High Heel Wearers Can Take Sensible Steps to Avoid Injury: NY Podiatrist 
 
While many believe the right pair of shoes can make or break an outfit, podiatrist Dr. Suzanne Levine says high heels have the potential to break something more serious. According to Dr. Levine, there is a laundry list of issues that arise when women wear high heels; like lateral ankle sprain, stress fractures, worsening of bunions, hammertoes, neuroma pain, and burning under the balls of the foot.

Dr. Levine said that while it’s not necessary to kick off the heels just yet, wearers should plan accordingly. “[Take] another pair of shoes with you. If you are going to wear that four-inch heel for dinner, you can’t expect to wear that all day if you’re running around the office,” she said. She also said there are stretching and strengthening exercises that can be done to prepare feet and ankles for high heels.

Pittsburgh Pays Podiatrists Top Dollar in U.S. 
 
Eds and meds are frequently cited as defining Pittsburgh’s economy and primary areas of expertise, so it’s little surprise that highly educated professionals in the medical sector are drawing the healthiest pay here. Surgeons had the top-paying profession locally, bringing home an average annual salary of $238,500 in 2014, according to a 106-market study of U.S. Bureau of Labor Statistics conducted by American City Business Journals, the parent company of the Pittsburgh Business Times. In total, eight of the 10 highest-paying jobs in the region were in the medical sector.
 
The healthcare job where Pittsburgh pays top dollar in the U.S. was podiatrist. There may only be 140 such jobs in the region, but with an average income of $175,960, there are good reasons to put this career on a short list. Podiatry also ranked No. 7 among the highest-paying jobs in the region.

Query: Call Center
 
Our group is considering starting our own call center to handle incoming phone calls. Currently, the front staff (in multiple locations, for multiple doctors) answers the phone. They do this while doing other tasks (check in, check out, etc.). Adding a call center would require us to add at least 2 more employees. Has anyone done this? Has it been beneficial?

Query: Medicare Provider Address Change
 
I need to change my Medicare provider address, but I want to still bill Medicare. How much time can I hold my claims for Medicare before they are void? Is it one year?

Response: You have one year to submit a claim. 
 
That said, once you change address, that becomes effective immediately. Medicare will not forward any mail, and any mail returned to them will result in your account being frozen. I would clear the deck with your claims before the switch date or you can get caught up in a very sticky mess (translated: no payments for a long time).

The poster appropriately aspirated the lesion in question, clinically presenting as a cystic mass. It's hard to understand how the pathologic description listed in the post leads to a diagnosis of "low grade fibrous neoplasm." Low grade what? Grade usually is stated in association with malignancy, OR cellularity. But with an acellular specimen, the query as posted is confusing. First and foremost, it has to be determined if there is a solid component to the lesion. This could be accomplished with Ultrasound or MRI. Plain films also should be done.
 
I would never inject steroid until definitive pathology is known. In the case of a ganglion, cytology studies of aspirated fluid tend to yield a report that at least indicates that the specimen is consistent with a ganglionic cyst. At that point, I think a steroid injection is appropriate. Any solid mass must be biopsied before ANY treatment intervention is planned. The next issue of Foot and Ankle Quarterly (DataTrace) thoroughly addresses this issue.

We use Office Ally for billing, together with Practice Fusion as our EHR. Supposedly, you can just upload your superbills to Office Ally in ANSI format and send. Unfortunately, PF does not export superbills in that format, so we have to manually enter the info into Office Ally. Once you input the CPT codes, description and fee one time, there are drop-down menus for subsequent visits, making it pretty quick and efficient for billing. Also, it tracks your A/R allowing you to enter payments against charges, create invoices, and collection reports. Best of all, if <50% of your claims are gov't (Medicare/Medicaid), it's free! Otherwise, it's a reasonable $20/month.

I had a problem with the collimator on my x-ray machine, and it turned out to be a problem with the circuit board. If replacing the bulb doesn't help, then look there next. That issue took a professional repair person, and my machine did not pass inspection until it was fixed.

APWH certainly provides strong support for podiatrists. There are a few issues involved here. First of all, Medicare currently does not reimburse biomechanical/gait analyses performed by podiatrists. Secondly, this functional assessment requires more than the gait analysis. For this proposed LCD, we should ask for podiatrists to be allowed to perform the orthopedic/gait analysis in collaboration with the patient's primary care providers.
 
Alternatively, we could request for the partial/total foot amputations to be exempt from this functional assessment. In the long term, we should work for getting all podiatrists approved to perform H&Ps, and to perform and be reimbursed for the biomechanical/gait analyses.

I was very pleased to see the APWH response to the recent draft LCD attempting to eliminate podiatric care for millions of Americans at risk for or suffering from amputations of the lower extremity. Podiatric physicians have been inadvertently left off of the definition of physician which we have enjoyed for many years now. Podiatrists have been doing comprehensive lower extremity assessments on diabetic, vascular, arthritic, and traumatic patients at risk for or suffering from amputations for many years. They are an integral part of the toe and flow model for limb salvage which includes a podiatric physician and a vascular surgeon.
 
Podiatrists are experts in wound healing and partial foot amputations as well as experts in biomechanics of the foot and ankle. This is a skill not shared by many physicians and an essential skill to have when assessing or prescribing prosthetics and braces of the lower extremity. The LCMP definition needs to be amended to include podiatric physicians and include them in any wording regarding DMEPOS. Also, they should eliminate the term "natural gait" from the document, and if anything, increase the benefit for prostheses and ambulatory assistive devices, not restrict it.
