MA Podiatrist Discusses Hammertoes
 
According to Thanh Dinh, DPM, a foot and ankle surgeon and president of the American College of Foot and Ankle Surgeons (ACFAS), one of the most common types of pain women feel in their feet in autumn comes from hammertoes. “A hammertoe is a ‘bending’ or contracture deformity of one or both joints of a toe,” Dr. Dinh says. “The abnormal bending puts pressure on the toe when wearing shoes and causes problems to develop -- which can start mildly and worsen over time.”

Women with hammertoes can experience pain or irritation of the affected toe when wearing shoes. Corns and calluses on the toe, between the toes or on the ball of the foot can occur from the constant friction against the shoes with hammertoes. Inflammation, redness, or a burning sensation are also possible and, in severe cases, open sores may form.

Time in Range — Not HbA1c — Should Guide Most Diabetes Treatment Decisions
 
Clinicians should consider temporary continuous glucose monitoring for any person with diabetes to assess their time spent in the recommended glucose range as an effective guide for treatment decisions, according to a speaker. HbA1c may underestimate or overestimate a person’s glucose levels, does not reveal glycemic variability, and has limited utility for insulin dosing decisions, George Grunberger, MD, FACP, chairman of the Grunberger Diabetes Institute in Bloomfield Hills, Michigan, said during a presentation at the Heart in Diabetes CME Conference.
 
HbA1c is also unreliable for people with hemolytic anemia and iron deficiency and for those who are pregnant. HbA1c correlation with mean glucose can also vary among racial backgrounds. Time in range emerged as a prominent theme among diabetes experts as early as 2016, when the American Association of Clinical Endocrinology convened a consensus conference on CGM to determine new standard simple metrics that could inform therapy adjustment and “get away from HbA1c culture,” Grunberger said.

CMS to Require Staff at Hospitals and Other Facilities to Get COVID-19 Vaccines
 
The Biden administration is moving to require all Medicare and Medicaid facilities to vaccinate their workforce, expanding a mandate already applied to nursing home staff. The Centers for Medicare & Medicaid Services (CMS) last week announced its intention to release an interim final rule in October to require hospitals, dialysis facilities, ambulatory surgical centers, and other facilities to vaccinate their staff as a condition for participating in Medicare and Medicaid.
 
“Data show that the higher the level of vaccination rates among providers and staff, the lower the infection rate is among patients who are dependent upon them for care,” said CMS Administrator Chiquita Brooks-LaSure in a statement. “Now is the time to act.” Healthcare workers currently employed in affected facilities who are not vaccinated need to begin the process immediately, CMS said.

Here’s your guide to six common phrases physicians too often say to patients, but shouldn’t:
 
6. “Oops.” This was the favorite among our informal mini-survey of physicians, but all jokes aside, if you make a mistake this is not the phrase you want to lead with when telling the patient. Transparency and admission of error are good rules of thumb, but hearing the doctor utter “Oops” or some similar term while on the exam table strikes fear into even the toughest patients. Try to avoid exclamations and tell the patient the truth in a way that exudes competence and professionalism.

Query: Big Bunion Surgery
 
I have a patient with a very large bunion deformity. I am planning a Lapidus bunionectomy and plan to combine this with an Akin osteotomy and McBride procedure. Are these all separate procedures for this case?
 
Response: The CPT book is getting away from using names such as McBride/Lapidus/etc. as it can not fully reflect what procedures are being performed.
 
If you are fusing the metatarsal cuneiform joint and removing the medial eminence of the first metatarsal head, that is one procedure. The phalanx osteotomy would be billable separately. Your operative report needs to document what exactly you are doing to justify the procedure(s) performed.

RE: ASPS Voting to Allow Non-Surgical Board Status to Qualify for Fellow Status
 
I recently decided to cancel my Fellow status with ASPS. I rejoined the ACFAS. I recently was informed that ASPS, which is a society of “podiatric surgeons”, is asking its members to vote to change its bylaws. To be a Fellow, you previously needed to be certified by ABFAS. Now they are trying to say you can be certified by ABFAS and/or ABPM (which is NOT a surgical board!). Many members should be offended that this is even up for a vote. This society would lose all of its credibility if it allows members who are not certified in podiatric surgery to be called a Fellow from a society that calls its Fellows podiatric surgeons.
