MI Podiatrist Receives Ellis J. Bonner Award

Mark Saffer, DPM, president and CEO of Dearborn-based Midwest Health Plan Inc., was given the Ellis J. Bonner Outstanding Achievement award during the Michigan Association of Health Plans' 25th annual conference, held in July.

Barry Students Donate to Yucatan Crippled Children’s Project  

The Hispanic Podiatric Medical Student Association (HPMSA) donated $550 to help support the mission of Barry University’s Yucatan Crippled Children’s Project (YCCP). The YCCP was established in 1996 to provide surgical and medical treatment to crippled children in the Yucatan Peninsula of Mexico. For the past 14 years, physicians and surgeons from Barry University’s School of Podiatric Medicine have helped thousands of children who suffer from disabilities get back on their feet.

Marien Rodriguez, president of HPMSA and a member of the class of 2012 indicated that “HPMSA raised funds by selling lab coats and gloves, Spanish dictionaries and cards with medical phrases translated into Spanish, as well as holding a fundraising potluck dinner.” The HPMSA is a multicultural club that enables students from diverse ethnic backgrounds to learn about issues that affect practicing podiatric minority members and to help advance recognition of the podiatric profession. In addition, HPMSA advocates for awareness about health issues that affect minorities in the community.

PA Podiatrist Joins Rothman Institute

James A. Cancilleri, DPM, is the newest podiatrist at the Rothman Institute, the region's largest orthopaedic practice, based in Philadelphia, PA. Dr. Cancilleri's areas of expertise include nearly all aspects of foot and ankle care from severe bunions to foot fractures which require surgery for stabilization, treatment of athletes, and customized foot orthotics. As a "Physician Certified in Wound Care" Dr. Cancilleri brings to the Rothman Institute unique training in prevention and treatment of difficult wounds of the lower extremities.

Dr. Cancilleri earned his Doctorate of Podiatric Medicine from the Rosalind Franklin University of Medicine and Science, Scholl College of Podiatric Medicine in Chicago, IL and did his Podiatric Surgical Residency at Lawndale Community Hospital in Philadelphia. He was previously the owner and president of The Centre for Foot and Ankle Care in Somers Point, NJ for 16 years.

Medicaid Programs Receive CMS Matching Funds for EHR

Four state Medicaid programs will receive more than $6.9 million in total matching funds from the CMS to help them implement the electronic health record incentive program created by the stimulus law. This last round of funds brings the total matching dollars awarded to $81.44 million, for 49 states and the District of Columbia, Puerto Rico, and the U.S.Virgin Islands, the CMS said.

The HITECH provisions of the stimulus law stipulate that eligible Medicaid providers can receive up to 85% of $75,000 – or $63,750 – in incentive payments over six years for EHR implementation, use, and support services.

Query: Dealing With Specialists Who Refuse to Send Out Reports
 
We have all been taught how important it is to make proper and timely referrals to medical and surgical specialists. This is not only important from a medical legal perspective but it's good medical practice. A problem I have encountered in my local area is that not every vascular surgeon, internist, or orthopedic surgeon will sent us a report after they have evaluated our patients. I tend to refer to those practitioners who do send me reports but there are a handful of busy doctors who will never send reports. Calling them does not help and some are too busy to even return phone calls. What makes the matter worse is that when I send a patient to a specialist, if the doctor does not send me a report or give me a call, I will never know if the patient even went to see the doctor. In some cases, when I know the doctor's style, I have even told the patient to demand from the doctor to send me a report. It seldom helps.
 
I am certain that I am not alone in encountering this problem. What have some of you done to protect yourselves in similar situations? Many patients will prefer a certain specialist because they have either gone to that practitioner before, a family member has, or the insurance company will only allow that particular doctor or group. Invariably that is the group of doctors that tends to not keep us apprised of their clinical findings.

I also successfully use "On Your Toes" and have found it to be effective for bromhidrosis. However, it is good to also treat the hyperhidrosis. There are several methods that are non-prescription. First, try any good anti-perspirant (not deodorant). Patients need to apply it at bedtime. If they apply it in the morning, it washes away with the sweat before it can do any good. It is a daily application.

Black tea soaks are also occasionally effective, especially for those patients who are attuned to always trying something "natural" first. The tannin is the effective ingredient in tea, and it is highest in black tea. Of course, there is also Drysol, and other prescriptions are available, along with oral anti-cholinergics, as needed, and iontophoresis. Botox can also be used if you or a local dermatologist are trained in its use.

The diseases described would only be transmissible if your employee's blood got into a patient's system. The only scenario would be if the employee assisted in surgery and punctured herself during the procedure. If proper universal precautions are used, the chance of infecting a patient would be remote. If, however, the employee was assisting in surgery and punctured herself during the procedure, the patient would have to be informed of the exposure and be treated immediately.
 
The other question is whether a surgical assisting staff person could be suspended, fired, or even have her work assignments changed because he or she has an infectious disease such as hepatitis or HIV. For that you have to heed Dr. Block's advice and find out what the laws are in the state in which you practice. Your state podiatry society might also be a source for this information.
