Boaters Need to Wear Socks Too: FL Podiatrist
Even if you aren’t the biggest fan of how your boat shoes look paired with socks, you should throw some on anyway to avoid damaging your feet. According to podiatrist Stephanie Fields, DPM, not wearing socks causes excessive sweating which, in turn, “causes the formation of blisters and the development of foot and nail fungus.”

The only thing worse than wearing shoes without socks is wearing the same socks over and over again. Per Dr. Fields, doing this can lead to “the development of fungus and other diseases”—which is not only bad for your body, but it’s rather unsightly, too!

MD Podiatrist Provides Winter Boot Buying Tips
On the surface, snow looks soft, fluffy, pristine. Trudging through the slush is anything but. That's why it's so important to invest in comfortable winter boots to help you brave the harsh elements. Go for a round and wide toe box. A deep and round toe box allows you to wear thicker socks and still give your feet some breathing room. "Your toes should be able to wiggle," says Priya Parthasarathy, DPM, a podiatrist based in Silver Spring, MD and a spokesperson for the American Podiatric Medical Association (APMA). A wide toe box with a soft flexible upper is also ideal for people with bunions.

Choose a pair with a cushioned, removable insole and a deep heel cup. An insole that's thick and cushioned will provide a lot of support for your arches and heels—a bonus for people who suffer from plantar fasciitis or have flat feet—while also keeping your feet warm. "A removable insole allows you to use custom orthotics or more supportive insoles if needed," Dr. Parthasarathy says.

AAPSM Holds Successful Conference in Cambridge, MA
American Academy of Podiatric Sports Medicine (AAPSM) members from around the world gathered in Cambridge, Massachusetts this past week to learn about every specialty that deals with podiatric sports medicine injuries. During this 4-day conference, the AAPSM honored those individuals who have made a significant impact on podiatric sports medicine. This year’s John Pagliano Golden Foot Award Recipient was Olympic gold medalist Joan Benoit Samuelson.

Other awards presented at the conference included: the AAPSM Lifetime Achievement Award presented to Gary Gordon, DPM. The Robert Barnes Distinguished Service Award was presented to the 2019 AAPSM Stand Alone Meeting Chairman Brian Fullem, DPM. The President’s Award – presented to a non-DPM for continued promotion and on-going support of advancing podiatric sports medicine went to Ludger Gerdesmeyer, MD. Outgoing AAPSM President Maggie Fournier, DPM received the presidential plaque and gavel for her years of devotion.

Failure to Provide Patient Records can Result in a HIPAA Fine - Part 1
After 22 years of HIPAA and 10 years of the HITECH Act on the books, it should be common knowledge that patients have a right of access to their medical records. Although physician practices or business associates can charge a fee for patient medical records, it must be reasonable, and the format (e.g., paper or electronic) matters. “The HIPAA Privacy Rule at 45 CFR 164.524(c)(4) permits a covered entity to charge a reasonable, cost-based fee that covers only certain limited labor, supply and postage costs that may apply in providing an individual with a copy of PHI in the form and format requested or agreed to by the individual.”
Additionally, an entity may charge a flat fee “not to exceed $6.50 per request” to avoid going through the process of calculating allowable costs for electronic copies of PHI maintained electronically. Regardless of the cost issue, which is meant to be minimal, patients do have a right of access to their own medical records, including those of an unborn fetus as part of maternal care.

5 Ways to Avoid Common Billing Mistakes - Part 5
It’s easy to make small mistakes when billing. Unfortunately, small mistakes can sometimes cost you big money. Fortunately, it might not be as difficult as you think to avoid them. Here are five ways to prevent some of the most common — and costly — billing mistakes
5. Explain your decisions - Not getting prior authorizations when payers require them for procedures and medications is another common mistake. They’re also a hassle. “Pre-authorizations are a big administrative burden. It’s one of the biggest issues for our members,” Cantrell says. “If you have enough staff, it’s ideal to have one person in charge of this.” If that’s not possible, you may need to consider reviewing and adjusting workloads.
You also need to make sure the diagnosis code and the procedure code match in order to demonstrate medical necessity to payers. If not, the claim will be refused. It’s important that your staff be well trained on coding if you want to avoid this common error. This applies to clinical staff as well as billing staff. "It’s a good idea if the billing department does in-service training so that the clinical staff has a better idea of how to code complicated cases," Lake advises. "They don’t need to know all the details, but they do need to know when to call in the billing department for clarification."

Query: Bundling a Bunionectomy and a Hallux Exostectomy
Our doctor performed a bunionectomy with 1st metatarsal osteotomy (CPT 28296) and an exostectomy of the dorsal aspect of the distal phalanx of the hallux of the same foot (CPT 28124). These two codes have an edit with an indicator of 1 meaning a 59 modifier can be appended. BCBS Nebraska denied my reconsideration stating that the decision was upheld to bundle the codes. The operative report was attached proving that the procedures were through separate incisions and the bunionectomy is at the 1st MTPJ and the exostectomy at the distal phalanx. Should these be reimbursed separately?

Response: If the exostosis was at the base of the proximal phalanx, I would agree that it would be bundled as part of the bunion procedure. However, in the way you describe your case, this in my opinion, is separate and should be paid. This is not a common part of a “bunionectomy.” The fact that a separate incision was made is not a valid point. Codes are codes and computers and even first level reviewers are not going to see past that. I would ask for a second level review or a peer-to-peer review or a medical director review. That way you can get to someone who understands what procedures you did and they may reverse this decision for you.
