NY Podiatrist Discusses Blister Care
To beat blisters for good, Jacqueline Sutera, DPM says you first need to understand why they form in the first place. "Blisters are fluid build-ups under the most superficial layers of skin and can form in areas of motion, such as joints. They typically arise near bony prominences, which make contact with the shoe while running or walking," she says. "That friction causes the layers of the skin to pull apart and form a little bubble, which in turn, fills with fluid."

"The best thing to do is just leave the blister alone. In a few days, it will break and the fluid inside will drain on its own. If the blister is causing you discomfort, you can take action to express the liquid, very carefully," says Dr. Sutera. "Once the blister is flat, apply a dab of triple antibiotic ointment or cream, and cover it with a bandaid. Change the bandage every day for the next three to five days, until the blister has healed. If the areas get red and swollen and you see pus, those are signs of infection, so call your doctor for advice on what to do next," says Dr. Sutera.

New CPT Codes for COVID-19 Released
The American Medical Association (AMA) recently published an update to the Current Procedural Terminology (CPT®) code set that includes two code additions for reporting medical services sparked by the public health response to the COVID-19 pandemic. The first addition, CPT code 99072, was approved in response to sweeping measures adopted by medical practices and healthcare organizations to stem the spread of the novel coronavirus (SARS-CoV-2), while safely providing patients with access to high-quality care during in-person interactions with healthcare professionals. The additional supplies and clinical staff time to perform safety protocols described by code 99072 allow for the provision of evaluation, treatment, or procedural services during a public health emergency in a setting where extra precautions are taken to ensure the safety of patients as well as healthcare professionals.
The second addition, CPT code 86413, was approved in response to the development of laboratory tests that provide quantitative measurements of SARS-CoV-2 antibodies, as opposed to a qualitative assessment (positive/negative) of SAR-CoV-2 antibodies provided by laboratory tests reported by other CPT codes.

RE: The Passing of Jeffrey Siegel, DPM
It is with tremendous sadness that I inform PM News readers of the untimely passing of Dr. Jeffrey Siegel. He died peacefully in the presence of his beautiful wife Laura at a hospital in Florida due to kidney and GI complications. Jeff was not only my dear friend but for the past 17 years accompanied me and so very many residents, students, and attendings on our annual mission trip to Nicaragua. The lives he has touched, the limbs he has saved, and the students he has taught would be too voluminous to list here.

Jeff was a Navy veteran, a loving husband and father to his sons and daughters, and one of the most spiritual people I have known. Those of us who knew him will have beautiful memories forever. Query: Source for Dental-Type Sani-Grinder
Does anyone know who manufactures a Sani-Grinder? I’m looking for one that does not have a vacuum attached but dispenses a fine water spray similar to those used by dentists. I recall a vendor in NY but he moved to FL and I believe he still has sales. I would appreciate any assistance available.

RE: Treating Plantar Warts with 5-FU Solution Combined with Microneedling

In 86.7% of patients having plantar warts a complete resolution resulted in a study employing a combination of 5-fluorouracil (5-FU) solution and microneedling. Treatment was provided for a maximum of six sessions two weeks apart. There was no recurrence reported. The use of microneedling was well tolerated when evaluating therapeutic efficacy, safety, and tolerability. When 5-FU was administered intralesionally, its use was limited because of intense pain. First, I would never allow someone in the office to copy charts unless it was Medicare. As previously stated, you are entitled to get compensated for the charts requested. By sending Ciox an invoice requesting reimbursement, you are in full compliance of their request. If you ignore their request, it possibly could get sticky.

Read your contract with the insurance company requesting the risk adjustment audit through a third party (CIOX/others). In most contracts, it will state that you are obligated to send the requested records or you will be terminated from your contract with said insurance company. They aren't looking to get refunds from you; that is not what a risk adjustment audit is about. It is about the insurance company getting more money from Medicare for patients with MA plans. These patients have a higher risk based on medical conditions that the insurance companies may not know about, but can become aware of through various doctors' records.

This is something that has been discussed a couple of times now. This is clearly illogical and just plain dumb. I was wondering if the APMA can share with readership if this is something they deal with and if they have dealt with it.

Dr. Poggio queried, “What is the medical necessity of removing a benign lesion beyond the fact that it is there?” Dr. Boudreau responded, "In my residency 35 years ago, I was privileged to follow an “old school” dermatologist in his clinic one day per month. He biopsied EVERYTHING a patient was concerned about. I asked him why he did that, and his response was brilliant and I incorporated it into my practice. His answer: 1. You never know 100% what a lesion/mass is until you cut it out and look at it under a microscope, and 2. It can’t hurt you if it’s in a jar. This, Dr. Poggio, is why you remove a lesion."
First, I am a complete advocate for podiatrists doing more biopsies. However, although I can appreciate Dr. Boudreau's enthusiasm and intent on protecting his patients, his old school mentor's advice and his following it to this day, to say the least, is no longer appropriate in any way. Although this practice was commonplace...
