KY Podiatrist Discusses Stress Fractures and Activity
 
If you are one of the many Dancing with the Stars fans out there who is anxiously awaiting the start of a new season, you may have heard about reigning champion Rumer Willis’ foot problems over the summer. Rumer suffered a metatarsal stress fracture in her right foot in June and has been in a walking boot ever since - until recently.

"Metatarsal stress fractures can be very painful and are often aggravated with weight-bearing activity. This means that even Rumer, the daughter of Die Hard‘s mighty John McClane (Bruce Willis) likely experienced pain and discomfort with her injury. Swelling, tenderness, and bruising also may occur with metatarsal stress fractures," says podiatrist Dr. Matthew Ellsworth.

Canadian Podiatrist Provides Tips for Buying Kids Shoes
 
While kids’ feet don’t generally swell after long walks like adults’ feet do, it doesn’t hurt to go shopping for shoes in the afternoon to ensure there’s adequate room, says Dr. Brad Sonnema, a podiatrist in Edmonton. “When trying on shoes, children aren’t always very responsive and they don’t know what to look for,” says Sonnema. 
 
To find out if the shoe truly fits well, have your child try on the shoes with socks. Have her walk around in the shoes for ten minutes, then take off the socks. Inspect your child’s feet for signs of redness – that will give you an indication of whether you need to go up a size. If the shoe is slipping out or rubbing against the back of the feet, then it’s too big. Sonnema prefers this method to the age-old “there should be a thumb’s width between the tip of the shoe and the big toe” rule. If one foot is larger than the other, go for the shoe that fits the larger foot.

AAPSM “Tools of the Trade” Meeting in New York a Success
 
Under the chairmanship of Rob Conenello, DPM, this past weekend, the AAPSM greeted over 130 registrants at The Historic Thayer Hotel in West Point, New York who came to hear top sports medicine speakers from all over the world. Exhibit space was sold out, and shoe companies gave out over 100 pairs of free shoes.

Two Topical Analgesic Creams Superior to Voltaren Gel for Chronic Pain
 
Cream I (20% Flurbiprofen, 5% Tramadol, .2% Clonidine, 4% Cyclobenzaprine and 3% Bupivacaine) and Cream II (20% Flurbiprofen, 2% Baclofen, 0.2% Clonidine, 10% Gabapentin and 5% Lidocaine) were significantly more effective than Voltaren gel (1% diclofenac sodium) for patients with chronic extremity, joint, musculoskeletal, neuropathic, and other non-cancer pain conditions.  Click here to read full abstract.

ICD-10 UPDATE
One Symptom of ICD-10: Doctor Anxiety
 
The nation’s healthcare providers are under orders to start using a new system of medical codes to describe illnesses and injuries in more detail than ever before. The codes will cover common ailments: Did a diabetic also have kidney disease? But also included are some that are far less common: whether the patient was crushed by a crocodile or sucked into a jet engine. The more than 100,000 new codes, which will take effect on Oct. 1, have potential benefits, as they will require doctors to make a deeper assessment of many patients.
 
But the change is causing waves of anxiety among healthcare providers, who fear that claims will be denied and payments delayed if they do not use the new codes, or do not use them properly. Some doctors and hospitals are already obtaining lines of credit because they fear that the transition to the new system will cause cash-flow problems.

Health Savings Accounts Growing, Especially Among the Better Paid
 
The number of people with health savings accounts has ballooned, but higher-income families are much more likely to fund the accounts, a new analysis found. The number of health savings accounts, or HSAs, has been growing by about one million a year, reaching more than 6.5 million in 2012, according to research published this week in the journal Health Affairs. The analysis covered eight years, although the accounts have been around for more than a decade.
 
Health savings accounts let consumers save and pay for medical care, free of federal income taxes. Balances are carried from year to year, and consumers can often invest the money for tax-free growth. The accounts are not available to everyone, however. They must be used with a specific type of health insurance plan with a high deductible (at least $1,300 for a single person in 2015).

Query: Internal Hardware Coding
 
Can commercial payers be billed for internal hardware (screws) in an outpatient office setting? If so, what might the codes be? If not, can the patient be billed?

Response: Yes, you can bill for implants used in surgery. The most common code I have seen billed in L8699 (prosthetic implant, not otherwise specified). You should pre-authorize this (and a tray/room fee) by providing the payer information about the implant. In-office surgery may not be worth it if you are not compensated for your material and fixed costs. Also make sure that the implant you use is considered medically necessary for the surgery you are doing. 
 
It would not be a bad idea to ask about the reimbursement rate. There are a number of versions of surgical screws out there: standard, cannulated, dual thread, etc. There can be a significant variation in the cost of these items. If your payer only allows a certain amount for screws placed in surgery, then you may need to choose between the various options out there to make it cost-effective for you. 
 
If the screw is not covered, you could bill the patient as long as the payer doesn't have in your contract that you cannot or that the item is included in the surgical professional allowance. Make sure you have the patient sign an ABN or equivalent. Screws are not cheap. You can not balance bill them simply because you believe the reimbursement for the screw is not adequate.
