You use your feet every day. But despite their importance, you may not think about them much unless there’s a problem. That’s where podiatrists come in. A podiatrist isn’t the only clinician who can treat foot problems; your primary care physician, a physical therapist or an orthopedist may also be helpful. But podiatrists are definitely the most comprehensive.

SC Podiatrist Named "Best Podiatrist" by Post and Courier Readers
Ralph S. Sprinkle, DPM of Pawleys Podiatry has been named "Best Podiatrist" by the readers of The Post and Courier. Dr. Sprinkle was born and raised in Winston-Salem, North Carolina where he was one of six sons of well known podiatrist, Dr. Robert L. Sprinkle. Growing up passionate about foot health, Ralph decided to follow in his father’s footsteps and become a Doctor of Podiatric Medicine. In addition, two other brothers also became podiatrists.

OH Podiatrist Explains Why Shoe Size Changes
You’ve worn size 9 shoes for as long as you can remember. They always fit perfectly — and then one day, they just inexplicably don’t. Try as you might, you can’t cram a foot into that familiar size. First, relax. You’re not going crazy. Second, it’s natural for your feet to gradually change in length and width over time. Podiatrist Nicole Nicolosi, DPM explains how this happens. Most people gain weight as they get older — and those extra pounds press down on the ligaments and tendons that support your foot.

As a result, your foot can widen or lengthen. “It adds up over time,” says Dr. Nicolosi. With every step you take, your body absorbs two to three times its weight due to gravity’s unrelenting pull. That takes a toll on your feet — so much so that it actually changes their shape. “The ligaments and tendons that support the arch in your foot become worn from use,” explains Dr. Nicolosi. “As that happens, the arch slowly collapses — and that flattening results in your foot getting a bit longer or wider.”

ACFAP Annual Meeting a Success
The American College of Foot & Ankle Pediatrics (ACFAP) hosted its 7th Annual International Pediatric lower extremity conference at Badlands National Park on September 15-17, 2022. The in-person conference featured tours to Mount Rushmore, Crazy Horse Memorial, and Badlands National Park The scientific portion featured exceptional CME lectures along with pediatric workshops featuring cadaveric surgical training, biomechanics, clubfoot casting, and lower leg bracing.

5 Tips for Hiring and Retaining Medical Practice Staff
Tip #1. Be Strategic
Put the time into finding qualified candidates — don’t just post the position to a job site and hope the right people will come forward. Also, think outside traditional job postings when appropriate. For example, if you’re looking for front desk people, they may not need extensive medical office experience. Consider people with a customer service background from a number of other industries, including retail. They could be natural for interacting with patients.
One simple strategy is to request interested candidates send an email with “I am your perfect candidate” in the subject line. If they don’t do it, it doesn’t necessarily rule them out for the job, but it could show they have trouble following directions.

Query: Challenges with Bilateral DME
I am an office manager for a podiatry practice in Oklahoma. Beginning in spring 2022, I have sometimes (not always) had a problem with a major insurance company only paying one code when I bill that code bilaterally. This has happened on the codes L3000, L4397 and CPT 73630. I billed with LT or RT modifiers on each one. The denial states “duplicate service.” When I called the insurance company to have them reprocessed, thinking it was just an error, they told me I am missing a modifier in addition to the LT or RT. I cannot find any guidance on any other modifiers. Then just this week, I had a different major company deny L4397 saying the same thing. Healthchoice did not pay either side and they said I need a modifier in addition to LT or RT on both L4397 items. Do you know the answer here? Do you know of a modifier?

Response: For the DME products like L4397, we use -KX for all insurance companies, and we have found with a few, the -NU is also required.
As for x-rays, we bill each x-ray on a separate line with the laterality modifier. We have not had any trouble getting paid with this method. The -50 modifier does not work on x-rays judging on our experience.
For orthotics such as L3000, we bill them on separate lines as well. Since we are dealing with commercial payers for this service, the rules can vary widely. We have to be ready to correct the coding if we run into trouble.

While I wholeheartedly agree with you, unfortunately, to my knowledge, non-employed physicians (e.g. Independent practitioners or groups), are prohibited from going on strike by some antiquated labor Anti-Trust laws. That being said, if there are any legal genius types or innovators out there that have a way to bypass these antiquated laws, by all means count me in (along with probably thousands of other physicians).

Yes, indeed, there are masters of our profession and the torch continues to be handed down from one generation to the next. In 1988, our resident at Suburban General in Norristown, PA, was the wonderful Dr. Gina Carlo who we sadly lost to cancer in 2014.
We required our residents to produce a publishable paper and conduct a scientific seminar to complete their residency requirements. I personally worked with Gina on her outstanding seminar we called "A Master Class." We invited Dr. James Ganley (Norristown) and Dr. Donald Green (San Diego) to spend a memorable weekend presenting cases and critiquing each other's thoughts and theories. It was an outstanding seminar that is still remembered by those in attendance, including a young practitioner named Dr. Michael Downey.

After reading Dr. Roth’s letter regarding new orthotic theories, I recalled attending a lecture given by Dr. Ed Glaser at Lake Tahoe about ten plus years ago. I sat enthralled and captivated by each and every word that this mechanics genius spoke. I was in shock that almost every DPM at the seminar was hitting the slopes instead of hearing his words. He certified me after his workshop but I never switched to his lab.
I laughed at foam boxes being from the sixties and proudly showed my patients the latest and greatest Ipad scanner that I used. Shame on me for not realizing that his foam technique didn't capture a pronated impression, and the doctor had to know a precise method to hold the foot in order to capture the correct impression.
