CA Podiatrist Discusses Fall Prevention
This is Fall Prevention Week. "When discussing fall prevention, everything from slippery floors, loose throw rugs, poor lighting, medication side-effects, wet spots, and pets have been cited as contributing to the growing tendency to fall as we age. Foot pain and foot problems, when mentioned, tend to be near the bottom of the list," said podiatric foot and ankle specialist Diane Branks, DPM, Vice President of the California Podiatric Medical Association.

"Since our feet are the foundation for our balance and mobility, it only makes sense that healthy feet would play a key role in helping to prevent falls," said Dr. Branks. In a randomized clinical trial of foot care as a way to prevent falls, Australian researchers found that the volunteers who received foot care experienced 36 percent fewer falls than those in the control group that did not.

Hal Ornstein, DPM, Managing Partner of Affiliated Foot & Ankle Center, LLP in Howell, New Jersey presented a lecture entitled "How to Move You and Your Business to the Next Level". This event was open to business leaders in the community and members of the Howell Chamber of Commerce.

Ornstein discussed practical ways to elevate various aspects of business and personal lives. “What many want is often in your own backyard; the door just needs to be open and the path cleared,” Ornstein said. He says that changing one’s paradigm and taking daily action can help to open that door. He handed out several inspirational books and stressed the importance of reading for professional and personal growth.

NJ Podiatrist Helps Clear Bees from Hospital's Roof
Ocean Medical Center has 100,000 bees on its roof. People like Point Pleasant Beach resident Greg Mazzatta, DPM, a podiatrist who sidelines as a beekeeper, are doing what they can to help them out. “To be a good beekeeper, you need to be involved with your bees,” Mazzatta said. "A lot of people get them and think they take care of themselves. They think, 'I’m going to go in and get the honey and that’s it.'"

Mazzatta manages Ocean Medical’s apiary. He started with two colonies, building 2-foot rectangular wooden hives for each. There is one queen bee per hive, and as the apiary’s population grew, an additional queen bee emerged to lead a breakoff colony. That group announced its arrival by swarming on one of the hospital’s outer walls, 30,000 strong.

3 Low Tech Ways to Boost Collections - Part 2
2. Learn the magic words - Asking for money is not everyone’s strong suit. Be sure your staff is well trained in the best collection practices. This means knowing how to stay on the right side of collections law as well as how to communicate effectively. Barbie Hays, CPC, coding and compliance strategist for the AAFP, suggests having scripts for employees to follow. “It’s important to teach them not only what to say, but what not to say,” she says.
Training shouldn’t be limited to the front desk. Back office staff need to know the basic fee schedule, too. Nurses and physicians are often asked about costs and payment arrangements. Sometimes, a provider may reassure a patient who is worried about the cost of a procedure without really knowing what the procedure costs. Back office staff also need to know how to talk about payments legally and know not to promise patients things the billing office can’t deliver.

Query: Metanx, Vitamin B6 and Diabetic Neuropathy
I recently prescribed Metanx for a patient with diabetic neuropathy. A few weeks later, the same patient was seen by a neurologist who apparently is a “top neuropathy specialist”. The patient showed the neurologist the bottle of Metanx and was told “it’s the worst thing you can take” and was told to throw it out. The neurologist stated that recent research has shown Vitamin B6, Metanx, is not indicated for diabetic neuropathy. To say the least, I was very surprised to hear that. I have been prescribing Metanx since it has been on the market. Is anyone aware of any recent research that Metanx, Vitamin B6, is contra-indicated for diabetic neuropathy?

I have been using a scribe in my clinic for about three years. The benefits of a scribe are many. The benefits of a virtual scribe include one less body in an already cramped exam room, no necessity to purchase equipment, and no additional employee on your payroll.
The downside of a virtual scribe involves occasional, but rare, IT concerns. In my opinion, if you choose the correct virtual provider, you will be very pleased.

I have not used a virtual scribe, so I cannot offer an opinion about that. Pretty sure Dragon Naturally Speaking, which I have used, is not the same. For years, I have used a scribe and have found it helpful in many ways. Patients have commented about how their other doctors are buried in their computer during the encounter, whereas my patient has my undivided attention the entire time. I ramble off my physical exam quietly to my scribe, who checks it off on "Quicksoap". I speak to the patient about their condition and treatment plan as the scribe writes it down.
My scribe will also note if the patient expresses pain with a facial expression, as they don't always tell me as instructed. As I begin my treatment, my scribe is dismissed, and she goes to start the note in the computer at another location. This situation has worked well for me. My scribe has learned to pick up on the subjective part of the note and she relays the follow-up plan to the scheduler.
There are additional benefits. Patients have also developed a rapport with my scribe and will ask about her if she is not present on a given day. She has also developed the habit of reviewing the patient's chart. She will let me know if there are any insurance coverage issues and she will let me know if the patient is due for new diabetic shoes, should they desire it. I have found this system to be invaluable.
