PODIATRISTS AND SPORTS MEDICINE
Warming Up Prior to Sports Activity Can Reduce Injuries: CA Podiatrist

“Foot and ankle injuries are common in sports, especially football, cross-country, tennis, soccer, and volleyball. I see many high school athletes throughout their competitive seasons who have suffered from ankle sprains, stress fractures, and various tendinopathies,” said L. Marie Keplinger, DPM, a podiatric surgeon practicing in Fullerton, CA.

Dr. L. Marie Keplinger

“These injuries can sideline an athlete for all or part of a season, depending on the severity. Athletes and sports enthusiasts alike can decrease the risk of injury by taking some simple precautions,” Dr. Keplinger continued. Each sport carries its own inherent risks, but if these simple precautions such as as warming up prior to any sports activity are followed, an athlete can reduce his/her chance of becoming injured and losing valuable playing time.

INTERNATIONAL PODIATRISTS IN THE NEWS
UK Podiatrist Discusses Treatment of Plantar Warts

“A verruca is a wart on the foot, usually found on the sole. They can be extremely painful and are caused by a virus that is very contagious, often transmitted from person to person by walking barefoot on a damp surface,” explains Dr. Tariq Khan, Consultant Podiatrist at Carnation Footcare. Standard verruca treatments help to remove the verruca but unfortunately offer very little in the way of comfort while the removal process takes place.

Dr. Tariq Khan

An effective self-treatment method comes in the form of Verruca Care. Four medicated plasters containing salicylic acid effectively and gently remove the verruca, while a felt ring relieves pressure and cushions the tender area surrounding the verruca while the gradual and thorough removal process takes place.

PODIATRISTS AND DIABETES
AZ Podiatrist Demonstrates Blood Glucose App for iPhones

A new device and app are taking blood glucose testing into the realm of iPhones and iPads. Yes, there's an app for that. The iBGStar® is a blood glucose meter that plugs in to your iPhone, iPod Touch, or iPad. It's designed specifically "to easily display, manage, and communicate your diabetes information." Dr. Richard Jacoby of Scottsdale Neuropathy showed 3TV's Stella Inger how it works.

Dr. Richard Jacoby

Jacoby said the iBGStar is particularly good for people with pre-diabetes, a condition affecting 79 million people. "Fifty percent of people over age 65 are pre-diabetic," Jacoby added. "The important thing about this is that it logs the amount of sugar you have in your blood dependent on the meals that you eat," Jacoby said.

PODIATRISTS ON CAPITOL HILL
DC Podiatrist Featured Speaker at Capitol Hill Forum

John S. Steinberg, DPM was one of two featured speakers at a Capitol Hill Forum on Advanced Wound Care Policy. The event was held at the Congressional Rayburn House Office Building in Washington DC and was organized by AdvaMed’s Wound Healing and Tissue Regeneration Sector to inform members of Congress and their staff about the growing public health concerns of problem wounds. In the United States, chronic wounds affect 6 million patients and cost healthcare systems an estimated $20 billion annually.

(L-R) Drs. John Steinberg and William Ennis with DeChane Dorsey, VP, Payment and Health Care Delivery Policy at AdvaMed

Dr. Steinberg, a podiatric surgeon, is an Associate Professor in the Department of Plastic Surgery at Georgetown University School of Medicine in Washington DC. “This was a great opportunity for congressional leaders to hear about the team approach to limb salvage and the need for access to advanced wound care options.” The audience included congressional staff, U.S. Department of Health and Human Services staff, and also leadership from the American Association of Orthopaedic Surgeons, American College of Surgeons, Association of American Medical Colleges, American Academy of Nursing, and the American Association of People with Disabilities.

ON THE INTERNATIONAL LECTURE CIRCIT
AZ Podiatrist Keynotes China Forum

Dr. David G. Armstrong, Professor of Surgery and Director of the University of Arizona's Southern Arizona Limb Salvage Alliance (SALSA) delivered a keynote address at the China National Forum on the Diabetic Foot held in Chengdu. In it, he showed data revealing massive differences in amputation rates not only from nation to nation, but from hospital to hospital. "These huge disparities call for a unified approach to training, education, and ultimately care," noted Armstrong.

The forum also featured Dr. Robert Frykberg of the Carl T. Haden Veterans Affairs Medical Center and JP Hong, Chief of Plastic and Reconstructive Surgery at Asan Medical Center in Seoul.

