Regardless of age or ability, almost all runners would like to improve. They would like to run faster. Few of us are athletically gifted enough to compete in an arena, but we all want to enjoy our sport and avoid the pitfalls of poor training, which often result in overuse injuries that keep us from running on a daily basis.

Easier said than done, but injuries can be minimized, if not entirely avoided. Is there one best way to avoid running injuries? Listen to Megan Leahy, DPM, a Chicago podiatrist: “Investing the time into a gait analysis and running shoe fitting on the front end can hopefully keep you out of the podiatrist’s office. Many of the injuries I see can be traced back to worn-out shoes or the wrong shoe for your foot type.”

IL Foot and Ankle Center Marks 20th Anniversary
 
2016 marks the 20th anniversary of the Foot & Ankle Centers of Yorkville (Drs. Paul Bishop and Tom Rappette). Dr. Bishop completed his residency training in foot and ankle surgery in Houston, Texas, and has been practicing in Illinois since 1996. Being a lecturer on complex infections, external fixations, and pediatric and adult foot deformity, he also serves as a guest teaching faculty member and surgeon. Shriners Hospital for Children in Mexico City, Mexico, and the University of Des Moines, IA, hosted him in 2015.

Dr. Rappette was residency-trained in Houston, Texas, and has extensive training in pediatric and adult flatfoot surgery, sports medicine, and foot and ankle trauma. His special interest of pediatric flatfoot reconstruction and advanced treatment heel pain was displayed and practiced in 2015 during a Haiti mission trip. While in Haiti, Dr. Rappette was able to treat many of the foot and ankle conditions affecting the adults and children of the village.

DC Podiatrist Keynotes at Conference in Melbourne, Australia

John Steinberg, DPM was featured last week in Melbourne, Australia as the Keynote Speaker at the Lower Extremity Amputation Prevention Conference 2016. The conference was jointly organized by the Advanced Practicing Podiatrists High Risk Foot Group and the Australasian College of Podiatric Surgeons.

Lecture topics presented by Steinberg focused on the team approach to limb salvage and surgical offloading of the compromised and deformed lower extremity. Dr. Steinberg is Professor at Georgetown University School of Medicine and Residency Director at MedStar Washington Hospital Center in Washington, DC.

IL Podiatrist Conducts Midnight Medical Review Marathon 
 
In preparation for the USMLE step 1, a midnight marathon review session was held for medical students in Chicago. Conducted by Sev Hrywnak, MD, DPM, the 8-hour review session covered all basic biomedical sciences. “The lectures are all 20 minutes per topic” explained Hrywnak, “The use of short lectures, videos, computer-generated medical problems, and clinical cases helps the students retain information.

The days of sitting in class for a 2-hour lecture in a professional medical education program are gone. Medical education has to be restructured in order to catch up with 21st century technology,” added Hrywnak. The 52 students who attended the marathon session came away feeling confident that they will perform well on their USMLE Step 1 examination.

MACRA UPDATE
New MACRA Rules Make Medicare Pay Cut in 2019 Less Likely
 
Under pressure from organized medicine, the Medicare program has drastically simplified what physicians need to do next year to avoid a penalty in 2019 under the Medicare Access and CHIP Reauthorization Act (MACRA). Medicare isn't delaying the new payment system outright, but it promises to do something close to that. CMS acting administrator Andy Slavitt announced in his blog that his agency would give physicians four MACRA options for 2017 that would let them pick their own pace of implementation — a very slow pace, if they choose. And each one would steer clear of a penalty in 2019.
 
The first MACRA option announced by Slavitt today, an option dubbed "test the QPP," allows physicians in MIPS to avoid the 2019 penalty by merely reporting some quality and cost data — "some" is undefined for now — in 2017. Under the second option, physicians in MIPS can submit quality and cost data for just part of 2017, and the performance period could begin later than January 1. Besides avoiding a penalty, these physicians could qualify for a small bonus in 2019. Submitting performance data for the full calendar year in 2017 is the third option, and Slavitt expects that many practices will be able to do this successfully. The fourth option is to join an Advanced APM, which, like the third option, has been available to physicians all along.

Query: Coverage: Medicare Advantage Plans
 
I was under the impression that Medicare Advantage plans must provide or offer the same level of benefits as traditional Medicare. I run into plans in New York City (e.g., UnitedHealthcare and VNS plans) that limit "routine care" to 4 visits per year. This is obviously less than the every 61 day coverage that traditional Medicare offers. Is this legal?

Response: In Pennsylvania, some plans, in ADDITION to the Medicare-required routine care for at-risk patients (diabetes, peripheral arterial disease, etc.), also provide a certain number of "routine care" visits for those NOT at any risk -- that benefit would be over and above Medicare Part B's allowance, and certainly would be legal. So are you sure that is not what you are experiencing?

I wholeheartedly agree with and commend you for seeking cost containment strategies in your practice. But please use some caution when performing due diligence on medical malpractice companies. While "one apple" may appear to look like the other apple, these illusions can result in extremely costly events and permanent damage to your reputation. Unfortunately, there are insurance companies who enter the podiatry medical malpractice insurance space only to temporarily boost their top line revenue. After doing so, they often disappear, leaving many physicians scrambling for new insurance. Heaven help those who are in the middle of battling a malpractice claim.
 
For your reputation's sake, it's essential to predict the longevity and sustainability of the insurance carrier's pricing model. How long have they been in business? What is their AM Best Company credit rating? Do they understand podiatry and the unique pricing that goes along with maintaining their sustainability? Are they part of a larger entity, not only for their own financial stability, but so that they might be able to also offer you other valuable protection, such as administrative defense coverage, cyber liability coverage, etc.? 
 
I can go on and on, and I think you might know where I'm going with this. I have absolutely no affiliation or financial relationship with PICA, but having a podiatry-specific, financially stable, and podiatry-committed carrier behind you should count for a "bushel full of apples" when doing a comparison.

Dr. Cowin cites an interesting case. It does and does not have relevance to podiatrists. In the case cited, a patient was given antidepressants and the following day went on to commit suicide. The court felt that the doctor could have done more for the patient's mental state than just change the medication.
 
What is relevant is that although we all see patients who have mental health issues, the treatment is out of our scope. The doctor described in the case was clearly addressing his patient's emotional disorder. Even though we prescribe antidepressants such as Cymbalta, we are using it to treat foot pain, not clinical depression. Hence, we could not be held liable for not properly managing a mental health disorder in our practices.
 
I frequently refer patients to psychiatrists, clinical psychologists, and psychiatric social workers. Unlike referrals to surgeons or endocrinologists, a mental health referral has its own set of challenges. Utmost time and professionalism is essential because if a patient thinks that you are accusing him or her of "being crazy", he or she will not only decline your advice but might take leave of your podiatric services as well.

RE: Remembering 9/11

New York Giants wide receivers Odell Beckham Jr. and Victor Cruz are preparing to kick off the season in patriotic style. Beckham and Cruz plan to wear special 9/11 cleats before and during Sunday's season opener against the Dallas Cowboys at AT&T Stadium. They each have a pair for pre-game and during the game that serve as a tribute to the 15-year anniversary of the Sept. 11 tragedy.

The cleats by Kickasso like Picasso will include American flag versions for the game. Beckham will wear a special 9/11 cleat during warmups, while Cruz will have a fire truck cleat with his late father's engine number on them. Cruz's dad was a fireman in Paterson, New Jersey.
