CA Podiatrist Discusses Gout
 
If you notice an enlarged, painful big toe, it could be: What you ate. Gorged on wine and steak? The painful aftermath could be gout, a type of arthritis that usually affects the joint of the big toe. Foods high in purine, a chemical compound found in red meats, fish, and certain alcohol, can trigger an attack by raising levels of uric acid in the body. Uric acid is normally excreted through urine, but is overproduced or under-excreted in some people.

“You’ll see the deposition of the uric acid in the joint, most commonly the big toe or the ankle,” says Bob Baravarian, DPM, a podiatric foot and ankle specialist at Providence Saint John's Health Center in Santa Monica, California. “The patient will wake up with a hard, red, swollen joint. It’s extremely painful.” A doctor may prescribe anti-inflammatory drugs for short-term relief and suggest a low-purine diet for long-term prevention.

PA Podiatrist Volunteers at Walk a Mile in Her Shoes Event
 
YWCA Lancaster is holding its first Walk a Mile in Her Shoes event to raise money and awareness about domestic violence and sexual assault. The walk also is a way for men to express their concern about an issue that mostly affects women. "Although it's a very, very serious topic, it's a lighthearted way for guys to get involved," said Jill Brown, who organized the walk and is interim events coordinator at the YWCA.

Men can wear any kind of women's shoes. "They can wear slippers or sneakers, but we love stilettos," Brown said. If there are foot problems, YWCA board member Dr. Marilyn Henderson is a podiatrist who will be at the walk to offer help as needed.

UK Podiatrist Pedals to Raise Money for Kids with Rare Brain Condition
 
Despite recent knee surgery, a kind-hearted Sale podiatrist is hoping her legs will last for 150km of pedal power to raise funds for her friend’s young children who have a rare brain condition. Andrea Hunt, co-owner of Maple Road Podiatry Practice in Brooklands, is determined to complete the challenging Cycletta Cheshire on September 27 in support of the Reverse Rett charity.

The 46-year-old, from Hale, said: "My friends have beautiful girls, 15-year-old Beth and seven-year-old Kitt, who have Rett Syndrome. This is a rare brain condition which strikes little girls around their first birthday." Andrea, who’s a self-confessed cycle-fan, has also had a few tips from Olympic hopeful, Ciara Horne, a regular Great Britain Cycling Team member who’s training for Rio 2016.

Avoid this global statement, and any variations, even when patients ask if everything is okay. Instead, always respond with specificity. For example, "All of the tests we performed were normal."

Query: Use of Regranex in 2015
  
I have followed the development of Regranex from its introduction in 1997 (Ortho-McNeil-Janssen) to its current reincarnation (Smith & Nephew). I believe in the product and the science behind it. I have been hesitant in recent years to use Regranex due to the black box warning concerning malignancy. I would like to get a barometer reading from my colleagues in wound care concerning their opinion about the use of Regranex in 2015. How frequently is Regranex being prescribed? How does it compare to newer comparable therapies? How much emphasis is being placed on the black box warning when prescribing and using Regranex?

Query: Casting for Orthotics
 
We normally bill CPT 29799 (unlisted procedure, casting or strapping) when billing for casting of the orthotics. Some insurance companies pay; most do not. Has anyone billed code, S0395 - impression casting of a foot performed by a practitioner other than the manufacturer of the orthotic?

Response: S0395 is a valid HCPCS code, but in my experience (maybe it is regional), many payers are looking for a regular CPT code. I have found it hit or miss depending on the payer, even depending on the plan. You might want to check with your most common commercial payers to see what their code preference is.

I started a call center in my veterinary practice about 8 years ago. This is a 5-doctor practice. It required an initial increase of two staff members. However, once we had the bugs worked out, we were able to decrease our receptionists 'on the floor" from three to two, so our ultimate payroll hours showed a minimal increase. It has worked well for us. The phone persons need a different skill set than our receptionists. The phone persons make appointments and triage calls.
 
Since our receptionists are hired to welcome patients to the practice, they found it nearly impossible to efficiently handle the phones. Calls were continually lost or placed on hold for excessive time periods because they could not "escape' from the call. On the other hand, a single phone person can handle our 5 incoming lines very well. We hired initially from a temp service because they were able to screen for the skills we needed. We did not have a commitment to hire, but eventually found two we really like, and they are both now our employees.
 
I have not initiated this position in my podiatry practice because I do not think it is necessary in my solo practice.

After 36 years of practice, I believe that this is really a true misconception by a great many practitioners. I have found through experience that the secret is to say what you do know and what you don’t know, both your personal knowledge base as well as the entire profession’s knowledge base in a manner that doesn’t lose the patient’s trust. Often, I say the following, “I don’t know, yet.” This indicates to the patient that I do want to know, and am continuing to study their particular problem.  
 
Unfortunately, I have found an inverse relationship between what physicians say they know and what they really do know. For the truly knowledgeable, the book of things they don’t know should be growing faster than the book of things they do know. The more we learn, the more we learn how little we really do know, and it humbles our attitude. I have found that honesty with patients in our knowledge base is the best way to inspire trust – which is the real foundation of any rewarding doctor-patient relationship.
 
The best way to lose that trust is for the patient to find out later that you really didn’t know as much as you first appeared to know -- and with the Internet, the speed at which they find out is much faster than it used to be. What most patients really want is basic competence, true caring, hard work, and honesty.
