CA Podiatrist Discusses Nail Health

“Toenail health is important because nail thickness can cause pressure in the shoes, which can hurt,” said Henry Shin, a doctor of podiatric medicine in National City. “If you leave the nail too long, there is a chance for the nail to get into the corners of the skin and cause ingrown toenails.” Discoloration is a surefire sign of an infection. If your nail is black or blue, it could be a sign of bleeding underneath, according to Shin.

There are ways to care for unhealthy nails outside a podiatrist’s office. Shin offered a few tips with things that can be found around the house or with a quick trip to the local drugstore. “I tell patients to get an old toothbrush and use a topical antifungal cream and scrub the nail plate down to get medicine all over the area,” Shin said, advising not to mix it up with your regular toothbrush. “Other home remedies are to use a bowl full of warm to hot water with a few capfuls of vinegar and soak the toes for five minutes.”

Bruce R. Saferin, DPM has been appointed to the Ohio State Medical Board, effective 9/10/13 by Ohio Governor, John Kasich. His term will run through December 27, 2017.

NYSPMA Cautions Consumer to Be Aware of Healthcare Insurance Changes

The New York State Podiatric Medical Association (NYSPMA) is urging all consumers to examine their coverage and their choices carefully. "Patients who are being treated by a podiatrist for diabetic ulcers, arthritis, plantar fasciitis and many other conditions should pay particular attention. Those who have switched coverage to lower costs are often surprised to learn their podiatric office visits are not covered under their new plan," said Dr. Robert Russo, President of the New York State Podiatric Medical Association. "This can be a cause of great stress," he added.

Palm Beach County Conference Tallies Largest Number of Attendees Yet

The 8th annual "Toes in the Sand" conference was held August 23-25 at the Eau Palm Beach Resort and Spa (formerly the Ritz-Carlton Palm Beach) in Manalapan, Florida. Over 150 podiatric physicians from around the state participated in lectures and family activities, making this the eighth consecutive year the attendance record was broken.

The conference, sponsored in part by the Palm Beach County Podiatric Medical Association and Barry University, gave attendees the opportunity  to earn 10 CME credits and enjoy the end of summer at the beautiful, 5-star, oceanfront resort. Nationally known lecturers including Bradley Bakotic, DPM, DO, Stephen Barrett, DPM, and David Davidson, DPM were among many providing insight to those in attendance.

HIPAA UPDATE

Deadline for Updating HIPAA Privacy Notice Is September 23

Medical practices that have not added new patient rights to their privacy notices, as required by the federal government, have until September 23 to become law-abiding. These new rules became effective as of March 25, 2013, but an extension was provided requiring compliance by the September 23 date.

Instead of rewriting their current NPPs, practices can download an updated and customizable NPP from the websites of state and national medical societies.

I have written previously about a patient with severe pain S/P a plantar fasciotomy. A local nerve block confirms the nerve origin of his pain. We are considering radio frequency ablation. I have seen about 10 of these lesions in the last 35 years. They are usually on the second toe and many times bilateral. I have had great success doing an MIS partial plantar proximal phalangeal head resections, and occasionally an MIS proximal phalangeal head resection. As far as I know, everyone of these surgeries has been successful in getting rid of these lesions.

What is disconcerting about this case history is that it appears that after futile attempts at debridement of the lesions, deep skin followed by tendon and osseous surgical procedures were chosen. Why wasn't a biomechanical exam performed along with an attempt at the judicious use of orthotics? The lesions are symmetrical and on weight-bearing surfaces and if abnormal biomechanics turns out to the culprit, short of amputation, no surgery will alleviate this patient's problems.

RE:  Effective Treatment for Hyperhidrosis (Billie  A. Bondar, DPM)

Bromhidrosis is caused by bacteria forming on the skin of the foot.  One of the protocols we have worked out for our office is as follows.
 
1. Soak both feet in a 50/50 solution of rubbing alcohol and water for ten minutes twice a day. 
 
2. Purchase Certain Dri pads (or like product) and apply to feet every morning.
 
3. Follow the Certain Dri with a spray deodorant.
 
4. Apply an absorbent powder to feet and shoes. We recommend Zeasorb-AF powder.

I agree with all the previous excellent advice. I suggest considering the Artassist device. One may also consider L- argentine in topical or oral forms in lieu of nitroglycerin.

I think it is irrelevant that this occurred under the care of an orthopedist. The same outcome could of easily occurred to a podiatrist. I think the case is horrible and pray the patient gets better.

Again, we have seen a recent publication of a podiatrist who will serve prison time due to a fraud conviction. His life and family are devastated. Healthcare auditors and prosecutors now regard podiatrists as one of their most accessible and easy sources for personal career advancement. All they have to do is generate computer printouts on repetitive Medicare claims for ingrown nail surgeries, especially in nursing homes. They concentrate on those who have the greatest numbers.

Then, they ask for your chart notes. Sometimes they just go right to the NHs and start interviewing patients. Incredibly, their....
