NC Podiatrist Discusses Plantar Fasciitis
Plantar fasciitis is inflammation of a band of tissue on the sole connecting the heel bone to bones near the ball of the foot. People’s Pharmacy podiatrist Jane Andersen, DPM says that one of the most important steps to treat this condition is to wear supportive footgear, such as OOFOS or Vionic sandals, even in the house. No walking barefoot!

Dr. Andersen recommends gentle stretching before getting out of bed. When the heel is painful, icing or massaging may help. Over-the-counter or customized orthotic inserts may also be beneficial. Some people take an anti-inflammatory supplement such as turmeric or curcumin, and that may sometimes ease the pain.

NY Podiatrist Named Recipient of Cullman Family Award
Bryan C. Markinson, DPM, Chief of Podiatric Medicine and Surgery at the Icahn School of Medicine at Mount Sinai has announced that Irina Blustein, DPM, a podiatry attending in the Department of Orthopedic Surgery, has been named a recipient of the 2022 Cullman Family Award for Excellence in Physician Communication.

This award honors Mount Sinai physicians and advanced practice providers who ranked in the top 1% nationally as measured in CMS's Clinician and Group Consumer Assessment of Healthcare Providers and System's patient experience survey. Dr. Blustein is a 2012 graduate of the New York College of Podiatric Medicine. She completed her residency training at Mount Sinai, and has been an attending clinician since 2015.

ACLES Runs Successful On-Site Conference
The American College of Lower Extremity Surgeons (ACLES) conference was held the weekend of September 10 in beautiful Saratoga Springs, NY. This has become an annual event for the ACLES now running for its 12th year.

What to Consider Before Starting a Concierge Practice
A concierge practice can be a lucrative way for a provider to achieve work-life balance by reducing the number of patients, increasing time with patients and making great profits. But before you transition, speak with an experienced attorney who can help the transition.
Here are some possible pitfalls:
4. Understand how Medicare addresses concierge medicine.
As for governmental healthcare programs, Medicare providers may engage in concierge services if the provider complies with Medicare regulations and the terms of assignment agreements, most notably the requirement to not charge Medicare beneficiaries extra for services already covered by Medicare.
Medicare’s concerns about concierge medicine and assignment agreements are outlined in a 2004 Office of Inspector General (OIG) advisory opinion alert titled “OIG Alerts Physicians About Added Charges for Covered Services — Extra Contractual Charges Beyond Medicare’s Deductible, Co-insurance: A Potential Assignment Violation.” Medicare providers should also review federal authorities for civil monetary penalties.

Query: Arthrodesis Revision Coding
How would you code a resection and revision of a non-union of a first metatarsocuneiform arthrodesis? This involved the removal of previous hardware and implantation of bone graft with new fixation.

Response: The question states “revision of a non-union of a first metatarsocuneiform arthrodesis.” In this case, I believe that CPT 28320 “Repair, non-union or malunion; tarsal bones” would be the most specific code and describes what was performed. CPT 20900 is also appropriate because there is no CCI edit for this pair. The RVUs are almost the same for CPT 28740 and CPT 28320.

In my 40+ years of imaging, I have never seen the amount of vascular clips on an x-ray before. This finding (grains of rice) is consistent with a calcified cysticercosis or small calcificied cysts secondary to infection with Taenia solium.

I was absolutely shocked today when I received an email from APMA on behalf of a private company wanting information from doctors and only offering $10 compensation. I can’t believe that our leaders think so little of us, and the value to industry we provide. I’m sure each of our board of trustee members who lecture on behalf of many of the corporations that deal with products for our profession are unwilling to travel without a substantial honorarium. How embarrassing they allow this to occur.

I believe that the misinformation class is a grand idea. The podiatric sector that hears the patients declare that this is what the Internet said I have to do, rules the thought pattern of the public for footcare. Frankly, the MDs/DOs and PAs have the same info, so where is the true education regarding the lower extremity developing in institutions of medicine? I have heard stories how at the UC, a daughter and mother could see the PA looking at the computer screen, then wander back to the treatment room and try to perform an avulsion.
Then there is the story how the primary physician treated a callus for over 3 years, but it was an SCC that only got larger. The info is so obsolete and incorrect for what to do with an ingrown toenail or say that an HM medial toe is a wart!
I roll my eyes every day when the words come out of patients' mouths how his or her doctor said the way to help plantar fasciitis is to put Dr. Scholl inserts in shoes. That arch support might hold up a toddler, but for the 200 lb. female, it’s NOT happening! Yes, educate the young doctors of all branches of medicine to deal with misinformation and set up a few seminars for older practicing docs as well.

All is not lost, indeed! As I read your editorial “Biomechanics and the Survival of Podiatry”, I realized there is growing evidence that we are actually entering a new era of biomechanics and this is a welcome sign. But it is not necessarily the biomechanics of my teachers, as research expands our knowledge. Western University College of Podiatric Medicine is contributing to this renaissance. A modern biomechanics lab opened this fall, with instruments that evaluate both kinesthetics and kinematics.
The mainstay of traditional orthotic production is taught by Dr. Arnold Ross, and through a gift from KevinRoot Medical, our students will learn creation of orthotics through scanning technology. The late Paul Scherer, DPM and Dr. Aaron Meltzer established a Biomechanics Fellowship at WesternU with the intent of educating podiatric physicians who have an interest in teaching...

The argument as to why many insurance companies do not pay for orthotics is an old one. Dr. Roth may have hit on the reason when he writes that he tells his son, a podiatry student, that when he graduates, he should forget all that he learned in biomechanics and subscribe exclusively to Dr. Glaser's theory.
There are so many great biomechanics sages in our profession. There was Root, Weed, Orion, Schuster, Langer, and currently Shavelson, Glaser, and others. The list goes on and on. The never-ending problem is that all of these wise doctors' theories differ from each other. Hearing their lectures made believers out of the listeners, but insurance companies looking over their shoulders see conflicting theories and there is little evidence-based medicine to prove one theory from another.
The solution is for our labs to do what drug companies do. Invest in research, publish the research, and only then can the APMA and others go to Medicare and other insurance companies with credible reasons why patients should be entitled to be covered for their orthotics. We know that our patients feel better with orthotics, but that alone doesn't cut the mustard in the world of insurance.
