 TX Podiatrist Discusses Exercises for the Feet

A routine stretch that wakes up the muscles in the feet should be done in the morning before getting out of bed, Dr. Leslie Campbell, a spokeswoman for the American Podiatric Medical Association, says. Sit with legs straight out in front of you and angle your toes toward your head and then away from it. This works the Achilles tendon and the fascial band, which connects the front and back of the foot. "Jumping straight out of bed in the morning when the muscles are cold can harm the fascial band and cause plantar fasciitis, a very common and painful inflammation of the bottom of the foot," she says.

Other exercises include rolling your foot over a tennis or golf ball, which stimulates the nerve endings in the feet and protects from injury. Picking up marbles or towels with your toes strengthens the muscles in the toes and central part of the foot. And wrapping a rubber band around the toes and then expanding the toes outward supports the major muscles of the foot, Dr. Campbell says.

NY Podiatrist Keynotes at NYCPM Whitecoat Ceremony
 
On Tuesday, September 10, the New York College of Podiatric Medicine (NYCPM) welcomed 107 students in the Class of 2017 at the College’s annual White Coat Ceremony, in which the future Doctors of Podiatric Medicine symbolically don their white coats and together recite the Hippocratic Oath. After opening remarks by President Louis L. Levine welcoming the Class of 2017, Deputy Board Chairman Rock G. Positano, DPM, MSc, MPH offered greetings to the assembled College community and guests.

Jaime Torres, DPM, Regional Director, U.S. Department of Health & Human Services, Region II, delivered the keynote address. In his speech, Dr. Torres spoke about empathy, and urged the student doctors to remember they would be treating real people behind every symptom and every diagnosis. Dr. Torres received his Doctor of Podiatric Medicine degree from NYCPM, and a Master’s degree in Community Health from Long Island University.

ON THE INTERNATIONAL LECTURE CIRCIT

DC Podiatrists Speak at DFSI Conference in India

Upon invitation of the Diabetic Foot Society of India (DFSI), Drs. John Steinberg, Vinay Matai, and Virit Butani presented at the podium this week during the 12th Annual DFSI Conference held in Jaipur, India. Topics presented by the U.S. speakers included the team approach to limb salvage, local flaps and grafts for wound closure, and oral abstracts on non -invasive imaging and 1st metatarsal ulceration. The meeting was attended by over 400 physicians from all parts of India and sparked interest in advanced limb salvage techniques and outcomes from this specialized care.

Query: Unstable Scar
 
I have a 47 year old male diabetic with multiple bouts of ulcerations as well as bilateral hallux amputations and Achilles tenotomies which were all done prior to my seeing him. He is currently healed, but has obvious fat pad loss with a chronic pre-ulcerative unstable scar. Any input on what can be done to prevent or protect the area?

I have tried CROW boots, orthotics, and viscoelastic inserts, all of which have failed. Surgical approaches can be considered as well. I have contemplated augmentation with tissue matrix, but I feel the severe atrophy would doom it to fail. He is 6'7" tall, so he is rather large-framed, but not obese.

I feel the patient may have lipodermatosclerosis. Lipodermatosclerosis describes bound-down, sclerotic, red skin involving the lower extremities. Lipodermatosclerosis is associated with venous disorders, female sex, middle age, high body mass. Compression therapy is the only treatment, and one may have to get custom stockings. Also one can limit standing for long periods and lose weight.

By looking at the picture and seeing that the redness and swelling are almost identical in location on both feet, I  suspect a case of contact dermatitis. I would question her on the types of footwear she wears and on any recent additions to her wardrobe, as well as on the possibility that she may have soaked her feet in some form of liquid that may contain an irritant, causing this pattern.

Have you considered the possibility of venous insufficiency? A detailed duplex ultrasound of the great saphenous veins bilaterally, as well as the deep veins, would rule in or out this diagnosis. If positive, she would likely find relief in an endovenous ablation. You noted that she has tried support stockings with no success; however, patients with venous insufficiency often fail conservative treatments such as support stockings. An appropriate referral to a local vein specialist, which can be found at phlebology.org/ may benefit the patient. Lower extremity venous valvular incompetence is commonly overlooked in edema work-ups. Venous ablation is a simple outpatient procedure covered by most major insurances that can be done in a relatively short amount of time, and has been found to greatly reduce lower extremity edema and improve overall quality of life.

Our practice of 4 DPMs with 3 offices have been using Medicall by TelTech systems, Inc. It works extremely well with TRAKnet as well as other systems. We have been long-time users of Medicall prior to TRAKnet and have been using it for over 3 years with TRAKnet. We have very few "no shows" and the patients have come to appreciate the reminder.
 
Prior to Medicall, it took one employee 3 hours each day to confirm appointments. That was 15 hours/week. Once the system is set up, it takes 2 minutes each day. We have set up the system to tell new patients and established patients who have not been in the office recently to come in 20 minutes early to update paperwork.
