REMEMBERING 9/11

Editor’s note: On this, the 24th anniversary of 9/11, we continue our proud tradition of publishing the following note from the editor, which was published in PM News on 9/12/01 and circulated via the Internet throughout the world.

It is just after 3 AM here in New York City and I simply can't sleep. Looking out the window of my bedroom on the 17th floor, there is an unobstructed view of the Manhattan skyline. I search again for the once familiar sight of the two tall white buildings that marked the lower end of the skyscraper-filled island. All I see are some eerie search lights of the rescue crews.

Like everyone else, I have been in shock all day, particularly since I witnessed in real-time the morbid views of the second plane hitting the World Trade Center and the sequential collapse of both towers.

World Trade Center 9/11/01

The gamut of emotions runs from shock, fear, sadness, to anger. Barely one half-century after the Holocaust, despite all our technological advances, man remains uncivilized. The realization sets in that the terrorists who committed this crime could have and still could set off a nuclear bomb, killing millions. The safety and security we have taken for granted in America is and will never be the same.

My phone has been ringing all day. Family and friends call to determine that everyone is safe. My mom calls to tell me my brother was scheduled to be at the WTC at 9 AM and was at the subway station when the first plane hit. Had the plane hit five minutes later, he would have been on an elevator headed for certain death.

Unfortunately, many thousands were not so lucky, including many employees of Blue Cross/Blue Shield, which only a few years ago moved from midtown to this location. There will be many tears shed as the names of those murdered in this horrific disaster become known, many unnecessary funerals to attend. So many innocent lives lost for no reason. So many families destroyed.

Later this morning, my wife and I will donate blood and attempt to do what we can in this time of crisis. We know the entire podiatric community will collectively pray for those affected by this catastrophe and provide whatever support is needed.

AT THE COLLEGES

SMUCPM partners with Soccer Club to Highlight Podiatric Biomechanics

Official Higher Education Partner of the Oakland Roots & Soul soccer clubs, Samuel Merritt University College of Podiatric Medicine (SMUCPM) recently collaborated with Roots soccer players on a three-part “Boot Review” video series exploring foot health and performance in soccer.

(L-R) Dr. Tim Dutra discusses the effect of soccer cleat design on performance with Jurgen Damm, Roots soccer player.

Through a series of evaluations, SMUCPM  faculty worked closely with professional athletes at the University’s Motion Analysis Research Center to assess the functionality of soccer cleats made by the popular shoe brand, Charly. The resulting videos, which document the process of evaluating the cleats, have been featured on the big screen at the Oakland Coliseum during Roots games and appeared across SMU and Roots social media accounts. The first video in the series can be viewed here.

PODIATRISTS OVERCOMING ADVERSITY
Retired PA Podiatrist is Pancreatic Cancer Survivor

"On August 14, 2023, I received the shock of a lifetime: I was diagnosed with stage IIb pancreatic adenocarcinoma, says Eric Egelman, DPM. I began treatment at Northwell Health in Lake Success, New York, with  oncologist Dr. Anna Levy. Dr. Kara Raphael inserted a bile duct stent right away. I then started neoadjuvant chemotherapy with FOLFIRINOX. Once I completed my chemotherapy, Dr. Gary Deutsch performed a robotic-assisted Whipple procedure. After my surgery, my diagnosis was changed to stage III because the pathology report noted 10 positive lymph nodes. When I recovered from the surgery, I had adjuvant chemotherapy with FOLFIRINOX.

Dr. Eric Egelman

"It has been two years since my diagnosis, and I am alive (and active!). My Signatera™ molecular residual disease (MRD) tests and CT scans continue to show no evidence of recurrent disease post-diagnosis. I have traveled extensively to Europe, Mexico, and throughout the U.S. I am a retired podiatrist, and work part-time as a tour guide in New York City. I have been spending more time with family and friends—their support greatly helped me through this difficult time. I take nothing for granted. I’ve learned that life is fleeting, fragile, and funny," says Dr. Egelman.

PRACTICE MANAGEMENT TIP OF THE DAY
Ways to Refresh Your Front Desk for a Better Patient Experience

Here are some ways to refresh your front desk and strengthen both patient satisfaction and staff performance.

11. Make the space accessible

Accessibility is not optional, it’s a necessity. Beyond physical layout, practices can demonstrate inclusivity by offering multiple-sized blood pressure cuffs, gowns for different body types, and adjustable furniture. These considerations not only comply with accessibility standards but also communicate that all patients are valued. Physicians Practice highlights how inclusive design makes a practice more welcoming and trusted.

QUERIES (NON-CLINICAL)
Query: When an Orthopedist Tells Your Patient That Orthotics Are Bogus

I thought I had seen everything until today. A patient presented with plantar fasciitis, early bunions, and abnormal foot pronation. I cast him for custom orthotics. Today, he presented to the office all upset. He had gone to a local orthopedist for something else and ran past his foot history. The orthopedist told him that custom orthotics are bogus and he should get a refund from me and then go and buy Dr. Scholl’s at a kiosk at CVS.

I explained the differences between a Dr. Scholl’s OTC orthotic and what we made for him. How have others dealt with this problem? Would you give the patient a refund based on an orthopedist’s opinion on orthotics?

RESPONSES/COMMENTS (MEDICARE iSSUES)
RE: Flat Rate Payments for CAMPs
From: Paul Kesselman, DPM

As every medical professional seems to understand, CMS has put their stamp on the need to radically curtail spending on cellular tissue products. While this is laudable, a recent article in the Journal of Wound Care, entitled "Safeguarding access, fiscal responsibility and  innovation: a comprehensive reimbursement framework for CAMPs to preserve the Medicare Trust Fund" with headlined authors including our own David Armstrong, DPM and Vicki Driver, DPM points out that "An analysis of 2,023 Medicare claims data demonstrated that 26% of total cellular, acellular, and matrix-like products (CAMPs) spending came from just the top 10 providers in non-facility settings, compared to only 5% in facility settings. When expanded to the top 100 providers, the disparity becomes even more pronounced, accounting for an astounding approximately 64% of non-facility spending versus just 19% in the facility setting.

To put this in perspective, fewer than 3% of non-facility providers billing for CAMPs applications are responsible for 63.9% of all CMS spending in this category, while the remaining 97.3%...
