Feet are a Window to Your Overall Health: TX Podiatrist
If you want to take a step toward better health, see a foot doctor. You might learn something about a totally different (and seemingly unrelated) part of your body. Sometimes, “your feet are the first place where you can see warning signs of things like diabetes or vascular disease, or even skin cancer,” says Hira Mirza, DPM, a podiatrist at CLS Health in Houston. “If you look closely enough, it really is a window to the rest of your health.
|
Dr. Hira Mirza |
"If you have an ingrown nail that's red and irritated, don't ignore it, because it's only going to get worse,” Mirza says. “Those infections can go really wrong, really quickly.” In addition to turning red, keep an eye out for tenderness, pain when you’re walking, drainage, or dried-up blood. Don’t try to fix the problem at home: “What happens is people unfortunately try to mess with that toenail with a non-sterile instrument or tool,” she says.

PRACTICE MANAGEMENT TIP OF THE DAY |
Ways to Cut Overhead Without Cutting Staff
Here are nine practical ways to cut overhead without cutting staff,
3. Cut wasted energy and gaps in appointments
From lights left on overnight to unused appointment slots, small inefficiencies add up. Physicians Practice points out in its list of hidden practice inefficiencies that addressing these issues can reduce overhead while also creating a smoother patient experience. An example is changing to low-energy LED light bulbs.

RESPONSES/COMMENTS (VALUE-BASED CARE) |
RE: Value-Based Healthcare is Coming...Please Be Ready
From: Michael King, DPM
DPMs, Are you prepared for the changes to come with Medicare over the next few years? CMS wants to move to a value-based care (VBC) compensation system by 2030: CMS has set a goal of having 100% of Traditional Medicare beneficiaries and the vast majority of Medicaid beneficiaries in accountable care relationships by 2030. CMS VBC 2030 fee-for-service will not be gone right away, but DPMs must be prepared for the move to VBC.
Patient management within a value-based system can be both cost-saving and rewarding for patients and providers. We must be part of these changes early to assure we are part of the solution and not the problem. ACO REACH allows the DPM to be part of the gate-keeping system for patients, affording them access to all levels of care necessary to keep them healthy. We have the opportunity to change the paradigm of healthcare. Let's be ready.
Michael King, DPM, Nashville, TN

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION) - PART 1A |
From: Jon Purdy, DPM
In reading the response from APMA, I had to take a sideways look at the words: uncertainty, apprehension, and concerns. So, the end result was the APMA not wanting to “participate in an exploratory process.” I’ve always thought the best way to alleviate or validate uncertainty, apprehension, and concern was through the exploratory process.
Nowhere in the response did I read that the ACGME requires signatures, commitment, or anything binding. Additionally, the rationale for this decision was based on “no guarantee the ACGME was interested in creating parity…” Again, I thought commitments and guarantees were end results of exploration and negotiation. I’m not sure what there is to debate in the future if no investigation was performed.
Another confusing word to me was “stakeholders.” Are the practicing podiatrists not the actual stakeholders? The entire response to me was a “tell.” What I inferred from the collective verbiage of the alphabet entities was a “concern” for our leadership positions; an “apprehension” about losing our power; and an “uncertainty” of our inclusion. Is it just me, or shouldn’t the APMA governance documents be irrelevant to “listening” to ideas of any magnitude? Shouldn’t these documents be well understood prior to saying no? At the very least, I guess we know who was involved in the decision-making.
Jon Purdy, DPM, New Iberia, LA

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION) - PART 1B |
From: Ivar E. Roth, DPM, MPH
As I have written earlier, I know of extreme cases of where the residencies failed their residents; like when a recent graduate never did one case skin-to-skin. We need to be part of ACGME to make sure our programs are legit. Unfortunately, we have too many people willing to look the other way and give a pass to a failed program. This has to stop for the betterment of our profession.
Ivar E. Roth, DPM, MPH, Newport Beach, CA

|
RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION) - PART 1C |
From: ABFAS Board of Directors
We commend the APMA for the solicitation of feedback and their detailed evaluation of the proposal to engage with ACGME. In our response to the APMA, we voiced serious concerns about the erosion of professional autonomy, the dilution of surgical standards, and the risks of transferring oversight to an external body lacking podiatric expertise.
We also support the independent assessments conducted by other organizations such as AACPM, ACFAS, COTH, CPME, and many state societies, each of which reached the same conclusion; that participation in this initiative is not in the best interest of podiatric medicine. This unified position reflects our shared commitment to preserving the integrity of podiatric education.
