REMEMBERING 9/11 |
Editor’s note: On this, the 15th anniversary of 9/11, we continue our proud tradition of publishing the following note from the editor, which was published in PM News on 9/12/01 and circulated via the Internet throughout the world.
It is just after 3 AM here in New York City and I simply can't sleep. Looking out the window of my bedroom on the 17th floor, there is an unobstructed view of the Manhattan skyline. I search again for the once familiar sight of the two tall white buildings that marked the lower end of the skyscraper-filled island. All I see are some eerie search lights of the rescue crews.
Like everyone else, I have been in shock all day, particularly since I witnessed in real-time the morbid views of the second plane hitting the World Trade Center and the sequential collapse of both towers.

The gamut of emotions runs from shock, fear, sadness, to anger. Barely one half-century after the Holocaust, despite all our technological advances, man remains uncivilized. The realization sets in that the terrorists who committed this crime could have and still could set off a nuclear bomb, killing millions. The safety and security we have taken for granted in America is and will never be the same.
My phone has been ringing all day. Family and friends call to determine that everyone is safe. My mom calls to tell me my brother was scheduled to be at the WTC at 9 AM and was at the subway station when the first plane hit. Had the plane hit five minutes later, he would have been on an elevator headed for certain death.
Unfortunately, many thousands were not so lucky, including many employees of Blue Cross/Blue Shield, which only a few years ago moved from midtown to this location. There will be many tears shed as the names of those murdered in this horrific disaster become known, many unnecessary funerals to attend. So many innocent lives lost for no reason. So many families destroyed.
Later this morning, my wife and I will donate blood and attempt to do what we can in this time of crisis. We know the entire podiatric community will collectively pray for those affected by this catastrophe and provide whatever support is needed.

PAD Increases Risk of Heart Disease and Stroke: VA Podiatrist
According to the U.S. Department of Health and Human Services, Peripheral Arterial Disease (PAD) affects on average 8 to 12 million Americans. According to Dr. Mitchell Waskin, a podiatrist with The Foot & Ankle Center, "PAD is basically a clogging of the arteries. It can happen anywhere in your body, but it is more common in the lower extremities and the legs. As it decreases bloodflow, it can lead to gangrene and amputations."

"What's significant about PAD is that it doesn't just affect the legs. People who have peripheral vascular disease are also at a greatly increased risk of heart disease and strokes," says Waskin.

Change to Fall Shoes Often Leads to Bunion Pain: PA Podiatrist
For those suffering with bunions, shoes can aggravate the bunion, leading to foot pain when walking and the inability to wear fashionable shoes, says podiatrist Dr. Michelle McCarroll, owner of That Foot Doctor in Allentown.

Bunions are most often caused by an inherited faulty mechanical structure of the foot. It is not the bunion itself that is inherited, but certain foot types that make a person prone to developing a bunion. Bunions get aggravated in the fall once you transition out of sandals into an enclosed, narrow shoe that rubs on a bunion. “Poorly fitted shoes don’t cause a bunion, but certainly aggravate one,” McCarroll says.

VA, NPS, and AARP Recognize AL Podiatrist on Her 100th Birthday
The Central Alabama Veterans Health Care System (CAVHCS), National Park Service (NPS) and the American Association of Retired Persons (AARP) are joining forces to honor Dr. Mildred Dixon on her 100th birthday. In 1957, Dr. Dixon became the first female to be hired as a full-time podiatrist in the Veterans Administration, as the Department of Veterans Affairs was then known. She was also the VA’s first African-American podiatrist.

Although Dr. Dixon worked primarily at the VA medical center in Tuskegee, where she is credited with establishing the facility’s first podiatry residency program, she also spent several years working part-time at VA facilities in Montgomery and Tuscaloosa. Dr. Dixon retired from the VA in 1985. “I cannot overstate the significance of Dr. Dixon’s contributions to the profession, to America’s veterans, and to the community she serves,” said Dr. Jeffrey Robbins, National Director of Podiatric Service (VACO). “She stands as an inspiration to us all for a life well-lived in service to others.”

