APMA NEWS
MD Podiatrist to Assume Office of APMA Executive Director on October 24th

James R. Christina, DPM, will assume the office of APMA executive director and CEO on October 24 at the fall meeting of the APMA Board of Trustees. Dr. Christina was selected as the Association's incoming executive director after a comprehensive national search process that began in 2014. His candidacy was approved by the APMA House of Delegates in March 2015, and he has been working closely with current Executive Director Glenn B. Gastwirth, DPM, since that time to transition into the position.

Dr. James Christina

Dr. Christina has served APMA as director of Scientific Affairs since 2005. Prior to joining the staff, he was a physician in private practice for 20 years in Rockville, MD. He was chief of Podiatry at Suburban Hospital in Bethesda, MD. Dr. Christina served as president of MPMA from 1991-1993. As an American Board of Foot and Ankle Surgery-certified podiatrist, he was instrumental in achieving an expansion of the Maryland scope of practice for podiatrists to include ankle privileges.

PODIATRISTS IN THE NEWS
Plantar Fasciitis Appears to be Increasing: NJ Podiatrist

Dr. Jeffrey Cohen, chief of podiatry at Englewood Hospital and Medical Center, said he had no formal statistics on rates of occurrence of plantar fasciitis, but that anecdotally, a typical day in his office yields an average of about three patients suffering from this condition.

Dr. Jeffrey Cohen

"It's something we definitely see often," Cohen says. "It has been talked about more lately, most likely because people know about it now and are more educated about it. But it has been around for years. If you compare it to 10 to 15 years ago, then yes, you might say we've been seeing it more often, because of increased sports involvement, weekend warriors. But even way back then, we might have just had a more generic name for it — 'arch strain.' "

PODIATRISTS AND FOOTWEAR
Canadian Podiatrist Offers Children's Shoe-Buying Tips

Finding the right kids’ shoes is just as important as what you put in their lunch bags. If the shoe doesn’t have a removable insole, use your thumb to press on the toe box to get a feel for where the toes are. There shouldn’t be more than a thumb’s width between the longest toe and the end of the shoe. Take note that the big toe is not necessarily the longest toe, says Dr. Hartley Miltchin, a Toronto-based doctor of podiatric medicine. “Parents may be pushing down on the big toe when their child may actually have a longer second or third toe,” he says.

Dr. Hartley Miltchin

“A shoe should fit perfectly right from the start,” says Miltchin. “If a shoe needs a ‘breaking in’ period to feel comfortable, then it means you’ve chosen a shoe that may potentially cause damage.”

PRACTICE MANAGEMENT TIP OF OF THE DAY
Eleven Things to Never Say to Patients - Part 9

9. "This treatment will definitely work."

Dr. Wenjay Sung

Never guarantee the outcome of any treatment. Providing probabilities and statistics is always a welcome practice, but issuing a blanket promise will quickly become problematic.

Treatments have varying degrees of success, based on a multitude of patient risk factors and co-morbidities. Physician skill also plays a role in obtaining specific outcomes, as with surgeries or injections, for example. There are simply too many variables that could disrupt an ideal conclusion. CODINGLINE CORNER
Query: Calcaneal Osteotomy

One of my doctors is asking if CPT 28300 (calcaneal osteotomy) can be billed twice during one surgery. He is talking about two separate incisions to do the type of procedure he wants to do, and I haven't seen anything one way or another stating whether the CPT 28300 code can be billed twice in one session.

Jennifer P., Office of Mid-South Foot & Ankle Specialists, Memphis, TN

Response: Yes, CPT 28300 can be billed twice during one surgery for two different osteotomies performed at two different locations to correct two different deformities

Your options are to bill it is as: CPT 28300 x 2 units or on two lines as:
CPT 28300
CPT 28300-59

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Al Plotkin, DPM

This is to report the passing of long-time Queens Division member Al Plotkin, DPM.  I knew him only as an acquaintance through scientific meetings, but he was always friendly and had a good reputation in the neighborhood. His son, Stuart is a Long Island podiatrist. Condolences to his family. May he rest in peace.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Sherwin Tucker, DPM

Perhaps the podiatrist who is a little rusty after a three-year retirement would also benefit from some hands-on practice as well. I think this would be a lot more helpful than reviewing podiatry texts. Maybe a month-long podiatry externship would be helpful. I do not know where the podiatrist is from, but my office does offer externships.

RESPONSES/COMMENTS (MEDICAL/LEGAL) - PART 1A
From: J.C. (Chris) Mahaffey, MS

If the Wyoming podiatrist referred to is an ACFAS member, please have him/her contact me regarding the College’s Credentialing and Privileging Advisors Team. We have a team of members, all of whom have served on a hospital credentialing committee, who can advise you on this matter. The College has other resources I would be happy to discuss with him/her.

RESPONSES/COMMENTS (MEDICAL/LEGAL) - PART 1B
From: William Deutsch, DPM

I have two suggestions for the DPM considering selling his practice and retiring.

1. Do not ratchet down your practice. Instead try increasing your income and patient load.

2. Market your practice to a podiatrist contented to do only forefoot/mid-foot surgery. After all, how many rear foot surgeries come breaking down your door?

As an alternative, market your practice to an enterprising orthopedist specializing in foot and ankle disorders and willing to open a foot treatment/wound clinic. Forget the hospital cutting podiatry a break. You might have to put your retirement off by a decade.
