NY Podiatrist Discusses Prevention of Long Distance Running Injuries
When running in marathons or half-marathons, sometimes injury is inevitable. Podiatrists say even the most experienced and well-prepared runners can hurt themselves. Experts say you can avoid foot pain altogether by training slowly and practicing prevention. "Warm up before you run, cool down when you're done. Those are really important things to do. Be careful of pain, any foot pain or signs of injury: swelling, redness or pain, make sure you see someone to get it taken care of before you continue running and do more damage," said podiatrist Dr. Robert A. Russo.

| HIPAA UPDATE | |
New HIPAA Rules Pose Complex Challenges for Providers
Safeguarding patients' personal health information has become a more complicated job—and potentially more punitive—thanks to a raft of new federal rules going into effect this week for healthcare companies and an untold number of their subcontractors. One particularly vexing provision requires providers to honor requests from patients to withhold sensitive records from insurance companies if the bills are paid out of pocket.
Healthcare providers say most of the provisions governing data privacy and security in the 563-page Omnibus HIPAA Final Rule are workable, commendable even. But a few areas will create headaches for years to come, including bigger penalties, a strong push for systemwide data encryption, and drafting contracts assigning new liabilities to hospitals' “business associates,” which now include contractors and subcontractors.

Richard Willner: It is not the same as you see on TV. You are not innocent until proven guilty. You are guilty until you are proven innocent. Administrative law (hospital rules, or what what I call Voodoo law) does not contain all the safeguards that civil law has. Rumors and hearsay are fine in administrative law. If a doctor is labeled as a disruptive physician, he or she needs to obtain legal help at once, as it's the ability to practice that is in jeopardy, not just money (like in a malpractice case).

What do you recommend for removing sensitive patient information from an office computer before giving it away?

Editor's comment: PM News does not provide legal advice. The key is to remove and manually destroy the hard drive. The same procedure applies to giving away or disposing of copy machines, which also save sensitive information on a hard drive.

Query: Billing Service for TRAKnet
We presently use TRAKnet, but we have farmed out our billing to a billing service. The problem is that we are not able to integrate the billing company's statements and billing records with TRAKnet. While I have been happy with my billing company, I really cannot keep good track of ledgers and what insurance companies are doing on a patient by patient basis. I have two choices: 1) switch software to MDsuite which is being used by my billing company (a lot of work). 2) let TRAKnet's billing company do my billing. My questions are: 1) Does anybody have experience with MDsuite? 2) What has been your experience with the TRAKnet billing service?

Query: Fluoroscopic-Guided Injection Coding
What is the correct code for a fluoroscopic-guided injection for a stiff arthritic joint?

Response: CPT 77002 is the code for fluoroscopic guidance for an injection. Since fluoroscopic guidance isn't typically needed for a joint injection, be sure that your medical record documentation describes what it was about the joint injection in THIS patient that required the use of fluoro.
If the charge for CPT 77002 is initially denied as not being a medically necessary service, I'd recommend appealing with a copy of your notes that clearly describes why, in this patient in this joint, fluoro was needed. You may be able to get this charge paid upon appeal.

This is a photo of post-op day one surgical debridement, which was performed 9/16/13 by a local vascular specialist. No HBOT treatments were ever initiated. HBOT was deemed not indicated by two local HBOT specialists as "there was no acute ischemia" and "demarcation was complete." Four specialists are involved in this case: Vascular, plastics, podiatry, and another orthopedic doc. No other ancillary modalities were initiated.

At this point, the boy is in the hospital waiting for likely Chopart or BKA amputation. Any opinions or thoughts at this point? Particularly on level of amputation i.e., how do young Chopart amputees do functionally? The etiology at this point is considered injury to dorsalis pedis artery vs. compartment syndrome vs. a combination of both. I will continue to update on this tragic case.

My wife had knee replacement surgery about 2 years ago. Although her orders included a narcotic pain med PRN, most of her pain was managed by a combination of Lyrica and Celebrex. The orthopod said that this combination was their part of their standard post-op protocol and very effective for this surgery. I have since tried combining an NSAID with either Neurontin or Lyrica with good results. I don't do it often, but it seems to be a good alternative for a narcotic pain medicine.

I have seen this in a case of a Achilles tendon tear and over-lengthening of the Achilles tendon. He ended up with a patellar tendon weight-bearing device, and it finally healed. I also did a FHL tenotomy because of a hallux malleus, and he healed quite rapidly after that.
Can we agree that the previous treatments did not go as planned; thus, it's time to step back and ask a lot of questions and get a very good medical history. Review the blood work, check the radiology tests, do gait analysis, etc. What we know is that the patient has diabetes with multiple amputations and Achilles tenotomies. The first thing that "pops" in my mind is "What does the other foot look like?" I want to know if this patient has a revasculized leg. I think we all need to gather more information.
