PODIATRISTS IN THE NEWS |
WI Podiatrist Discusses 3 Types of Ankle Arthritis
Acccording to Thurmond Lanier, DPM, "Ankle arthritis is a condition characterized by inflammation and gradual deterioration of the cartilage within the ankle joint. Cartilage is a crucial component that cushions joints and allows them to move smoothly. When it wears away, the bones can rub against each other, leading to pain, stiffness, and reduced range of motion. There are three primary types of arthritis that affect the ankle:
|
Dr. Thurmond Lanier |
Post-Traumatic Arthritis: This is the most common cause of ankle arthritis as most people have an injury to the ankle that then leads to arthritic changes. The injury can be as simple as an ankle sprain or as major as a fracture. Since an injury is usually the precursor to arthritis in the ankle, patients are usually younger when they start to experience symptoms from arthritis as compared to knee and hip arthritis. Osteoarthritis: Occurs as a result of the natural wear and tear on the joint over time. Factors such as age, genetics, and mechanical stress on the joint can contribute to the development of osteoarthritis. Rheumatoid Arthritis: This is an autoimmune disorder where the body's immune system mistakenly attacks the joints, including those in the ankles. It often leads to inflammation, pain, and joint damage."

PRACTICE MANAGEMENT TIP OF THE DAY |
5 Signs You Might Be Better Prepared for Retirement Than You Think
There are plenty of articles in the financial press about how Americans are woefully unprepared for retirement. Many of these stories mention how the average retirement balance is only a few hundred thousand dollars and that to “live a comfortable retirement,” you’ll need $1 million or more socked away in your accounts.
3. You’re Healthy
For some seniors, healthcare can be the biggest expense they face in retirement, even if they have insurance. While you can’t eliminate all healthcare costs, if you’re a healthy individual, you may be able to avoid some of the big-ticket items that plague many seniors, from expensive surgeries and hospital stays to specialized medications and long-term care costs.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 A |
RE: The Relationship Between APMA and CPME
From: Bret Ribotsky, DPM
The profession has been told for decades that APMA and CPME are separate companies, without any conflict of interest between them. I am a life member and remain a fan of APMA and the Council on Podiatric Medical Education (CPME). Many years ago, I served as the chair of the liaison Committee on Podiatric Medical Education. Today, it’s time to pull back the curtain and be honest, as change sometimes requires some discomfort and full disclosure.
1) The council’s office sits within APMA's physical office.
2) their phones are answered by the same receptionist when you call.
3) the current IRS form 990 form lists the executive director of CPME as a paid employee of the APMA.
The finger-pointing and blame game needs to come to an end. I attended the first summit on the board certification process and subsequently have spoken with the leadership or executive directors of the boards. So far, it has been a very disappointing investment. A resolution can be adopted that would appease the great majority of the issues facing DPMs today, and start us now on a better track for the future. Let’s put the egos aside, stop the game of blame, and all sit down at the table with a bunch of people who have “no dog in the fight” and solve this problem. If we do not take steps today, the profession that you’re all deeply invested in will sunset in the near future.
Bret Ribotsky, DPM, Fort Lauderdale, FL
Editor's note: APMA's and CPME's response appear in the next section.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 B |
RE: The Relationship Between APMA and CPME (Bret Ribotsky, DPM)
From: Keith D. Cook, DPM, Sylvia Virbulis, DPM
Dr. Ribotsky is correct that the Council on Podiatric Medical Education shares office space with APMA and that CPME staff are APMA employees. APMA provides administrative support to CPME. APMA and CPME have never attempted to obfuscate that arrangement. In fact, the CPME bylaws, a public document, clearly outline the relationship between CPME and APMA.
CPME operates as an independent, autonomous organization with a strict firewall between its accreditation activities and APMA. It stringently applies the same requirements to APMA educational activities as it does to other providers’ activities.
The US Department of Education recognizes CPME as the accrediting agency for the schools of podiatric medicine. The Department of Education has clear requirements that CPME operate independently of...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 |
From: Richard M. Maleski, DPM, RPh
I'm somewhat confused by the responses to this topic. I retired a few years ago, but I believe my information is current. In Pennsylvania, podiatrists are prohibited from using ancillary personnel to cut nails or calluses. Podiatrists must perform those services themselves, otherwise they are committing insurance fraud (if sent in to insurance.) Admittedly, this took place years ago, but in 2008, I spoke with the PPMA about training my certified podiatric assistant to cut nails and calluses, and was told that this was fraudulent if sent in to a third party payer, (billing for services that I wasn't providing) and allowing a person to perform services that I was not legally entitled or licensed to supervise.
I was seeing 40-50 patients daily 4 days per week at that time, and typically on any given day about 1/2 to 2/3 were "routine care." I was giving up new patients because...

RESPONSES/COMMENTS ( PODIATRISTS AND SPORTS MEDICINE) |
From: Joe Boylan, DPM
Professional ball players when surveyed in all sports have always subjectively felt better in general on natural grass surfaces. They feel less strain on their lower extremities and back. However, past retrospective studies of sport injuries have never shown a statistically significant difference of the incidence of injury on synthetic vs. natural field surfaces.
However, the NFL Players Union states, “data analyzed by the NFL Players Association (NFLPA) President JC Tretter shows that last year, the injury rate was 37% higher on turf. ”
My question: is the above statement by the union fact-checked? If accurate, changes should be considered regarding the use of synthetic fields in Pro and amateur sports.

RESPONSES/COMMENTS ( PM ARTICLES) |
From: Jeffrey Kass, DPM
I’m surprised in the poll conducted by PM News regarding the biggest existential threat that “paperwork” didn’t make the list. A recent article I read regarding one of the biggest issues among doctors cited “documentation burden” as the root of burnout among doctors. While not specific to podiatry, “documentation burden” could make anyone flee the medical profession, regardless of specialty or lack thereof.
BTW, has anyone found out yet, what the MIPS documentation is used for?
