PODIATRISTS AND FOOTWEAR |
Wearing High Heels Can Reduce Fat Pads: NY Podiatrist
"The reason heels are bad is because when you are in any kind of shoe that has elevation or a heel, your weight gets shifted forward to the ball of the foot," Jackie Sutera, DPM, a podiatric surgeon at City Podiatry in NYC, tells SELF. "The higher the heel, the more weight and pressure get shifted forward. Your knees and hips then have to push forward and your back has to hyperextend backwards to counterbalance," she explains. This can translate to leg, hip, and back pain. "It misaligns the whole skeleton and that’s in a nutshell why it's really bad for you."
|
Dr. Jacqueline Sutera |
"What ends up happening with overuse is the fat pad on the bottom of the foot starts to become a lot thinner over time," Sutera says. "When you don’t have a natural cushion anymore, you can get generalized pain on the bottom of the foot." This pain is called metatarsalgia. The rate at which these fat pads atrophy depends on the person, but over-wearing high heels and even walking around in crappy flip-flops can contribute to the process over time.

INTERNATIONAL PODIATRISTS IN THE NEWS |
UK Podiatrist Develops a Unique Footprint Measurement Tool
Footprints at a crime scene provide evidence that can be as crucial as fingerprints and DNA in catching and convicting culprits. But exceptional expertise is required for them to be interpreted correctly and then stand up in court. This is the realm of forensic podiatry – taught to a high level at the University of Huddersfield. ?It is the only UK university that offers a Master’s course in the subject, led by one of the world leaders in the discipline, Professor Wesley Vernon. Now, it has a senior lecturer whose own research has led to the development of an advanced new system of footprint measurement at crime scenes.
|
Dr. Sarah Reel |
It has been dubbed the Reel Method and is beginning to be employed by forensic podiatrists and disseminated in specialist publications. It takes its name from the University of Huddersfield’s Dr. Sarah Reel, who developed the technique during her PhD studies, for a thesis titled "Development and Evaluation of a Valid and Reliable Footprint Measurement Approach in Forensic Identification".

QUERIES (CLINICAL) |
Query: 7 Year Old with Crossover Toes
|
Crossover toes in 7 Year Old |
This is a 7 year old child with asymptomatic, cross-over toes. What are the best treatment options, if any?

CODINGLINE CORNER |
Query: Multi-Layered Compression Dressing
When billing CPT 29581 for a multi-layered compression dressing system, does anyone bill for the supplies as well (e.g., "A" codes) or have those typically been included in the payment for the application?
Mike King, DPM, Alpharetta, GA
Response: The costs of the supplies are included in the allowance for the application procedure and not separately.
The key to using CPT 29581 is that you are applying a multi-layer compression SYSTEM. There are specific items that are expected to be used, if you don't purchase pre-packaged kits. While it would be cheaper to put together your own layer materials (same as in the kits), the systems kits are more convenient.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B |
From: Robert Wunderlich, DPM
I have a patient who wears a size 17, and I was able to fit him into a Darco Body Armor Walker II. This was the only walking boot I could find that would fit him. Gill carries them in both a high and low model. If that doesn't work, another option would be to put him in a short-leg walking cast, or perhaps a slipper cast.
Robert Wunderlich, DPM, San Antonio, TX

RESPONSES/COMMENTS (EMR) |
From: Pamela Bobrow
I saw Dr. Borreggine's posting in PM News and thought this FAQ from CMS would be helpful. Here is the FAQ from the CMS website that talks about the alternate exclusions:
At the bottom of the FAQ, it states: An Alternate Exclusion may only be claimed for up to two measures; then the provider must either attest to or meet the exclusion requirements for the remaining measure described in 495.22 (e)(10)(i)(C).

YOU CAN'T MAKE THESE THINGS UP |
RE: Mrs. Malaprops Returns
When asked what her chief complaint was, the patient responded that she had "right ankle pain." She said that she went to the ER and had x-rays and she was told that she had "bone spears" in her ankle. I just wonder if you can bill that as a foreign body? Oh what patients say!
Rich Hofacker, DPM, Akron, OH
