REMEMBERING 9/11
NY Podiatrists Recite at the Calling of the Names Ceremony

In 2016, volunteers from the 2001-2002 St. Paul’s Chapel World Trade Center Relief Ministry created the first Calling of the Names Ceremony. Their intention was to honor all deceased 9/11 Rescue and Recovery Workers and Volunteers by having a ceremony similar to the National September 11 Memorial and Museum’s annual ceremony. They felt it meaningful to do so on the anniversary itself, and at St. Paul’s Chapel where many responders found respite.

(L-R) Drs. Kathy Reilly Fallon and Wilma Hunt-Watts

On this, the twenty first anniversary of the September 11th attacks on our nation, Kathy Reilly Fallon, DPM, DM, LHD and Wilma Hunt-Watts, DPM were honored to recite the names at the “7th Annual Calling of the Names Ceremony.” These podiatrists served as volunteers at St. Paul’s Chapel and rendered foot, ankle, and wound care to the NYPD, NYFD, Iron/Steel workers, and other recovery personnel.

PODIATRISTS AND PROFESSIONAL DISCIPLINE
KY Podiatrist With Recent Drug, Alcohol Problems Allowed to Practice

A Louisville foot doctor is still practicing after admitting dependence on alcohol and abuse of prescription drugs, while facing several lawsuits alleging negligence in treatment. Dr. Timothy Hanna's license was briefly suspended last month in an "emergency" action of the Kentucky Board of Podiatry after it received complaints alleging Hanna had practiced under the influence of alcohol and drugs, with evidence including documentation of surgical results, pictures, and x-ray images, according to documents obtained under the Kentucky Open Records Act.

Hanna's suspension didn't last long, however. A week later, on Aug. 19, the board and Hanna reached a settlement in which Hanna agreed to five years of monitoring by a recovery program and to obtain a psychological evaluation within the next two months. Hanna also agreed to pay $1,000 in fines. His license was placed on probation, but he is still allowed to practice as normal.

PRACTICE MANAGEMENT TIP OF THE DAY
What to Consider Before Starting a Concierge Practice

A concierge practice can be a lucrative way for a provider to achieve work-life balance by reducing the number of patients, increasing time with patients and making great profits. But before you transition, speak with an experienced attorney who can help the transition.

Here are some possible pitfalls:

1. Patient agreementsThe provider will create and enter a patient agreement, where the patient agrees to pay all fee schedule charges at the time of service for access to a specific menu of enhanced services.
  • Some patients cannot afford the services under the patient agreement.
  • Some patients will still try to pay with insurance instead of the patient agreement.
  • Understand and review the payor agreement to avoid breaching the contract, and refrain from offering concierge services that may be covered by a third-party payor agreement.

CODINGLINE CORNER
Query: Billing For Local Anesthetics With Procedures

I’m in an ongoing debate with my biller. I have always been under the impression that when billing for infected, ingrown nails that local anesthetics were considered part of the procedure. My biller thinks I am able to charge for them. What say you?

PM News Subscriber

Response: Local anesthesia and associated supplies (needle, syringe, solution, etc.) are always included in the overall procedure allowance. The next question is why did your biller suggest this? It is a good idea to review billings from your office or billing service. Remember that any mistakes made are your responsibility!

RESPONSES/COMMENTS (CLINICAL)
From: Andrew I. Levy, DPM, William Egelston, DPM, John Giurini, DPM, et al.

These are hard to identify from the picture, but my guess based on the 2nd most proximal FB is that they look like mini vascular clips that could have been used in the harvesting of the ray for transplant.

Andrew I. Levy, DPM (Retired), Jupiter, FL

Looks like vascular clips to me. Probably used when harvesting the 2nd ray for the hand.

William Egelston, DPM, Modesto, CA

Those are vascular clips. They are commonly used by vascular surgeons and plastic surgeons to ligate blood vessels.

John Giurini, DPM, Boston, MA

Editor's note: We would like to thank Drs. Bret Ribotsky, John Throckmorton, Rick Allen, and Paul Hilbert for similar responses.

RESPONSES/COMMENTS (MEDICAL EDUCATION)
RE: University of Chicago Medical School Offers Class on Misinformation
From: Leonard A. Levy, DPM, MPH

Especially during the COVID-19 pandemic, doctors’ voices have sometimes been drowned out by social media users who blast misinformation across the globe, leading patients to make questionable, and sometimes dangerous, choices about their health. (Diabetes ProSmart Brief, September 9, 2022).

The University of Chicago Pritzker School of Medicine provides a new class to equip doctors and other medical professionals to battle medical misinformation. The medical school began offering the class to medical students and residents last year and has developed a condensed version for others studying to be health professionals. Sara Serritella, one of the instructors, who also serves as director of communications for the UC Institute for Translational Medicine said, “As we saw during the pandemic, this whole crisis of having to communicate science in a way that builds...

RESPONSES/COMMENTS (PM ARTICLES)
From: Dennis Shavelson, DPM

Podiatry cannot remain tagged to the biomechanical and orthotic genius of Dr. Root and the gifts he gave us 45+ years ago. Subtalar joint neutral, rearfoot valgus, and over-pronation have had their runs. Podiatric promises that our orthotics cannot fulfill have dampened our orthotics in the eyes of other professions, the insurance companies, and patients as well. No podiatric orthotic places a patient in subtalar joint neutral. We have no proven definition of our orthotics, how to cast them, and what their goal sets. “Happy patients”, a doctors promise of success and anecdotes do not promote insurance acceptance.

Happy Feet gets $1,000 in cash for their OTC orthotics, so I dispense devices that are uncovered by all insurance because I want more income streams that I professionally own and control. My inserts are guaranteed to holistically provide stability, support, strength, symmetry, and balance to a patient's feet, not just reduce pronation and relieve heel pain. We need labs that do not accept “post to cast” as an Rx for a device with a 3 degree rearfoot post as correction. We need to provide a cut and paste biomechanical algorithm between doing nothing and foot surgery that has great evidence-based outcomes.

I retired as a DPM last year and practice as a certified health coach and CPed. I am cash only and am proud to state that I am podiatry-trained and experienced in biomechanics and human movement, but I am no longer a podiatrist.

Dennis Shavelson, DPM (retired), CPed, Tampa, FL
