CA Podiatrist Discusses Use of Proper Socks to Treat Hyperhydrosis

An estimated 3 percent of the U.S. population — some 8 million people — suffer from what's known as plantar hyperhidrosis, or excessive foot sweating, according to the American Podiatric Medical Association. This type of sweaty feet is not caused by the normal causes of sweating, such as high heat or exercise, but rather it's an all-the-time issue that occurs without a culprit. This condition can be both emotionally and physically debilitating, says Najwa Javed, DPM, a podiatrist with Silicon Valley Podiatry Group and founder of E'MAR Italy.

Dr. Najwa Javed

"While no one knows what causes this condition, it seems to be inherited and can affect palms, soles, armpit, hair, legs, and back," Dr. Javed says. "When affecting the foot, it can increase foot odor and can cause an increased risk of infections — including fungal toenails." One easy remedy is to ensure that you're purchasing the proper socks — ones that are breathable and both moisture-absorbing and moisture-wicking, Dr. Javed says. If your socks aren't made with the right materials and structure, they can exacerbate sweating and even lead to blisters, she warns.

VECTTOR Machine Helps UK Boy with CRPS

Dillon Wilford went from a a ‘happy, healthy, and normal 10-year-old’, before he woke up with a limp and agonizing pain in his leg in November 2021. It wasn’t until three months later in February this year, when Dillon was diagnosed with CRPS (Complex regional pain syndrome). His family originally aimed to raise £100,000 to send Dillon to America for specialist treatment at the Spero Clinic, in Arkansas. But they have come across the VECTTOR machine, which was £5,000 which is a lot cheaper and quicker.

Within an hour, Donald Rhodes, DPM conducted tests which diagnosed Dillon with other conditions such as PoTS and adrenal fatigue. By day two, his circulation improved dramatically, and his adrenal fatigue was resolved. The family have now brought the VECTTOR machine to their home, which is expected to help manage Dillon’s pain, although he will still need to continue with physiotherapy.

RELEVANT RESEARCH
Opioid-Free, Multimodal Protocol Post Foot, Ankle Surgery Recommended

For properly selected patients undergoing outpatient foot and ankle surgery, an opioid-free, multimodal pain management pathway provided safe and effective pain control with minimal adverse events, according to presented results. “We have a crisis of opiates and, as orthopedic surgeons, we are part of that problem,” Tyler Gonzales, MD, MBA, said in his presentation at the American Orthopaedic Foot & Ankle Society Annual Meeting. “The literature – especially in foot and ankle – has moved to showing that we maybe don’t need as many narcotics as we prescribe.”

Gonzales and colleagues prospectively analyzed 33 patients who underwent elective outpatient foot and ankle surgery, including bunion correction surgery with or without a lesser toe procedure, from Dec. 1, 2020, to Feb. 9, 2022. Mean age of patients in the study was 55 years. The average follow-up was 11.5 weeks. All patients completed a comprehensive opioid-free, multimodal pain management protocol, which included oral pregabalin, acetaminophen, ketorolac and cyclobenzaprine to be administered immediately after surgery. Gonzales noted patients were given meloxicam on post-operative day 4. Outcome measures included VAS pain scores, ED visits, urgent care visits, need for pain medication, complications, and patient satisfaction. Overall, 18% of patients required opioids during the first 2 weeks post-operatively; however, no patients required opioids after 2 weeks.

PRACTICE MANAGEMENT TIP OF THE DAY
5 Tips for Hiring and Retaining Medical Practice Staff

Tip #4. Be Flexible

The COVID pandemic upended a lot of things, including how future and current employees want to work. Many people now seek remote or hybrid positions. Some roles obviously need to be in person in a medical practice. However, other functions like billing, accounts receivable, or calling insurance companies could be done remotely.

Listing an opportunity that could include remote or hybrid work could increase the number of qualified candidates who answer your job listing.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: AACPM Statement on Board Certification
From: Moraith G. North

In reference to the recent actions by the American Board of Podiatric Medicine (ABPM) to promote a certificate of added qualification (CAQ) in podiatric surgery, the American Association of Colleges of Podiatric Medicine (AACPM) believes there is one pathway to surgical certification in the podiatric profession. The Association’s Board of Directors unanimously support the policies and procedures found in the Council of Podiatric Medical Education’s (CPME) documents Criteria and Guidelines for Recognition of a Specialty Board for Podiatric Medical Practice (CPME 220), and Procedures for Recognition of a Specialty Board for Podiatric Medical Practice (CPME 230) for determining and recognizing specialty boards within the profession.

The American Association of Colleges of Podiatric Medicine (AACPM) supports its member schools and colleges, as well as its teaching hospital member institutions in our mission to serve as the leader in facilitating and promoting excellence in podiatric medical education leading to the delivery of the highest quality lower extremity healthcare to the public.

RESPONSES.COMMENTS (NON-CLINICAL) - PART 2
RE: Use Reality TV for Student Recruitment
From: Tyler B. Brahm, DPM

The APMA should have a message on the TLC cable TV show My Feet are Killing Me, stating that if you or anyone you know should consider podiatric medicine as a profession, contact the APMA. And APMA should send excerpts of the show to colleges' pre-med advisors for them to show their pre-med students. The above would be a game-changer.

RESPONSES.COMMENTS (NON-CLINICAL) - PART 3
From: Chris Robertozzi, DPM

At the 2022 APMA House of Delegates, the New Jersey Podiatric Medical Society submitted a Policy Proposition to form a healthcare alliance with all national organizations that deal with healthcare. It passed without a single dissenting vote. The alliance is not just for physicians, but hospitals, hospice, nursing homes, assisted living, physical therapy, occupational therapy, etc. There has never been an alliance of all of healthcare. Our working alone all these years has allowed us to be taken advantage of. There is strength in numbers. It has always been that different national organizations lobby for their particular issue in DC, but healthcare has never spoken as one voice. We have more in common than we do differences.

When Obamacare was in its developmental stages, the American Public Health Association volunteered to...
