PODIATRISTS IN THE NEWS
Team-Based Approach Prevents Clinician Burnout:  MN Podiatrist

Clinician burnout has been an issue for years, and is a complex, systemic problem. Evidence tells us that it decreases the quality of care and damages the patient experience. Fixing it will take more than just improving the electronic health record (EHR). Mankato Clinic (Mankato, Minnesota, U.S.A.) addressed this problem with a new approach to care. The organization recognized that the requirements placed on clinicians have increased, and it implemented team-based care to spread the workload and ease frustration.

Dr. Andrew Lundquist

A recent case study shares Mankato Clinic's journey with team-based care. Here are some of the lessons learned from undertaking this organizational shift: Build from the ground up. “We've learned that a team-based approach has to come from the ground up,” Chief Medical Officer Andrew Lundquist, DPM said. “You have to look at processes, workflows, and interactions for everyone – from schedulers, to nurses, to physicians, to patient services – everyone has to work in a little different way.”

PODIATRISTS AND BIOMECHANICS
OH Podiatrist Comments on Orthotics and Heel Pain Study

A recent study in the British Journal of Sports Medicine took a look at treatments for a common heel pain called plantar fasciitis. The study found that stretching is often the most effective way to reduce plantar faciitis pain. "It's all about stretching, releasing the tension on your calf and the plantar facia," said Darren Di Iulio, DPM, podiatrist at St. Elizabeth Healthcare.

Dr. Darren Di Iulio

In the study, researchers discovered in the study that the orthotics work best combined with other treatments such as physical therapy, which teaches you how to stretch. "The second thing that the study showed is that not every patient needs a custom, super-expensive rolls-royce orthotic," Dr. Di Iulio said. "Most can use an over-the-counter orthotic that will help them just as much as the custom orthotic."

New Bridge Medical Center Welcomes NJ Podiatrist

New Bridge Medical Center (NBMC) has welcomed several new physicians from Rutgers New Jersey Medical School to their Ambulatory Care Center. Among this is Zachary Criswell, DPM, a fellowship trained foot and ankle surgeon. Dr. Criswell received his DPM degree from the Temple University School of Podiatric Medicine in Philadelphia. He then went on to complete his residency in podiatric surgery at University Hospital in Newark, NJ.

Next, he completed a one-year surgical fellowship through the Palo Alto Medical Foundation in Santa Cruz, California, with emphasis on lower extremity sports medicine and arthroscopy. He is board qualified through the American Board of Foot and Ankle Surgeons.

PODIATRISTS AND FOOTWEAR
NY Podiatrist Provides Shoe-Buying Advice

“Not every shoe within a brand is going to be right for you,” says Lori Weisenfeld, DPM, a sports podiatrist. “Generally, people with a flat foot need a supportive shoe and less cushioning,” she said. “If somebody has a very high-arched foot, they tend to need more cushioning.”

Dr. Lori Weisenfeld

If you want shoes for foot pain that will keep you comfortable while walking around town, running errands, or doing jobs that require you to be on your feet, avoid overly flat shoes. “For spending a lot of time on your feet you want it to have a little bit of heel rise, or you can add a heel lift into a very flat shoe,” Weisenfeld says. If you need more arch support, a well-made insole, even if it’s inside an inexpensive shoe, can go a long way.

PRACTICE MANAGEMENT TIP OF THE DAY
Get Your Website to the Top of Google Rankings — and Stay There - Part 5

5. Include the right keywords

Once you have identified the suitable keywords, it is important to incorporate them effectively into your marketing strategy. Every webpage and blog post should include those keywords in your content so search engines can find your practice. Hashtagging those keywords in social media posts can also improve your findability on those platforms, increase your audience, and cement your status as a leading medical expert. As an added bonus, public social media accounts can also find their way into Google search results.

CODINGLINE CORNER
Query: Nail Debridement Codes

This is what we tried to bill to Medicare, but G0127 didn’t get paid. May I ask what is the correct way to bill this?

Line 1 – 11720 with modifier 59 and Q8
Line 2 – G0127 with modifier 51

Kiss Woo, Dr Wendy Wu’s staff, Monterey Park, CA

Response: It is appropriate to bill these two codes together, however, NEVER use the 51 modifier for anything when billing Medicare; that alone will get your claimed kicked out. Subsequent claims will automatically be reduced in fee.

When billing this combo of codes, CPT 11720 is appended with the 59 modifier not HCPCS G0127. If the patient qualifies for the Q8 modifier, it should be appended to both codes.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Podiatry Outpatient Infection Guides from the Joint Commission and the CDC
From: Kevin McDonald, DPM

Podiatrists should do as much as possible to prevent the spread of infections in their offices.

The Centers for Disease Control and the Joint Commission have recently completed guidelines and a manual to prevent the spread of infection in outpatient podiatry settings. This Manual and Pocket Guide represent the most current and evidence-based knowledge and procedures in infection control for podiatry offices. Every practicing podiatrist should consider this information vital and applicable.

The Guide to Infection Prevention in Outpatient Podiatry Settings and companion pocket guide are available on the CDC and Joint Commission websites: CDC webpage Joint Commission webpage.

RESPONSES/COMMENTS (PM ARTICLES)
From: Gary S Smith, DPM

Student recruitment is becoming a problem in all medical schools. Schools and even the medical professions as a whole are being forced to compete for students. Over the last few years, the medical profession in general has made it easier to practice in several ways like loosening restrictions on board certification and training. They have made it easier to see more patients for less money by giving PAs more privileges. At the same time, podiatry has made board certification, state licensure, and hospital privileges more difficult. Our profession, in the name of parity, has hurled towards disparity.

My son is applying to medical school right now. I would love to have him take over my practice someday, but I think it's silly to do a three-year residency to become a podiatrist with such a limited license. It just doesn't make sense financially. The same three years would allow you to be a general, orthopedic, or OBGYN surgeon. After our 7 years of medical school and residency, we have PAs with two years of medical training look down their noses at us. Podiatry was much more marketable when graduates could choose to be a podiatrist or a podiatric surgeon. There was a time when people going to podiatry school wanted to be a podiatrist and chose this profession for what it once was. The three-year mandatory residency has killed our profession.