The Variable Flex Technology ™ system features removable inserts in the forefoot to allow for three levels of flexibility as the patient progresses in recovery. “With clinical studies and additional testing under way, we have already seen improved recovery in patients, with patients using these shoes compared to what’s currently available. RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Dr. Seymour (Sy) Frank
From: Elliot Udell, DPM

I am saddened to hear of the passing of Dr. Sy Frank. Sy was my past professor at NYCPM, where he chaired the department of surgery and taught many courses not only in surgery, but in all facets of podiatric surgery and biomechanics. He was a staunch advocate and proponent of the late Dr. Merton Root and often lectured on Root's theories. He also made his private office on Long Island available to any student who wanted to visit and learn.

I would not be a podiatrist if it were not for Dr. Frank's influence. When I was in graduate school with hopes of becoming a biochemist, my grandfather developed a painful foot problem. It was the bane of his existence, and no doctor was able to help him. Someone recommended Sy Frank and I took him there. Sy was able to alleviate his foot problem in one visit. I was so taken by what I saw that I gave up thoughts of  becoming a biochemist and applied to podiatry school. As his student at NYCPM, I learned things directly from him that I still use today. I still remember one of my other professors saying, "if you have a clinical problem that you cannot understand, hit all of the books and journals. If you still can't figure it out, then ask Sy Frank. If he can't help you, there is no answer." To podiatry, Dr. Frank was the ultimate professor.

RESPONSES/COMMENTS (CLINICAL) - PART 1A
RE: Unstable Scar
From: Mike Boxer, DPM, Samantha E. Bark, DPM

From the picture of the "unstable scar", it appears that the patient has a chronic pre-trophic area with some intracutaneous hemorrhage. This can best be treated with a healing sandal. The healing sandal was initially developed for the treatment of patients with Hansen's disease. The healing sandal consists of about two inches of Plastizote in a specially fabricated sandal. You need to find someone in your area who knows how to make them.

Mike Boxer, DPM, Woodmere, NY, mcbdpm@aol.com

It looks like the Achilles tenotomy has caused a calcaneal gate. An ankle fusion with his foot in 5 degrees of equinus could help.

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: Thomas Graziano, DPM, MD, Gary W Docks, DPM

Consider performing a biopsy of this "scar". A chronic ulceration, particularly in an immunocompromised patient, can degenerate into a malignancy. Before considering any other treatment modality, rule out malignancy.

Thomas Graziano, DPM, MD, Clifton, NJ, TGrazi6236@aol.com

You can either continue palliative therapy and use high quality gel heel pads or insoles along with a heel lift, or a "Sach" heel built up on the shoe (send it to a CPed), or consider sending the patient to a plastic surgeon for a sural artery pedicle flap from the posterior calf. I have seen some excellent results. This is truly the only way to resolve this; otherwise, all you can hope for is keeping the heel stable with frequent palliative care.

RESPONSES/COMMENTS (CLINICAL) - PART 2
From: Michael F Byrnes, DPM

This rash appears geographically symmetrical. Consider eczematous dermatitis. Use hypo-allergenic soap products and detergents. Double rinse stockings and clothing worn over the areas. Eliminate soap products in nylons, etc., as they can activate and create allergens when combined with sweat. I use a low-dose steroid combined with Eucerin cream in a compound. Once the rash is controlled, I switch to Eucerin cream alone, as the steroid can thin the skin. Avoid products with alcohol. Remember eczema is controllable, but not always curable, so you will need to follow up. Dermatology is a big potential niche in our practice, and it grows with each successfully treated patient.

RESPONSES/COMMENTS (NON- CLINICAL)
RE: A Major PR Victory for Podiatric Medicine
From: Daniel Taubman

About three weeks ago at ACC Media Days, the press conference event that proceeds college football season, Florida State quarterback Jameis Winston made a statement. He told the hundreds of reporters assembled that he would like to become a podiatrist when he was finished with his athletic career (he also is a star for the Seminole baseball team). This was first brought to my attention through PM News. Naturally, this piqued my interest, so I made an inquiry with the FSU athletic department, asking if they would make an exception to allow Jameis to talk to us about podiatry (underclassmen at major programs generally are not allowed to talk to the “media.”) I never expected a response, but figured that I was doing my office a disservice if I didn’t at least make an attempt.

On Monday, their sports information director responded to me saying that Jameis would be happy to talk to us. Yesterday, I conducted a ten-minute phone interview with him, and the young man gave me some great stuff. I have posted the transcript of the interview to the Podiatry School 101 blog that we started way back when, and have linked it to social media. The blog can be found here.
