OH Podiatrist Discusses How to Avoid Infections When Getting a Pedicure
 
Getting a pedicure is a good way to destress. But it can also be a headache if you end up walking out with an injury or infection. To avoid this, Joy Rowland, DPM, a podiatrist with Cleveland Clinic, encourages people to do some research beforehand. “Those wanting to get a pedicure should look into the salon they’re planning to go to,” explained Dr. Rowland. “They want to make sure their instruments are being sterilized – it’s okay to ask a salon about their sterilization process.”

Along with the tools used on your feet, foot baths or bowls need to be cleaned and disinfected after each client as well. During the pedicure, Dr. Rowland said to make sure your nails aren’t cut too short to prevent ingrown toenails.

5 Signs You Might Be Better Prepared for Retirement Than You Think
 
There are plenty of articles in the financial press about how Americans are woefully unprepared for retirement. Many of these stories mention how the average retirement balance is only a few hundred thousand dollars and that to “live a comfortable retirement,” you’ll need $1 million or more socked away in your accounts.
 
4. You Live in an Inexpensive City or State - If you live in the most expensive locations in America, like Hawaii, Manhattan or Los Angeles, it’s obviously going to be harder to stretch your retirement savings. But there are plenty of desirable locations across the country that have a cost of living 40% or more below these pricey areas.
 
According to Numbeo, you would only need about $5,500 in Cleveland to maintain the same standard of living that would cost you $9,500 in New York City. While there are certainly arguments to be made about how living in some cities is different than in others, saving $4,000 a month by simply moving from New York to Cleveland could make a huge difference in terms of how prepared for retirement you are. And this is just one example of literally hundreds. If you prefer to live in a warmer location with unique Southern charm, you could spend a whopping 75% less in rent by living in Charleston, SC, instead of New York City.

RE: New CTP Coverage Policies that Affect Providers in CGS, Novitas, and FCSO

To all of the providers in these 3 MAC jurisdictions (encompasses 15 states):
You should be worried if you apply a single CTP (skin substitute) in your clinical practice as part of your wound care practice. While these policies have been delayed until 10/1/2023 effective on 9/14/2023, as a stakeholder in these states you NEED to be your own advocate.
 
Write to your local US Representative and US Senators and IMPLORE them to send letters to CMS to stop these policies from becoming effective. The limitation of 4 applications of CTP products was a gross misinterpretation of the study that Dr. Armstrong, et al. authored. The MACs have used this evidence to qualify only 4 applications (which is what the data DID NOT SHOW).
 
This is a call to action for ANY DPM in ANY jurisdiction. The rumor mill has it that Palmetto is considering the same policy next. Be vocal. BE PROACTIVE.

I have two simple remarks in regard to recent issues in our profession. Firstly, CPME and AMPA may well be “separate” entities, but if you believe that, then I have a bridge to sell you over the Mississippi River. Secondly, there is letter after letter bemoaning the fact that we have given up routine foot care, but you have to have a 3-year surgical residency to practice. Does that not seem convoluted to you? Just a thought.

I started my Ohio practice in 1974. From the start, all kinds of podiatric procedures were in my purview: Warts, ingrown corrections, fractures, hammertoes, etc. Naturally, routine foot care (CNC) was a major component of my practice. I used staff to set up and finish the patient. In those days, Medicare would pay a little for a whirlpool. Patients would be finished with lotion and a foot massage. As the practice matured, I became ABPS board certified and practiced more foot surgery. But, C&C was always there and profitable.  
 
I retired in 2016. The major factor I considered, beyond the current insurance and management hassles, was/is degenerative arthritis in my dominant hand. My hand orthopod stated he never saw so much DJD in a hand. I've had episodes of Dupuytren's and digital spurs treated in that hand since retirement. If I had worked any longer, I would not have been able to enjoy retirement. In later years of practice, I still did all the C&C, and I had a medical assistant finish the mycotic nails with a Dremel drill and central vacuum. If I would have had access to an ancillary nail person, I would have readily accepted that and stayed in practice longer. I do believe that supervision and adequate training would be necessary for such a person to be in the practice and billing "ancillary" C&C.

I thought we established last year that you did a 2-year residency as there were only 2 three-year residencies when you graduated, none of which you matriculated in, but for some reason you keep mentioning it. I also find it odd that you were proud of upselling an elderly patient who came in for nail care and wound up with a $3,000 bill for varied services.
 
Would you appreciate it if an elderly relative went to an optometrist for a replacement of eyewear and wound up spending $3,000 due to up-selling?

As most of us have surmised, MIPS and "Meaningless Use" are nothing more than money grabs with big brother hovering over us. It is a long-term game…overburden the practice with documentation and clicks in the computer, increasing the cost to the practice, all the while having the medical profession grovel to stop a decrease in payments. Big brother is hoping to “starve” us out of private practice into socialized medicine, like taking lambs to slaughter. Talk to a Canadian and see how that system works.
 
Oh yes, and the autoworkers are striking for a 40% pay raise over 4 years.

As a former podiatric sports medicine consultant with a Division One college team. I did witness a higher percentage of lower extremity injuries on turf surfaces than on natural grass. The velocity players can achieve is quicker and faster on turf. I feel this contributes to the higher and more severe type of injuries. We all know that the further down the lower legs, the biomechanics (physics) greatly increases and is concentrated into the ankle and foot, which goes from a mobile adaptor to a ridge lever!
 
NFL commissioner Roger Goodell says the NFL biomechanics specialists will continue to examine the causes of injuries to determine which surface is better. He wants to follow the science. That makes sense! However, science data is objective data. Athletes are each biomechanically different! One can never be 100% accurate using objective data on subjective humans. DNA rules.
