DIABETES RESEARCH

Avandia Prevents Diabetes in Patients With Elevated Glucose: Study

Type 2 diabetes treatment Avandia, also known as rosiglitazone, delays or prevents diabetes in 60% of patients who have elevated blood sugar levels, according to a study published in the current issue of the Lancet. The study's authors write that the results "suggest that the addition of rosiglitazone to basic lifestyle recommendations substantially reduces the risk of developing diabetes by about two-thirds, offering a novel preventive approach that could be as, or more, effective and sustained than previously reported lifestyle approaches alone." Some experts say that diet and exercise changes still should remain the leading preventive approach for people with prediabetes.

QUERIES

Query: Dosage for Kenalog

While reviewing past issue of PM (March'06), I was intrigued by the dosage of Kenalog used to treat plantar fasciitis. The panel typically used 1 cc. of 10% Kenalog. I am not aware of any studies recommending a specific drug or dosage. My question: is that low dose of Kenalog effective enough to provide any long-term relief? I have used 0.5 cc of Depomedrol 80mg/ml with good success with no significant cortisone flare subsequently. Any consensus out there?

RE: Marathoning Against Cancer
From: Sahani C. Howie

This November, I will joining two of my classmates, Lennie Peters (2007) & Michael Walters (2008), and Dr. Sharon Barlizo, DPM (NYCPM attending) in running the NYC ING Marathon. While many people have a plethora of reasons why they are running, mine is the fight against blood-related cancers such as leukemia and lymphoma. My husband, Daniel, was diagnosed at the age of 26 with a non-Hodgkin’s lymphoma and was given three months or less to live. Four years later, Dan is alive and cancer-free.

Money raised from the leukemia and lymphoma societies Team in Training program goes towards research for cures, chemotherapy and patient care. This research has resulted in decreased morbidity and mortality associated with blood related cancers. My goal is to raise $2,000 by October 15th and I'm only a few hundred dollars away. All I'm asking for is a dollar for every mile I will be running (so, 26.2 miles = $26) If anyone would like to donate they can do so online at :

http://www.active.com/donate/tntnyc/tntnycSHowie or via mail at 31 Rustic Road, Yaphank, NY 11980. Thank you.

Sahani C. Howie, NYCPM Class of 2007, showie@nycpm.edu

RE: Lamisil for 12 Year Old (Bryan Markinson, DPM)
From: Eugene A. Batelli, DPM

I have used oral anti-fungals in children very safely as well. My original drug of choice based on the recommendtion of a well respected infectious disease specialist was Diflucan 200 mg weekly in divided doses for 3-6 months. I currently use Lamisil, as Dr. Markinson does, dosing based on Gupta's work as well. I have had no reactions with it. Despite pediatrician objection, most parents went for it after showing them before and after photos.

It also boggles my mind how many podiatrists are afraid of "off label uses." I was a student of Dr. Markinson so maybe this is why i think this way. I use Marcaine in children all the time. I'm sure almost everyone uses 4% Alcohol for neuromas. Alcohol isn't FDA approved for chemical neurolysis. I recently had a 13 year old female diagnosed with fibromyalgia. Her rheumatologist put her on Neurontin, Elavil, and Tolmentin. Talk about "off label" use! Maybe we as a profession need to really get our act together and start publishing more. It definitely brings our work to the forefront of medicine. Food for thought.
