While unionizing sounds great in theory, in my mind, the actual practice would be quite difficult. The power of the union is derived from the power to strike. This action is supposed to directly hurt the parent company by reducing revenue. If a company is not making x number of widgets, they are losing money. In the case of practicing doctors, who are we hurting? If you are in private practice, you are hurting yourself. But even worse, if many or all of us are not treating patients, are we "hurting" them as well? I can imagine a deluge of lawsuits claiming abandonment.
And from whom are we asking for more money? The federal government? Insurance companies? Fat chance they will give us any more. It has been said many times before that the way forward is an en masse transition away from accepting insurance altogether.

The query was made as to whether people are content with having the hospital system buy their practices and allowing themselves to become employees of that system. This reminds me of a friend who decided to take this move for his own practice and seemed delighted with the decision when we spoke. About a year later, he's on the phone with me, lamenting the choice, as the hospital system decided that he was making too many referrals outside the system for specialists and ordering too many MRI exams and decided not to renew his contract. They buy his practice. He's elated. Then they institute limitation of care initiatives to contain costs, and he's on the street. I offered him a listing of head-hunters I knew and helped him secure a new position. I'm guessing that he would likely NOT consider being an employee of a hospital system again.
Knowing a few people who work within the NHS in England and the Canadian system, cost containment is a huge issue. Just a few years ago, the NHS dealt with a strike by junior medical officers which lasted for a while and pointed out the poor reimbursement with long hours endured by doctors in the NHS. It is difficult to say what the final word will be on this, but with the effects of inflation upon current earnings and expenditures, something has to give. When you are reimbursed less when you do more procedures, you have turned the other cheek more than enough times.

Recently, multiple meetings concerning Boards have been held without resolution. These meetings ranged from the CPME and SBRC, multiple meetings behind closed doors of leadership to which we were not invited, Board Summits in 2015 and 2023, and hearings that we have had on the subject of Boards. One of the “recognized” Boards held a Town Hall on Board Certification in 2023, which APMA did its very best to thwart.
