PODIATRISTS AND SPORTS MEDICINE
CT Podiatrist Discusses Stress Fractures

According to podiatrist Robert Weiss, DPM, "We have been treating a large number of individuals with stress fractures in the past few months. We often find metatarsal-toe bone fractures from walking city streets on the hard surfaces, stubbing toes, but it is more common in sports. In the non-athlete, stress fractures may be caused by a number of factors. Overuse or strain in the foot can cause a fatigue fracture which is very common. Weak bones, caused by medical conditions such as osteoporosis, may result in another type of fracture."

Dr. Robert Weiss

"The use of steroid medication or an increased activity level can also lead to stress fractures. Tell-tale signs to look for include pain that increases with activity and diminishes with rest, pain that increases over time, or pain that persists even at rest. Many times, the stress fracture will have swelling over the affected area with a spot that is painful to the touch. Stress fractures may not show up for a number of weeks, and therefore, may not show in an x-ray. In some cases, an MRI, ultrasound, or bone scan will help make the definitive diagnosis. A stress factor should be treated with rest, ice, and OTC anti-inflammatory medications," says Dr. Weiss.

Dr. Nicholas Hugentobler, who owns eight Animas Foot & Ankle clinics in Colorado, Utah, and New Mexico, bought the practice in Loveland. Scott Taylor, DPM, now practices at the clinic.

PRACTICE MANAGEMENT TIP OF THE DAY
3 Low Tech Ways to Boost Collections - Part 1

1. Keep it simple - Everyone wants to be able to make sense of their medical bills, regardless of how they are delivered. “Medical bills can be confusing, even daunting,” says Brennan Cantrell, commercial health insurance strategist for the American Academy of Family Physicians (AAFP). Bills don’t have to be overwhelming, though.

A little tweaking can make them easy to understand. The best way to update your invoices is to strip them down to the basics: balance, date of service, due date, and a number to call with questions. “If the bill is too hard to understand, the patient is more likely to put it aside to deal with later,” Cantrell says. By putting only the necessary information on the bill, you make it easier for patients to pay—and that means they’re more likely to pay promptly.

CODINGLINE CORNER
Query: Billing for Injection

We tried to bill under Medicare, but only the CPT 20550 is denied. What is it missing here?

Line 1-CPT 99213 with -25
Line 2 -CPT 20550 with -LT
Line 3 – CPT 11721 with -59 and -Q9
Line 4 – J3301

Kiss Woo, Dr Wendy Wu’s staff, Monterey Park, CA

Response: This is a good example of why using the Medicare Correct Coding Initiative (CCI) Edits is so important when submitting multiple procedures on a Medicare patient. There is no Medicare CCI edit pairing CPT 11721 and CPT 20550. Therefore, when both of these are performed on a Medicare patient, no -59 or “X” modifier is needed. I suggest re-submitting without the -59 modifier.

This is a rare scenario where the coding and modifier use is different for a Medicare patient than it is for a non-Medicare patient and why it is so important to use the Medicare CCI Edits.

QUERIES (NON-CLINICAL)
Query: In-room vs. Virtual Scribes

I have a question for those of you DPMs who are using either in-room or virtual scribes. What are the pros and cons of each? Which would be the easiest to implement? I have been looking into virtual scribes, but am not sure which would be the most satisfactory.

RESPONSES/COMMENTS (PM EDITORIALS)
From: Ira Baum, DPM

This topic has been discussed for years. All the research and marketing, if history is any indication, will most likely do nothing to change the public’s opinion that our profession has pigeon-holed ourselves into. The current condition was not purposefully done, but we proudly changed our title because our training, and the services we provided dictated the necessity of a change.

With advances in post-graduate training, notwithstanding many other specialties involved with foot and ankle care, in order to resolve the public’s identity crisis to identify the terms podiatrist, podiatric physician, podiatric surgeon, and foot and ankle surgeon, something other than a public relations and marketing program will fall short of how we see ourselves.

RESPONSES/COMMENTS (PM ARTICLES)
From: Steven Selby Blanken, DPM

I am the grandson of a podiatrist/chiropodist (M.L. Selby, DPM), a TUSPM grad of 1990, and have a son, Brandon Selby Blanken, TUSPM Class of 2020. We were and are proud to be podiatrists. I am very happy my son picked the field I am in and hopefully he will join me in around 5 years. As SGA acting president at TUSPM, I find my son very active in the affairs of our profession along with student recruitment.

There is now a push for all students at all the schools to go visit their undergraduate schools and educate them about our field. I have been a mentor for years and helped recruit over 10 students to podiatry schools. None of them are mad at me for it. You see, if you go out and learn as much as you can, you can be successful in our field. Picking podiatry means that...

YOU CAN'T MAKE THESE THINGS UP
RE: Bedside Manners

Many years ago, I made a house call on an old woman with a wicked sense of humor. When I knocked on the door, she told me to come right in and when I got there, she called out that she was in the bedroom and asked if I could care for her in there. No problem, as I sat at the foot of the bed and started working. As I worked, her phone rang. She picked it up and said "I can't talk to you now. I have a man in my bed" and hung up as I doubled over laughing.
