REMEMBERING 9/11

Editor’s note: On this, the 14th anniversary of 9/11, we continue our proud tradition of publishing the following note from the editor, which was published in PM News on 9/12/01 and circulated via the Internet throughout the world.

It is just after 3 AM here in New York City and I simply can't sleep. Looking out the window of my bedroom on the 17th floor, there is an unobstructed view of the Manhattan skyline. I search again for the once familiar sight of the two tall white buildings that marked the lower end of the skyscraper-filled island. All I see are some eerie search lights of the rescue crews.

Like everyone else, I have been in shock all day, particularly since I witnessed in real-time the morbid views of the second plane hitting the World Trade Center and the sequential collapse of both towers.

The gamut of emotions runs from shock, fear, sadness, to anger. Barely one half-century after the Holocaust, despite all our technological advances, man remains uncivilized. The realization sets in that the terrorists who committed this crime could have and still could set off a nuclear bomb, killing millions. The safety and security we have taken for granted in America is and will never be the same.

My phone has been ringing all day. Family and friends call to determine that everyone is safe. My mom calls to tell me my brother was scheduled to be at the WTC at 9 AM and was at the subway station when the first plane hit. Had the plane hit five minutes later, he would have been on an elevator headed for certain death.

Unfortunately, many thousands were not so lucky, including many employees of Blue Cross/Blue Shield, which only a few years ago moved from midtown to this location. There will be many tears shed as the names of those murdered in this horrific disaster become known, many unnecessary funerals to attend. So many innocent lives lost for no reason. So many families destroyed.

Later this morning, my wife and I will donate blood and attempt to do what we can in this time of crisis. We know the entire podiatric community will collectively pray for those affected by this catastrophe and provide whatever support is needed.

FL Podiatrist Meets with Florida Surgeon General’s Office
 
The leadership of the Florida Diabetes Advisory Council met recently with the state’s number two health officer to present annual recommendations regarding issues affecting Florida citizens who have, or are affected by, diabetes. Dr. Jennifer Marks, current chair and Dr. Chet Evans, immediate past chair, proposed recommendations to Florida Deputy Secretary of Health, Celeste M. Philip, MD, MPH that could help improve the quality of life and benefit those Floridians who suffer from the burden of diabetes.

Dr. Marks is a diabetic endocrinologist at the University of Miami Miller School of Medicine and is Director of the Diabetes Management Program at the Miami VAMC. Dr. Evans is Executive Director of the Rollins College Center for Health Innovation and former Vice President for Medical Education and Program Development at Lake Erie College of Osteopathic Medicine.

APWH Writes Rebuttal Letter To Medicare Regarding Amputation LCD 
 
The Academy of Physicians in Wound Healing (APWH) recently submitted a rebuttal letter signed by APWH President Paul Glat, MD and APWH Executive Director Steven Kravitz, DPM to Medicare regarding legislative changes that would restrict podiatrists’ ability to perform “comprehensive functional assessments on patients with amputations affecting the foot. This LCD would preclude podiatrists from performing the evaluations on their own patients as well as when another physician refers a patient for an in-person functional assessment.

While there were additional aspects of the draft LCD also opposed by the APWH, the letter concludes “that which is most striking is omitting (DPM) from the definition of a “Licensed or Certified Medical Professional (LCMP). This has far reaching implications that would negatively impact patient care and in fact has potential of putting patients at increased risk by having professionals less trained in the subject area being forced to provide these services.”

QUERIES (CLINICAL)
Query: Low-Grade Fibrous Neoplasm
 
My patient is a healthy white female in her early 70s. She felt a painful lump on top of her left foot around the 2-4 metatarsal shaft areas for a week. She denies trauma. There is mild swelling, but no redness or bruising. The lesion glows with direct light. I thought it was a ganglion cyst and aspirated a watery and clear fluid, which is not typical of a ganglion.
 
The pathological review and results came back as "Scant non-viable debris and sparse acute inflammation; largely acellular specimen; malignant cells are not identified." It was deemed as a non-specific benign and low-grade fibrous neoplasm. Before the results were ready, she came back 6 days after the first aspiration because the lesion grew back to its original size and was painful. I aspirated again. An order was given to the patient for an MRI with contrast. I was planning to inject her with a steroid. Any suggestions?

Query: Office Ally’s Practice Mate 
 
I am looking at Office Ally’s Practice Mate product for billing only. To anyone who has it, how was the process of importing patient data and CPT codes? It does not appear to have a CPT library built in. Did you actually have to add your own with CPT descriptions and pricing details? What else should I be warned of, or what positives should I be aware of? How simple is the patient billing and EOB posting process?

I recently posted for a nearly full-time medical assistant position. I posted ads to Craigslist and local Facebook group pages. You can find them by doing a search by typing in your town/surrounding towns on Facebook. I think that the content of the ad is vital, but I can tell you that, with no exaggeration, I had close to 125 responses to my ads. I could write a book about the often unbelievably poor quality of close to 90% of the responses, but that’s another story for another time.

I had to do this a couple of years ago. The lightbulb is very difficult to see and is hidden, at least on my machine. I called the EXCEL company and they told me how to perform the task and that I could find a replacement bulb at a local hardware store for under $10. I went to two specialty hardware stores with unusual bulbs and, although they were the same model number, they did not work. Eventually, I had to order the bulb from the company which I believe was $96. I think I remember it coming from Japan or something. It was outrageous, but I finally got the machine working again. The company which fabricated the unit may be willing to help you as they helped me.

I read an excellent article published in the NY Times last year that shows a compelling story about why the government needs to step in and reign in out-of-network billing practices. 
 
As a U.S. citizen, small business owner, and practicing podiatrist, I see that healthcare is ripe for reform. I'm not an economist, and I won't pretend that I have the answers for this problem. We are on track to spend close to 20% of our GDP for the year on healthcare, which definitely seems like a big problem to me. Bigger, is that in many areas, our healthcare system fails large segments of the population. 
 
Healthcare is fundamentally different from other goods and services. Nobody chooses to be sick or injured, and when they are, they are often in no condition to make a financial decision regarding their care. We all took an oath, and part of that oath was to take care of our fellow man. Do not confuse the out-of-network pair of orthotics for $700, with the out-of-network craniotomy that winds up bankrupting the patient with a $70,000 bill that their carrier won't pay.
