Exercise Caution When Using Nail Salons: NY Podiatrist
 
“Pedicures are completely fine as long as the salon is hygienic,” says Miguel Cunha, DPM, podiatrist and founder of Gotham Footcare in Manhattan. “I suggest ensuring that the pedicurist uses steriled instruments on your foot to ensure you don’t contact any fungus or bacteria that could lead to athlete’s foot or plantar warts.”

Experts like Dr. Cunha say that before you get too comfortable in a space, keep your eye out for anything that could be a breeding ground for bacteria, as the organisms can cause damaged toenails and cuticles. Cunha doesn’t recommend allowing your cuticles to be cut because your cuticles provide protection from potential infections and ingrown nails.

PODIATRIC PRODUCTS AND SERVICES IN THE NEWS
 3 Wound Care Quality Measures Now Included on CMS Physician Compare Website
 
Three wound care quality measures developed by the Alliance of Wound Care Stakeholders and the US Wound Registry (USWR) are now included on Centers of Medicare and Medicaid Services’ (CMS) Physician Compare website following a summertime content update that expanded quality performance data on the site.
 
The USWR quality measures selected for Physician Compare reporting are:
  • 1. “Adequate off-loading of diabetic foot ulcer at each treatment visit.”
  • 2. “Adequate compression of venous leg ulcers at each treatment visit.”
  • 3. “Vascular assessment of patients with chronic leg ulcers.”

Here are some strategies to help you re-focus and enhance your patient interactions:
 
5. Ask open-ended questions 
 
Open-ended questions elicit the patient’s story. That may include, “Tell me what happened,” “Go on,” or “Help me understand.” At the beginning of the visit, we cast a wide net of open-ended questions to build rapport, create a dialogue and gather information. As we begin to understand the problem or concern, we can shift to closed-ended questions to fill in the gaps in our understanding and construct a differential diagnosis.

Query: Code For Open Osteochondral Drilling of the Talus
 
What code should I use for an open osteochondral drilling of the talus? All I can find is a code involving a scope.

Response: An open osteochondral drilling of the talus would fall under the CPT codes for arthrotomy of the ankle. There are several codes that may be used:
 
CPT 27610 Arthrotomy, ankle, including exploration, drainage, or removal of foreign body
CPT 27620 Arthrotomy, ankle, with joint exploration, with or without biopsy, with or without removal of loose or foreign body
CPT 27625 Arthrotomy, with synovectomy, ankle
CPT 27626 Arthrotomy, with synovectomy, ankle; including tenosynovectomy
 
One other code that “might” be applicable is CPT 28120 Partial excision (craterization, saucerization, sequestrectomy, or diaphysectomy) bone (e.g., osteomyelitis or bossing); talus or calcaneus

Thank you, Dr. Levy for shining a light on such an interesting topic. We should all weigh in on this!
 
Podiatrists very often see patients more frequently than their primary care medical doctors. We also take care of patients who do not have primary care general physicians. In both cases, should we be asking more questions on a regular basis regarding certain aspects of their general medical history. We ask patients about diabetes because it relates to foot pathology. Should we be asking patients if they have been vaccinated for measles and other illnesses?
 
What about asking patients past the age of 50 if they have had a colonoscopy? What about asking all post-menopausal women if they have had their first bone density scan? There is more we can ask and in so doing we can make an even greater contribution in the public health arena. By referring these patients to appropriate specialists, we would also increase our professional relationship with a host of other medical professionals.

Although the day has each of us remembering where we were and how we felt, one factor for me will always be that phone call. I respect your repeating the raw emotion in the editorial so that we never forget.

I just read Michael Brody’s article in the September issue of Podiatry Management. As a leader in the podiatry world, podiatrists look to Podiatry Management as the ‘source’ for all things podiatry and they rely on the information provided. I have to point out several factual errors and outright wrong advice that he has provided to the readers in Michael Brody’s recent article.
 
First, in the first column Michael states that “Both LabCorp and Quest are ‘covered entities’ in that they do not have a direct relationship with patients.” This is exactly the opposite of the...
