CT Podiatrist Discusses Training for Marathons

"It is important to remember that the marathon really starts at the 20 mile mark and it's the last 6.2 miles that really makes or breaks you, says Dr. Robert F. Weiss, a sports podiatrist. "First time marathoners in training should run approximately 34-40 miles per week for at least 6-7 weeks before the marathon, building up to at least two, 15-18 mile runs over this period. The last two weeks should be maintained with 25-30 miles each week, and the final week prior to the marathon should be kept light, says Weiss, a member of the Medical Advisory Committee of the 1984 and 1988 Olympic Marathon Trials.

"Experienced marathoners should be running between 30-50 miles per week for the six-seven weeks prior to the marathon. There should be a long training run, at least one 20 miler, every other week in this six-seven week period before the marathon. No one should attempt to run the marathon distance during training as it could lower the body's resistance to injury and influenza. The best method to improve speed is to do speed work by entering a 5K or 10K road race on the recovery weekends. Don't think about what lies ahead; but, instead, concentrate on maintaining your pace and keeping a strong psychological attitude right to the finish line."

AT THE COLLEGES

AAPPM’s First Student Practice Management Course Launched at OCPM

Dr. Hal Ornstein and Dr. John Guiliana, of the American Academy of Podiatric Practice Management (AAPPM), helped kick off the 2009-2010 academic year at the Ohio College of Podiatric Medicine (OCPM) by speaking at the first-ever student practice management course. This historic practice management course was created through a joint effort involving the AAPPM and the OCPM. The unique first-year curriculum delivered by the AAPPM consists of monthly 2-hour lectures designed to introduce the students to the critical elements of self, patient, and practice management. The first course program provided information to help the students develop a positive mental attitude, along with vitally important time and study management skills.

"As the students progress through the years, they will continuously be exposed to information that is relevant to their stage of education," explained Dr. Hal Ornstein, chairman of the AAPPM. "In the years to come, the students will learn skills to enhance patient communication, conflict resolution, stress management, and the business administration skills needed for success in private practice," added Dr. Guiliana, a fellow and board member of the AAPPM.

“OCPM is very proud to be the first podiatric school to work with the AAPPM to introduce this new practice management course into our curriculum and Drs. Ornstein and Guiliana have gotten us off to a great start.  Practice management courses will be a major educational advantage for our students as they apply the knowledge learned into their daily lives,” said Dr. Thomas V. Melillo, president of the OCPM.

Sometimes we unintentionally take more steps than are needed to complete a task because “that’s the way it has always been done!” If you feel something takes longer than it really should, re-evaluate it by breaking it down into pieces. Visualizing it in this way can identify which steps can be eliminated to make the task more efficient.

QUERIES (CLINICAL)

Query: Intractable Plantar Keratoma Sub 2nd Metatarsal

This 67 year old female  has painful nucleated kerototic lesions sub met 2 B/L. Conservative care has consisted of debridements every 3 weeks (for over a year), orthotics, paddings and metatarsal bars.

IPK Sub 2nd Met

X-rays performed with marker show the lesions directly under the 2nd met heads. The left foot has a hammered 2nd digit, the right does not. The metatarsal parabola is normal. Can anyone give me an opinion on a selection of surgical procedures?

Your diagnosis may be correct in terms of the serpentine tract. I have used ethyl chloride sprayed on the leading edge of the tract.This usually kills the organism & it is degraded/absorbed. Despite the onset/duration, the fifth toe still appears to be involved & may be addressed as deemed appropriate. Is the 4th interspace involved?

My father, Milton Kornfeld, DPM, NYCPM Class of '50, passed away on Monday. He was a man as passionate about podiatry as any I have ever known. He loved his practice and his patients. He was one of the pioneers of minimal incision surgery and the Academy of Ambulatory Foot Surgery. He obviously instilled in me the podiatry gene as well. His generation of podiatrists are the ones who took us from obscurity to a thriving and viable medical specialty.  Dad, I will do my best to carry on your legacy.

I subscribed to LocalPodiatry.com for about six months. During this time, I tracked the number of calls paid for per month and the actual number of new patients scheduled. In addition, we questioned each new patient as to who had referred them. Interestingly, ALL of the new patients acquired through LocalPodiatry.com cited a friend or a physician as the actual source of their referral. They were already looking for our practice online but happened to click on the LocalPodiatry website.
 
The last month that I had the service, I was charged for 8 calls.  When I listened to the actual calls, I found that only 2 were valid (i.e., not hangups or solicitations). When I cancelled my service and requested that the credit be put back on my credit card, I was told that the credit could only be applied to the following month's charges.
 
I feel that this service was a big waste of advertising dollars and I would not recommend it.

RE: Barefoot Running

Barefoot running has gained some popularity because of the book Born to Run. The book is an interesting read and goes against most biomechanical training. I find it interesting how running goes in cycles. The book also promotes forefoot strike instead of heel strike. Chi running and Pose method also promote forefoot striking. As a result of the ambiguity, I decided to post a survey on our website. I expect to get about 1,000 responses over the next few months and will publish the results.

If you are a distance runner, please take the survey. I would recommend reading the books on these topics if you deal with runners. I especially found interesting that, according to the author of Born to Run, a 64 year male should be able to complete a marathon in the same time as a 19 year old.

RE: Med Students Found Misbehaving on Net

Newsday’s recent article entitled “Med Students found misbehaving on Net” should be of significant concern to all universities educating medical students. This article showed medical students using Facebook pages to post inappropriate patient information and communication, inappropriate dress and behavior, and other moral ineptitudes.

Not being involved in undergraduate medical education, I am unaware whether or not our students must sign a moral code of conduct contract prior to beginning any patient contact. I also wonder if medical ethics courses in the podiatric medical school curriculum cover Internet behavior. Certainly, these are important issues and should be proactively resolved rather than for us to read more stories about moral ineptitude in our daily papers.
