TPMF Mission Returns to Mexico
The Texas Podiatric Medical Foundation (TPMF) returned to San Miguel de Allende, Mexico Aug. 26-31. The mission team consisted of 30 volunteers. The mission had many logistic changes post-covid.

Despite challenges, TPMF volunteers saw 600 patients and provided 2,000 orthotics donated by Forward Motion. They also distributed over 100 diabetic shoes donated by Dr. Comfort, and performed 20 reconstructive surgeries and provided medicines and shoes for orphanages.

Dry, Flaky Feet Could Indicate Thyroid Problems: IL Podiatrist
If your feet are dry and flaky, it could be thyroid problems, especially if moisturizer doesn’t help. When the thyroid gland (the butterfly-shaped gland at the base of your neck) goes on the fritz, it doesn’t properly produce thyroid hormones, which control metabolic rate, blood pressure, tissue growth, and skeletal and nervous system development. "Thyroid problems cause severe dryness of the skin,” says Marlene Reid, DPM, a foot specialist at Family Podiatry Center in Naperville, Illinois.

“When we see cracking on the feet, or if moisturizer doesn’t improve dryness over a few days, we usually refer patients to their primary doctor to make sure their thyroids are okay.” Brittle toenails can also signal thyroid complications. Thyroid problems are more common than you might think— according to the Cleveland Clinic, they impact around 20 million people in the U.S.— so it's always worth a conversation with your doctor if you see this warning sign.

Goldfarb Foundation Hosts Another Successful Meeting in Montana
52 attendees took advantage of the opportunity to obtain 12 CME credits in Missoula, Montana during The Goldfarb Foundation’s Annual Montana meeting. Attendees enjoyed fly fishing during downtime, networking, and a subset of the group gathered for an informal reunion with Temple University School of Podiatric Medicine’s Dean John Mattiacci, DPM.

What to Consider Before Starting a Concierge Practice
A concierge practice can be a lucrative way for a provider to achieve work-life balance by reducing the number of patients, increasing time with patients, and making great profits. But before you transition, speak with an experienced attorney who can help the transition.
Here are some possible pitfalls:

Spend time with your attorney reviewing your patient agreement to ensure it does not violate state law.
Some provider arrangements may appear less like patient-provider contracts and more like an insurance agreement. In most states, insurance laws have several requirements before any person can offer this to any patient. Thus, it’s important your attorney reviews your state’s insurance licensure laws and provides you a summary of this review before drafting a provider-patient agreement.

RE: Recruiting in Podiatry

Every industry is facing shortages of every kind in the work force. The challenges are compounded by the non-stop flow of government money. We have seen what this does to the economy, but more so how it impacts each industry. This shortage is critical in medicine as it creates access issues, delays in care, and unfortunately, complications in medical treatment.
For those podiatrists who have opted to partner with a recruiter, know that although this may open doors, it is also short sighted. I am constantly getting calls and emails with recruiters having 3-5 candidates who are lined up ready to interview. These recruiters are charging employers 15-20%, if not more, on the negotiated one year salary of the physician. This can impact your salary. First, the employer is motivated to pay you less in order to decrease the commission amount to the recruiter. Second, each practice has a budget to pay a salary. With a commission looming, the employer will need to pay less in order to stay within budget. This limits many other job offers that could potentially be better. Why give this commission to recruiters (head hunters)?
Why not ask for a signing bonus from the employer and pocket the money yourselves. As an employer of a medium sized practice looking for 1-2 new associates, I would much rather create a better relationship with my associate with a signing bonus or higher salary than deal with the sales pressures of a recruiter. Think about this when you are considering a position change.

Thanks, Dr. Levy, for posting this information. Is there a way that we can take such a course, online? Could you organize such a course? Medical disinformation is out there, getting worse and is a genuine threat to public health. In New York State, where I live, we are dealing with communities that will not vaccinate their kids for measles and mumps and now polio. This year, polio raised its ugly head in upstate New York for the first time in over half a century. This is on top of frequent outbreaks of measles and mumps because the people who will not vaccinate their kids are convinced, based on misinformation, that vaccines cause autism.

When it comes to so-called “medical misinformation,” there is always the same question: “Who is the absolute authority?” During the years of Dr. Ignatz Semmelweis, the ultimate authority did not feel it necessary to wash hands between examining and treating female patients in childbirth. Dr. Semmelweis was hounded into a mental institution by greater powers who disagreed with him. We now have political leaders who believed in vaccines as the answer to Covid. As it turned out, the Chinese product did not work very well. So-called "natural immunity" was relegated into "medical misinformation." We are still dealing with issues of possible spike protein damage (from mRNA vaccines) to a cadre’ of young men who were not particularly at risk of death from Covid in the first place.

Dr. Shavelson wrote “Podiatry cannot remain tagged to the biomechanical and orthotic genius of Dr. Root and the gifts he gave us 45+ years ago. Subtalar joint neutral, rearfoot valgus, and over-pronation have had their runs”. He also said, “We have no proven definition of our orthotics, how to cast them, and what their goal sets. Part of my father’s “genius” was in recognizing that in order to make foot orthotic therapy more scientific, there needed to be terms to describe motion, position, structure, and function of the foot. Dr. Root set out to do just that.
Dr. Root developed the concept of the neural position of the subtalar joint. This enabled him to differentiate between a foot that was supinated and a foot that was pronated. Dr. Root then began to compare feet and foot structure by placing the STJ in the neutral position and the midtarsal joint in a fully pronated position. Using this standard positioning technique, he was able to classify...
