Key Textbook by NYCPM Dean Translated into Chinese
 
Mark H. Swartz, MD, Dean for Medical Education and VP for Professional and Medical Affairs at NYCPM and Medical Director of the Foot Center of New York, published the first edition of his Textbook of Physical Diagnosis: History & Examination in 1989; it is now in its 7th edition (2014) and is the standard text for teaching the subject in the U.S. The text has just been translated into its 15th language – Chinese -- and translated and released by Peking Union Medical College Hospital, which is often referred to as the “Harvard of China.”

Dr. Swartz was invited by the college and visited there in early September to celebrate the translation and to teach clinical skills to its students via lectures, workshops, bedside patient rounds with students and faculty, demonstrations of the physical exam, and standardized patient (SP) methodology. His lectures were translated simultaneously.

IL Podiatrist to Supervise Complex Lower Extremity Reconstruction Service

Presence St. Joseph Hospital has announced the establishment of its Complex Lower Extremity Reconstruction Service. The program will be under the direction of Edgardo Rodriguez Collazo, DPM, Director of the Chicago Foot & Ankle Deformity Correction Center and the Associate Residency Director for the Presence Saint Joseph Hospital PGY-3 Program.

Rodriguez Collazo is also the Chairman of the annual Chicago Lower Extremity Surgery Symposium in October in Chicago (clesf.org). He has extensive experience in muscle flap procedures and peripheral nerve surgery, which will also be integral to Presence St. Joseph Hospital’s LE Reconstruction Service.

NY Podiatrist Receives Community Service Award
 
Wagner College's DaVinci Society launched its observance of Italian Heritage Month by hosting its annual scholarship dinner. A Community Service Award was presented to Dr. Stephanie Famulari. Dr. Famulari is a practicing podiatrist on Staten Island. A 2004 Wagner College grad, she earned her doctorate from the New York College of Podiatric Medicine in 2009.

Dr. Famulari is the podiatric physician in Eger Health Care & Rehabilitation Center and Harbor House, Egbertville and New Vanderbilt Rehabilitation Center, Clifton. She gives her time to Camp Good Grief, Richmond University Medical Center, and Workman's Circle Nursing Home in the Bronx, as well as at Bishop High School, Graniteville.

Query: Contusion Great Toe Left
 
I keep having problems understanding the right way to use "x" character in ICD-10. I saw a patient for a contusion to the great toe nail, left foot. I billed S90.45xA and Aetna rejected it. 
What did I do wrong?

Response: You used a code for superficial foreign body in the toe, rather than for a contusion. And you would not have used the "x" as there is a valid 6th place number specifying the location. The "x" is used only as a place marker when there are no character choices listed for the sixth spot when a seventh is required (for example). There are some codes that use more than one "x" as place markers, so you need to research carefully. 
 
Had the patient presented with a superficial foreign body of the right great toe, active care encounter, you would have coded S90.451A.  However, you said you treated a contusion that included damage to the nail, so that would be coded S90.211A (contusion of right great toe with damage to nail, active management). You did not say what CPT code you used, but it should be appropriate for the treatment of a contusion right great toe with damaged nail.

MACRA UPDATE
Use MACRA Flexibility to Prepare, Not Slack Off
 
One thing practices of all kinds have going for them to comply with MACRA is a bundle of resources at their fingertips. There are resources on complying with MACRA from all sorts of places: CMS itself, organizations like AMA and MGMA, system vendors, and consultants. As Andrew Gurman, an orthopedic hand surgeon from Hollidaysburg, PA and president of the AMA, said in an interview, "Start looking at the things you can look at when it comes to the quality measures…everyone wants doctors to succeed with MACRA, there are resources for that."
 
ICD-10 used to get delayed every year. Often, you'd talk with providers and they said they couldn't aptly prepare because it seemed as if it'd be delayed forever. That will not be the case with MACRA because the timelines are baked into the law. Moreover, CMS is gearing up for a switch to value-based care and MACRA is the vehicle in which they'll make that transition more of a reality. The train has left the station. You can both continue serving Medicare patients and get ready for MACRA, or you can get off and try another route.

It is my sad duty to inform our alumni that Marc Karpo, DPM, Assistant Professor in the Department of Podiatric Medicine, was killed yesterday morning in a tragic traffic accident. Dr. Karpo was an alumnus of TUSPM (formerly the Pennsylvania College of Podiatric Medicine) from the Class of 1982. Dr. Karpo served on the faculty at TUSPM since 2008.

Excerpted from Codingline of Sept 14:CMS announced today (September 8, 2016) that rather than a delay, "eligible physicians and other clinicians will be given four options to comply with new payment schemes [emphasis added} such as the Merit-based Incentive Payment System (MIPS) or an alternative payment model such as accountable care organizations."
 
From Merriam-Webster Dictionary: Simple Definition of scheme: a clever and often dishonest plan to do or get something
From Macmillan Dictionary: a plan for achieving something, especially something illegal or dishonest
From Urban Dictionary: to devise a devious plan. To be shady or otherwise use intellectual power to deceive or carry out a plan for a personal interest. A plan with a twisted plot.
 
I just thought that CMS’s use of “scheme” should be evaluated a bit more closely. Maybe they are telling the truth?
