NY Podiatrist Explains How HBOT Can Help with Healing
 
Joseph Canzoneri, DPM, a podiatric surgeon with Rochester Regional Health explains how HBOT therapy works, who can benefit from it, and what patients can expect when they begin treatment. “Hyperbaric oxygen therapy is considered an adjunctive therapy,” Dr. Canzoneri said. “This therapy is not a treatment that is done on its own; it is often combined with other methods of care – including wound and infection management.”

Patients who undergo this therapy will need to have 30-40 sessions that are conducted five days a week. “From the first treatment to the last one, hyperbaric oxygen therapy is a safe and reliable therapy for those who need it,” Dr. Canzoneri said.

Earlie Hairston, DPM, who maintains a private podiatry practice in Spring Hill, volunteers at the PHP Health Center once a month for uninsured low income individuals who meet eligibility.

You might think that the pay rate would top the list of what people consider for staying at a particular job. However, job fulfillment, personal growth and development, and appreciation can rank higher in terms of employee satisfaction, according to informal surveys I’ve done at medical conferences.
 
These factors can go a long way to boosting employee retention. Employees need to feel inspired and motivated. Obviously, we have to pay them fairly, but they also have to feel like they’re appreciated. Two words at the end of a workday can go a long way: Thank you. You can also say, “Thank you so much for what you did today,” or “Thank you so much team — you’ve done a great job today.”

Query: List of FDA-Approved Medications for the Treatment of Mycotic Toenails
 
Florida’s First Coast Service Options’ LCD L33922 states: “Appropriate anti-fungal treatment is necessary to qualify nail debridement as a medically necessary and reimbursable service unless contraindicated. If an anti-fungal treatment is not used, the contraindication must be documented in the medical record.”
 
A recent publication that was mailed to me from First Coast Service Options states: “For coverage of mycotic nail debridement, mycologic confirmation by culture, potassium hydroxide examination, or dermatophyte testing is expected to differentiate fungal disease from other nail pathology in certain circumstances including but not limited to the following conditions: previous unsatisfactory treatment results (recurrent nail disease, unsuccessful treatment with FDA-approved antifungal medications, long term - beyond 12 debridements per 24 months, etc.) and for patients whose debridement is prescribed absent of concomitant pharmacologic therapy.
 
Definitive treatment of mycotic nails involves the appropriate use of effective antifungal pharmacologic agents with or without periodic debridement of dystrophic nail plates to lessen the fungal load. Medicare will cover debridement of mycotic nails as an adjunct to pharmacologic treatment with a prescription antifungal agent indicated per its Food and Drug Administration (FDA) label for the treatment of fungal nail infections.” What are the FDA-approved medications for the treatment of mycotic toenails?

RE: Association of Lower Extremity Peripheral Nerve Impairment with Dementia

The journal Neurology, in an editorial by Paretsky, MS and Roman, GC, (May 3, 2022), describes that evidence is emerging that lower extremity peripheral nerve impairments are associated with and may even precede dementia. Clinically, this is manifested by slowed walking speed. In addition, poor gait has been shown to predict dementia. The authors indicate that motor and sensory impairments have been associated with Alzheimer's disease. Mechanisms linking the peripheral nervous system to dementia are unclear. However, pathogenic factors support the relationship.
 
A typical pattern of peripheral neuropathy sensory loss (distal, bilateral, symmetrical) has been described in the elderly. This pattern is thought to be due to loss of energy production in the cell bodies in the dorsal root ganglia because of metabolic, toxic, and neurodegenerative etiologies. Neurodegenerative proteins found in the dorsal root ganglia suggest that the same process that occurs in brain neurons in the CNS also occurs in the peripheral nervous system.

The recent press release by the AACPM seemingly opposing ABPM’s CAQ in podiatric surgery is unfortunate. It implies the ABPM is in non-compliance with CPME Documents, which we are not. The ABPM issues CAQs under its own authority and will continue to do so. 
 
Thus, we believe our time is better spent inviting the AACPM to collaborate with the ABPM to address their student recruitment crisis, in which there were only 716 applicants for 697 seats in 2022, a 21% decrease of applicants from 2021. If you read online student forums, prospective students perceive uncertainty in an educational process that results in an abysmal pass rate for a board which holds itself out to be the gatekeeper to operating rooms across the country. We, as a profession, must do better at working together in order to remain relevant and viable.
