Roll-up Ballet Flats Not Designed for Walking: CA Podiatrist

Those cute little roll-up ballet flats women carry in purses to slide on during foot-pain emergencies have rapidly achieved most-favored streetwear status among millions — and worst-footwear status among podiatrists. When used only for their intended purpose — worn just a few minutes as relief from aches, or while deskbound or on a plane — they're not terrible, foot specialists say.

But it's a problem when women wear the slipperlike coverings, made by several companies and selling for $8 to about $35, as regular shoes. "There's no structure, essentially no sole, they're too flat, and there's no support," says Dr. Michele Colon, a podiatrist and shoe blogger in El Monte, CA.

MPMS Elects New Officers for 2011-2012

Scott Aronson, DPM has been elected president of the Massachusetts Podiatric Medical Society (MPMS) for the 2011-2012 year. Dr. Aronson is a past chairman of the MPMS marketing committee and also serves as secretary/treasurer of the APMA Region One Conference and secretary of the MPMS Community Service Foundation, the non-profit, public health and education arm of the MPMS. Dr. Aronson has been in private practice in Stoughton, MA for the past 12 years.

James DiResta, DPM has been elected to the president elect position and Alfred “Jay” Phillips, DPM became immediate past president. Susan Walsh, DPM was elected as vice president and Robert J. O’Leary, DPM was elected to fill the secretary/treasurer position. Frank Campo, DPM, Dominick Garibaldi, DPM, Barry Rosenblum, DPM, Harry Schneider, DPM, and Louis DeCaro, DPM, all members of the 2010-2011 board of trustees, were elected to continue for 2011-2012. They are joined by new board members Emily Cook, DPM, Jeremy Cook, DPM, Caroline Gauthier, DPM, Thanh Dinh, DPM, and Michael Theodoulou, DPM.

GA Podiatrist Owns Medically-Staffed Salon

Dr. Alap P. Shah, a podiatrist, is passionate about his work.While he takes care of all kinds of foot problems, he specializes in diabetic foot and wound care. He’d rather not have to take care of those problems, however. Shah wishes more people would visit a podiatrist before they develop severe problems.

Shah owns a salon called Footique. It is is the only podiatrist-owned salon in Columbus. "When you have an establishment affiliated with medical personnel, people feel more comfortable and safer. We make sure all the instruments are cleaned properly in an autoclave. My goal is to franchise this in Auburn or Newnan. I think little cities get gypped," says Shah.

QUERY (CLINICAL)

Query: MTPJ Stabilization with AVN

A couple of years ago, I performed an Austin bunionectomy on a 50 y/o female who had HAV and an AVN of her third met head. I remodeled the third met head. I chose not to replace the joint due to my less-than-stellar experiences with lesser MTPJ implants. I performed a chylectomy/exostectomy to get the third MTPJ moving and pain-free. This worked initially, but now the patient has laterally-deviated second and third MTPJs, mainly in the transverse plane. The hallux remains in an acceptable position. The third MTPJ is not really painful. There is limited but pain-free motion at the third MTPJ- it looks much worse on x-ray than it is clinically.

I am leaning towards a lesser MTPJ joint replacement for the third met, and soft tissue balance procedures for the second and third mets. Are there any lesser met head replacements that don't stay swollen for years? How about a soft tissue interposition with about eight weeks of K-wire immobilization? Suggestions welcome.

RE: Support Homeless Veterans

I would like to report some good news. Dr. Michael Rushton of Oregon this weekend donated a 15-passenger Chevy van to the Madison Street Veterans Association in Phoenix. I also received $17,200 in donations this weekend, the 10th anniversary of 9/11. The Madison Street Veterans Association (Marine, Army, Navy, Air Force) houses 52 homeless veterans in downtown Phoenix. I volunteer there two days a week and am proud to report I have raised over $80,000 and obtained three vehicles for the shelter this year.

I invite any members of APMA to donate goods or cash to our benefit, or simply serve as as medical adcvisor to your local veteran shelter. There are 119,000 homeless veterans in the United States. Veterans are 1% of our population, but comprise 30% of homelessness. Also they are triple the national average in joblessness. Never forget!

As an interesting side note to professionalism: Dr. Jeff Schmidt has written that the process of professional training, while it appears to be ideologically neutral, is in fact biased towards those with superior class background and formal education, and more conservative political opinions, who are unlikely to challenge the orthodoxy of the profession. His 2000 book, Disciplined Minds is subtitled "A Critical Look at Salaried Professionals and the Soul-Battering System that Shapes their Lives." Schmidt begins his attack on professionalism by observing that qualified professionals are less creative and diverse in their opinions and habits than non-professionals, which he attributes to the subtle indoctrination and filtering which accompanies the process of professional training. His evidence is both qualitative and quantitative, including professional examinations, industry statistics, and personal accounts of trainees and professionals.
 
Podiatry is one of my forms, not my substance; it is what I do, not who I am. I would be cautious to not advise being "professional" in all aspects of living as this can easily become pathological behavior. We've all witnessed brilliant and highly "successful" physicians and other professionals whose personal lives are train wrecks. Attempting to apply love for my fellow man, while living in the present moment, has been working for me, and has kept me adaptable, creative, and genuine in my relationships. Practice, as life, has been a journey, not a destination.

RE: APMA Nixes MD/DO Referendum (Elliot Udell, DPM)

Dr. Udell, first, a degree designation is not given...it is EARNED. Podiatry's biggest fear shouldn't be our colleagues wanting to leave our profession; it should be the establishment of a sense of entitlement to practice outside current limited licensure scope, i.e., practicing primary care medicine, once the initials are changed.

To that end, while I support any advancement our profession achieves that fosters attainment of its rightful equitable place in the medical marketplace, from a public safety perspective, the concept of a limited licensure scope commensurate with the DPM degree must be acknowledged, respected, and adhered to throughout our profession. To those wishing to practice primary care medicine, currently, your choice is simple; go back to medical school, or utilize available on-line pathways and EARN that degree.

Now...if our profession wants to expand its scope of practice via a degree designation change, it MUST...
