OH Podiatrist Discusses Toe Spacers and Separators
 
Podiatrist Georgeanne Botek, DPM recently talked about toe spacers and separators and what they can — and can’t — do for you. Dr. Botek says choosing a toe spacer or toe separator comes down to comfort and the problem you’re looking to fix. “In general, I like the pads that are made of silicone because they’re spongy but keep their shape well. Most of the time, those are reusable, too, so you’re not throwing them out every time you use them,” Dr. Botek says. “I also usually recommend an open-toe pad. Anything that closes over your toes won’t let your toes breathe as much.”

The thing to remember is that most toe spacers aren’t going to reverse any damage that has been done, Dr. Botek notes. The exception: A specialized device that’s worn outside of shoes, while resting your feet. A common example is the brand Yoga Toes®. “Toe spacers that are meant to be worn in your shoes are like eyeglasses,” Dr. Botek says. “These devices will work while they’re on, but they’re not correcting a problem at its root. When you take your glasses off, your vision hasn’t changed. It’s the same with toe separators and spacers. They won’t have a lasting effect on your feet.”

DOJ Joins Lawsuit Alleging Kickbacks and Fraud Against Arizona Medical Clinics 
 
A national chain of vascular clinics based in the Phoenix area was accused in a federal civil complaint of paying illegal kickbacks and performing unnecessary medical procedures. In court documents unsealed by a judge Friday, two outside doctors made claims against Modern Vascular and its constellation of related entities. Among them was that Modern Vascular founder and board chairman Yury Gampel pressured doctors at the clinics to “perform invasive procedures on as many referred patients as possible,” and that Gampel ensured the clinics treated patients “according to the procedures that received the highest Medicare reimbursement,” without regard to medical necessity, charging as much as $17,000 to $30,000 per procedure.
 
The complaint against Gampel focuses in part on claims about his actions when opening a Modern Vascular clinic in San Antonio in 2019. It says he held a conference call that included 16 podiatrists in the area and explained the terms, promising the investment was legal and $15,000 would turn into more than $1 million in less than five years.

5 Tips for Hiring and Retaining Medical Practice Staff
 
Tip #2. Be Thorough 
 
Everyone’s busy, and it could be tempting to shortcut some of the due diligence for screening potential candidates. However, taking the time to verify dates and places of employment and reach out to references can be worthwhile. Not everyone is completely honest on their résumés, and it’s better to find out ahead of time. 
 
Once you’ve narrowed down the candidates, arrange brief telephone interviews to screen them further and gauge their attitude and enthusiasm. If you’re still unsure, ask them to come in for an hour or two to “shadow” an existing staff member. That will give you and them a better idea if the fit is right. You can also try role playing — have someone in your office act as the patient and see how the candidates react to different situations.

It is my observation that all the healthcare professions have to face challenges and have overcome them. However, I do believe that these challenges have added significant weight for all professionals to bear. Most recently, there was a suggestion for podiatric physicians to strike. I performed a historical literature search using the Mesh terms: “Podiatric” and “physicians”, and strike” without results. I refined my search and found: Thompson SL, Salmon JW. Physician strikes. Chest. 2014 Nov;146(5):1369-1374. These authors’ state: “Although physicians have not historically used collective bargaining or the strike weapon, the rapidly changing practice environment in the United States might push physicians and other healthcare professionals toward unionization.”
 
The idea of hospital-privileged providers to strike arose in 2001 just when I was first privileged at a community hospital. Some members did indeed strike and traveled to my state capitol, marched, and formed a picket line. No resolution or solution was obtained with their endeavor. I was among the minority who stayed at the hospital and performed my duties. I will defer to our profession’s leadership to decide their means to meet the obvious frustration of colleagues and expect our leadership to answer in kind to these questions.

I’m glad to hear that Dr. Roth has had success with the MASS theory of foot orthotic therapy. I have spoken to other podiatrists who claim to have had success with it and others who did not experience adequate success and abandoned the MASS approach as a result. I have also spoken to podiatrists who have had tremendous success with Root’s approach and others who have had less than satisfactory results. How can one explain these contrasting experiences and outcomes? Part of the answer to that question is the fact that there is variability between and how clinicians examine a patient, cast or scan the foot, and in the devices that they order for their patients. In addition there is variability between custom foot orthotics made by different manufacturers who, in many cases, claim to subscribe to the same manufacturing theories and protocols.

Be careful what you ask for in payment for orthotics from insurance companies. Many of us remember when we were appropriately reimbursed for bunion/hallux valgus surgery $2,000. +/- as compared to the current reimbursement rate with the 90-day follow-up. That is what happens when insurance companies control free enterprise with participation contracts and make us providers for their insureds. This past week, I counted 19 advertisements on Facebook for all different kinds of orthotics to cure from plantar fasciitis to low back pain from $39.00 to $250.00. This is a sad state of affairs, creating confusion for the general public.