MACRA 'Escape Clause' Lets Docs Exclude Satisfaction Ratings in Determining Pay
Most physicians have long been critical of patient satisfaction surveys. They question whether the surveys truly measure quality and whether it's fair to use them to determine physician bonuses. They will finally get their wish under the new physician payment system under the Medicare Access & CHIP Reauthorization Act. The 90% of physicians expected to use the standard incentive program (dubbed MIPS for Merit-based Incentive Payment System) will have the option of excluding patient satisfaction ratings from the formula for determining rewards or penalties to their pay.
The CMS escape clause came in response to the general hostility most physicians have toward the surveys. In a survey for Modern Healthcare conducted by New York City-based SERMO, an online physicians' social networking website with over 600,000 members around the world, only 9% of 2,100 respondents thought patient satisfaction surveys accurately reflect the quality of care.

Doctors Spend Twice as Much Time with EHRs Than With Patients
For every one hour that ambulatory care physicians spend with patients, they spend nearly two hours on electronic health records (EHR) and desk work during a regular office day. They also spend an additional one to two hours of personal time at home each night to “keep up,” according to a time-and-motion study published September 6, 2016 in Annals of Internal Medicine.
So much time spent on EHRs instead of on patient care could be contributing to physician burnout, according to the study which was led by researchers from the American Medical Association (AMA). The researchers found that physicians spent 27% of their total office day on direct clinical face time with patients and more than 49% of their time on EHRs and desk work. In the exam room, physicians spent about 53% of the time on direct clinical patient interaction and 37% on EHRs and desk work.

Hospital Ownership of Medical Practices Grows by 86% in Three Years
Hospital ownership of physician practices has increased by 86% in the last three years, according to a new report. The analysis, conducted by healthcare consulting firm Avalere Health and the not-for-profit Physicians Advocacy Institute, found that from 2012 to 2015, hospitals acquired 31,000 physician practices in the U.S.
In 2012, about one in seven physician practices were owned by a hospital. In mid-2015, one in four medical practices, or 67,000 practices, were owned by hospitals. The report also found that in the same three-year period, physicians employed by hospitals increased by 50%. In 2015, about 140,000 physicians were employed by hospitals or systems, a rise from the 95,000 physicians who were employed by hospitals in 2012. Overall, about 38% of physicians in the U.S. are employed by a hospital or system, according to the report.

7 Reasons Why Physicians Work Locum Tenens - Part 4
Most physicians have heard of locum tenens and many have worked alongside locums doctors in a hospital or clinic. However, the majority of doctors are unaware of the benefits of locums or even how these temporary assignments really work.
The reasons physicians do locums vary but here are seven primary motives for working locum tenens:
4. Money on the side — Locums is a great way for physicians looking to make a little extra money on the side. Picking up a weekend shift or using vacation time to work locum tenens provides extra cash that can be used for student loan debt, savings, or other expenses. While most locum tenens physicians take assignments here and there, some physicians choose to book assignments back-to-back and make locum tenens their full-time job.

Query: Prescribing Medical Marijuana by Podiatrists
Now that nurse practitioners will be able to prescribe medical marijuana, I have two questions:
1) Where do we stand legislatively? As we are licensed to prescribe opiates?
2) What is our position as a profession? Do our patients with neuropathy, and chronic and ischemic pain qualify for this treatment?

I am in the final stages of closing on the sale of my practice. I know the following comment will not sit right with some, but the valuation of a practice should be a combination of knowledge of one’s practice and, more importantly, what the market dictates. Yes, it's nice getting a valuation but let's face it, the price you get is solely determined by what a buyer is willing to pay.
I used the 50-90% of gross as a very basic formula. I ended up getting 70% of my gross. Yes, I wanted more, but I got a price that was very fair to both the buyer and me. I invested a little more time in marketing via the classified section in PM News, but the money I saved not using an agent more than made up for the price I would have liked to have gotten.

RE: Outrageous Shoes of the Day
California Wildlife Center recently rescued a Northern Mockingbird whose feet were knuckling, meaning she was unable to open her feet to stand or perch. This was causing the bird to have to stand on the tops of her toes which were curled under and causing her additional injury.

CWC veterinary staff created “snowshoes” for the mockingbird to retrain her feet to open and allow the injuries to her toes to heal. The treatment was successful and the bird is now snowshoe-free and on the road to full recovery!
