Many Misconceptions in Chronic Wound Care: MI Podiatrist
According to Conway McLean, DPM, "The recommendation that a wound needs to “breathe” and should therefore be left uncovered remains common despite the fact that this notion was dispelled more than 50 years ago. Actually, the exact opposite is true. And there is no debate on the topic. We know for a fact skin injuries heal better when in a moist medium."

"The explanation is really quite logical. Every cell inside our body, meaning those covered by skin, are in a moist, watery setting and require this for health. A wound means some of these tissues, ones that should be covered by our skin, are exposed to the outside. Exposure to air will dry the living cells so revealed, causing them to sicken and die. Not conducive to reproducing, which is necessary to build new tissue so that the defect can heal, which occurs when it becomes covered by skin," says Dr. McLean.

NM Podiatrist Participates in Fall Risk Screening Event
When Irene Hobbs saw a flyer hanging at the Highland Senior Center promoting an upcoming fall risk assessment screening event, it came at the right time. Two weeks earlier, the 78-year-old fell in her backyard. She wasn’t hurt, but the ordeal left her concerned about what could happen the next time. So, she decided to stop by the event on Friday morning at the North Domingo Baca Multicultural Center.

At the event, she met with Janet Simon, DPM, a podiatrist with the New Mexico Foot and Ankle Institute, who performed an examination and sensitivity test to check the feeling in her feet. Afterward, Dr. Simon provided her with a full report on the condition of her feet, and Hobbs spoke with medical students about the layout of her house and ways to identify fall risks.

Collagenase Clostridium Histolyticum May Benefit Plantar Fibromatosis Treatment
Collagenase clostridium histolyticum may improve nodular hardness, nodular consistency, and investigator assessment in patients with plantar fibromatosis. In addition, collagenase clostridium histolyticum (Xiaflex, Endo Inc.) also trended toward greater pain reduction.

“The safety profile of the [collagenase clostridium histolyticum] CCH in this study was consistent with the known safety profile of CCH,” Jason Levy, DPM, of DMI Research, said in his presentation at the American Orthopaedic Foot & Ankle Society Annual Meeting. “The results warrant further investigation, and phase 3 trials are currently underway.”

Preventing Malpractice Lawsuits: The A-V-O-I-D Strategy
To proactively reduce the likelihood of a lawsuit, physicians can adopt a strategic approach embodied in the acronym A-V-O-I-D, originally developed by Laura Fortner, MD, an OB/GYN and Medical Malpractice Coach.
D: Document, document, document
Thorough documentation is the foundation of a strong legal defense in the event of a malpractice claim. Medical records should be detailed, accurate, and completed promptly. Physicians should document their clinical reasoning, the facts of the case, and the rationale for their decisions, ensuring that the medical record reflects a clear and comprehensive account of the patient’s care.
Avoid relying solely on electronic health record templates, as these can lead to omissions or inconsistencies. Instead, personalize entries to capture the unique aspects of each case. It is essential to record factual, objective information, including follow-up care plans and patient interactions. Physicians should refrain from using the medical record to document disagreements with colleagues or to include information unrelated to patient care. Additionally, backdating entries or creating records that appear self-serving can severely undermine the credibility of the documentation in a legal context.

RE: The Passing of Eric Carl Bruns, DPM
Dr. Eric Carl Bruns, age 62, of Lake Geneva, Wisconsin, died on June 18, 2024 peacefully in his sleep. He graduated from the Dr. William M. Scholl College of Podiatric Medicine in 1989.

He was a resident of Lake Geneva, WI since the early '90s when he opened his podiatry practice E. Bruns Foot and Ankle from which he retired this previous March 2024.

QUERIES (BIOMECHANICS) |
Query: Determining the Highest Point of the Arch (HPA)
I would like some to hear comments regarding where the highest point of the arch (HPA) exists relative to the heel and forefoot. I take it that it would be where the navicular tuberosity lies, but this may not be the case; does anyone know this? Now I realize that there is an infinite variation of the actual height of the arch at HPA, but where is this? If you were to draw a line from the medial 1st met head to the medial heel on a footprint (say a Harris mat), where (approximately) would be the HPA? I have had no success in finding biomechanical literature on this subject. Is there some formula one might adapt from autopsy findings?
