MI Podiatrist Discusses New Neuroma Surgical Techniques
 
According to Conway McLean, DPM, "One of the latest developments in the surgical treatment of neuromas is a technique which spares the nerve, instead attempting to minimize the physical pressure on the nerve. This method is associated with an extremely low complication rate in the small studies to date. This is not surprising since the incision is quite small and the nerve isn’t traumatized. The ligament which connects the two metatarsal bones is cut, removing one of the three structures trapping the nerve."

"Other concepts and methods are being investigated in an effort to make neuroma surgery safer. One such concept involves destruction of the nerve through the application of high intensity radio waves. These have the ability to shrink up soft tissues. This again does not need a large incision, minimizing the degree of trauma, easing post-operative pain. Further studies are needed to better define the success rate and possible complications," says Dr. McLean.

VA Podiatrist Discusses Skating Mechanics and Custom Shoes with Olympian
 
Ben Pearl, DPM had former Olympic speed skater KC Boutiette on his podcast, Fit Foot U, ahead of KC’s return to American Ninja Warrior. Pearl described KC as the Neil Armstrong of transitioning from inline skating to Olympic speed skating. Here is a short video clip from the podcast:

KC was a pioneer technician and tactician in his sport. He popularized the down-start position for skating, which he brought over from inline skating. KC became a custom shoe fitter after retiring from skating and said that pursuit began with tinkering with the set-up of his own skate blades. KC also designs custom biking shoes as well as unique designer footwear.

I cannot understand how a dialog with ACGME can be bad. It does not commit us to anything. It would allow EVERYONE in our profession to voice their opinion. It would allow the rest of the medical profession the opportunity to learn more about podiatry. Rejecting the offer to dialog makes it appear that we do not have confidence in the value of our profession and are afraid to enter mainstream medicine when we are given the chance. Hopefully, those in power will reconsider.

I'm no expert on ACOs, however after being in practice for over 30 years I can say that alphabet solutions to the financial side of healthcare have failed every step of the way. I'm talking about HMOs, PPOs, PHOs, MA (Medicare Advantage), MIPS/MACRA, and now ACOs. One key feature of an ACO is Quality Performance Measures, AKA pay-for-performance, where providers must meet specific quality standards to earn back withheld payments. Sound familiar? To me, it sounds very similar to MIPS/MACRA whereby providers meet specific measures in order to receive a bonus in their reimbursements. Well, we know how that has turned out. Originally, we were told that we could receive up to a 14% bonus but at the end of the day, it rarely exceeds 2% and for providers that do not participate, they receive a penalty.  
 
A colleague recently sent me an incentive update from a large ACO that read in part, "Our revenue is recognized as it is received during a calendar year, while reflecting the performance from the years prior. Unfortunately, the revenues received in 2024 were not sufficient to cover our program expenses and pay an incentive." All that work for nothing. ACOs which are part of a value-based system are supposed to reward "value over volume". Does anyone really think that providers are going to be paid more with less volume? They're setting us up to fail again. This will be a system designed to pay us even less than we are receiving now. As the saying goes, "the definition of insanity is doing the same thing over and over again and expecting different results."
