On September 21, 2013, the L.A. County Podiatric Medical Society held its 6th Annual One Voice Event at the Peninsula Hotel in Beverly Hills, California to raise funds for Cal PPAC. It was sponsored by Bako Laboratories, Arthrex, Rad Alliance, IV League, Pacific Vasucare, and Bauerfeind.

PA Podiatrist Named Trustee to Juniata College Board
 
The Juniata College board of trustees has added four new members to begin the 2013-2014 academic year. The new trustees appointed to begin their service Sept. 1, 2013 through August 2016 include Dr. Gary Raymond, of Hollidaysburg, PA. Dr. Raymond, a native of Gallitzin, PA, has been the president and owner of Advanced Regional Center for Ankle and Foot Care since 1982 and also is the owner and managing partner of the Summit Tennis and Athletic Club in Altoona, PA.

Raymond has been an active volunteer for Juniata serving as president of the Juniata College Parents Council. He also is a clinical assistant professor at Temple University's School of Podiatric Medicine, and has published articles on clinical research in medical journals. TN Podiatrist Testifies in Successful $2M Lawsuit Against Nail Salon  
 
The superficial nerve fibers on Jenny Howell’s feet were so damaged in a pedicure at a Regal Nails salon that a spinal stimulator had to be surgically implanted to enable her to walk. The damage is permanent and affects every aspect of her life. According to court records, podiatrist Dr. Robert Spalding of Chattanooga, TN, author of the book Death by Pedicure, along with industry consultant and educator Janet McCormick, Frostproof, FL, co-owners of Medinail Learning Centers testified in the lawsuit as industry experts.

Instructions for a callus softener called for only a 45-second application time. According to testimony, nail technician Lilly Le, aka “Helen” applied and left the product on Howell’s feet for over 25 minutes, resulting in extremely painful and widespread blister formation and damage.

AZ Podiatrist to Present Clinical Abstract on Ultraportable NPWT Device
 
Jodi L. Walters, DPM, director of Podiatry Research, Southern Arizona VA Health Care System, Tucson, AZ, assistant professor, University of Arizona, and Diplomate, American Board of Podiatric Surgery, will present a clinical abstract entitled, “Use of an Ultraportable Negative Pressure Wound Therapy Platform for Optimized Healing of a High Risk Surgical Incision and Chronic Ulceration in a Patient with Spina Bifida.” The abstract describes a 22-year-old college student with a history of spina bifida, chronic pain and chronic right foot ulceration with suspected clinical osteomyelitis. He suffered from an infection and a non-healing ulceration that existed for over three-and-a-half years. The patient was treated with the ciSNaP® for use on the closed incision followed by use of the SNaP System for standard NPWT.

The patient’s wound healed rapidly, enabling him to quickly return to active college life, and he remains completely healed after four months. “Use of both the ciSNaP and the SNaP Systems successfully treated a young, high-risk patient,” said Dr. Walters. “This patient’s quality of life had been seriously compromised for a long time due to his chronic wound and infection, resulting in missed classes; however, once we began our treatment regimen, he was completely healed in just over two week’s time.

A 55 year old female presented with pain and swelling in her 5th toe for two weeks, worse after wearing shoes to a wedding. Clinically, the toe is inflamed, swollen, and painful to touch. There is a hammertoe deformity. No history of trauma, ever.

Query: Denial of DME Application
 
I recently sent in my application for participation in the Medicare DME program. I paid, via American Express, the fee of $532, filled out the application (July 2013), and have been waiting for my identification numbers. My office manager called for follow-up recently, and was told that the application (and my file) were closed because (according to a supervisor) of a lack of a response requesting additional information. This additional information request was sent to me as a fax.  
 
I just spoke to a supervisor, and explained that we never received such a fax. She said that unfortunately there is nothing that can be done.  So right now, I am out $532, and cannot have my application re-opened. I would have to start again (and pay an additional $532). Does anyone have any suggestions as to how I would get this resolved?

I recommend that you convert this open amputation to a TMA. If there is a tissue defect, you can always fill it with wound VAC or even a muscle flap later on. HBOT will be helpful in wound healing, given that this was an ischemic event. Acute limb ischemia is indeed one of the recommended HBOT indications by the Undersea and Hyperbaric Medical Society Guidelines
 
I am advocating TMA because I have done many Chopart amputations, with less than ideal outcomes; they are hard to shoe and ambulate afterwards. You can always do a BKA later, if all else fails. Try everything and anything you can for limb salvage. Given that this is a young healthy individual, you will be surprised how well and fast he heals, compared to our usual diabetic and geriatric patients.

After ruling out osteo, vascular compromise, and skin cancer, place the patient in a TCC for 6-8 weeks, removing and debriding the area Q2 weeks. If he can revert this to intact skin, use a multi-density diabetic insole with additional layers of materials and a higher durometer outer shell to accommodate this larger patient as the usual thicknesses found in approved diabetic insoles would be meager and inadequate in this individual. 
 
The Achilles lengthening likely aggravated the heel. Add onto the shoe or CROW-walker outsole an 3/8" heel lift and counter. Adjust the thicknesses by using a pressure plate to verify pressure reduction by at least 1/2 at the heel. Add mid arch PPT layers to augment, as needed. 
 
A daily emollient will maintain the skin. If he has any fat pad left, I would try Tubi-grip, worn daily, or one of those new plantar fasciitis sleeves. The compression might be enough to limit the lateral excursion and tensile strain of the remaining fat pad and overlying skin, allowing for more cushioning of the area during ambulation. If the skin is unable to heal, I'd move to a round of debridement, then apply ACELL before taking this patient to surgery for a graft or flap.

RE: Unstable Scar

It is possible to reduce heel contact shear and pressure to the heel using heel and toe rockers (See attached photo). Rockers are designed to delay contact with the floor until the body passes over the heel and lands on the midfoot. MBT is an example of a ready-made shoe that offers this feature. It has both the delayed heel contact and SACH heel to absorb forces. The insole has to provide added protection.

A horseshoe-designed device with dispersion under the calcaneus using a combination of memory foam (PPT, for example) and pink Plastazote under the ulcer, and a firm frame for stability and support should help. Appropriate footgear is critical including appropriate shoe size and style, and heel height. What footwear is he wearing around the house? Maybe he’s wearing no shoes.

It is also VERY IMPORTANT to realize that computers are NOT the only devices that you need to remove sensitive information from before they leave your office. 
 
There was a recent case where Affinity Health Care paid a large fine for letting photocopy machines out the door with patient information on the hard drives in the copy machines. So the advice I am providing below is appropriate for any and all devices that may contain patient information including but not limited to...
