REMEMBERING 9/11

Editor’s note: On this, the 13th anniversary of 9/11, we continue our proud tradition of publishing the following note from the editor, which was published in PM News on 9/12/01 and circulated via the Internet throughout the world.

It is just after 3 AM here in New York City and I simply can't sleep. Looking out the window of my bedroom on the 17th floor, there is an unobstructed view of the Manhattan skyline. I search again for the once familiar sight of the two tall white buildings that marked the lower end of the skyscraper-filled island. All I see are some eerie search lights of the rescue crews.

Like everyone else, I have been in shock all day, particularly since I witnessed in real-time the morbid views of the second plane hitting the World Trade Center and the sequential collapse of both towers.

The gamut of emotions runs from shock, fear, sadness, to anger. Barely one half-century after the Holocaust, despite all our technological advances, man remains uncivilized. The realization sets in that the terrorists who committed this crime could have and still could set off a nuclear bomb, killing millions. The safety and security we have taken for granted in America is and will never be the same.

My phone has been ringing all day. Family and friends call to determine that everyone is safe. My mom calls to tell me my brother was scheduled to be at the WTC at 9 AM and was at the subway station when the first plane hit. Had the plane hit five minutes later, he would have been on an elevator headed for certain death.

Unfortunately, many thousands were not so lucky, including many employees of Blue Cross/Blue Shield, which only a few years ago moved from midtown to this location. There will be many tears shed as the names of those murdered in this horrific disaster become known, many unnecessary funerals to attend. So many innocent lives lost for no reason. So many families destroyed.

Later this morning, my wife and I will donate blood and attempt to do what we can in this time of crisis. We know the entire podiatric community will collectively pray for those affected by this catastrophe and provide whatever support is needed.

NYCPM Welcomes Class of 2018 With White Coat Ceremony
 
Ninety-seven members of the Class of 2018 were welcomed by the New York College of Podiatric Medicine (NYCPM) at its annual White Coat Ceremony, in which the future Doctors of Podiatric Medicine receive and don their white coats and together recite the Hippocratic Oath. The event took place at the New York Academy of Medicine in Manhattan. After opening remarks and introductions by the Hon. Louis L. Levine, MA, LHD, President and CEO of NYCPM, the Treasurer of the Board of Trustees, Peter J. Norton, BA, LHD, greeted the assembled College community and guests. Rock G. Positano, DPM, MSc, MPH, Deputy Board Chairman, then told the students that they will “train with the best in the country.”

Dr. Joseph C. D’Amico, DPM, Adjunct Professor of Orthopedics and Pediatrics at NYCPM and an NYCPM alumnus (Class of 1973), delivered the evening’s keynote address. He advised the students that they were about to put on the “most famous uniform in medicine.” He asked the students to wear their white coats with “honor, pride, and respect for the profession and patients you treat.”

Ballet Flats No Better Than Flip-Flops: NY Podiatrist

Many of the same problems found in flip-flops pertain to ballet flats, as they offer no arch support, says Dr. Dominic J. Catanese, a podiatric surgeon at Montefiore Medical Center in New York. "They have no cushioning or support, which can lead to heel pain, plantar fasciitis and pain under the ball of the foot." Those without foot problems or pain may be able to wear them, but otherwise choose sandals with a contoured arch and a heel cup, suggests Catanese.

Podiatric Services to Increase in Malta
 
An agreement was reached between the Department of Health and The Malta Workers' Union (UHM) today to increase podiatric services. This agreement will ensure that the best quality of service will be provided to patients requiring podiatry services across the health sector. This will lead to an increase in professionals and support staff in this service while proper procedures will result in a more transparent and professional service to the public. UHM Secretary General Josef Vella and Parliamentary Secretary for Health Chris Fearne expressed satisfaction at this outcome and agreed on the importance of dialogue to ensure the best interests of staff and patients.

ONC Scraps Proposed 2015 Edition EHR Testing Criteria
 
The Office of the National Coordinator (ONC) for Health Information Technology has abandoned its proposal to create non-binding testing and certification criteria intended to prepare EHR vendors for coming requirements for electronic health-record systems. In a 187-page final rule leased Wednesday, the ONC formally scrapped the plan for a voluntary 2015 Edition of EHR testing and certification criteria.
 
The ONC said Tuesday, “We note that EHR technology developers do not have to update and recertify their products to the 2014 Edition Release 2, nor do participating hospitals, physicians and other eligible professionals have to upgrade their existing 2014 Edition software to Release 2,” according to the rule. Rather, it asks that participants merely consider whether Release 2 “offers any opportunities they might want to pursue.”

Query: Proper Timing for EHR Attesting
 
If a practice received an incentive payment in early February (for attesting for 2013 at end of December), and is now getting ready to attest for the 3rd quarter of this year, will it qualify for two incentive payments in one calendar year? In other words, will Medicare provide a second payment this year or should one wait and attest for 2014 at the end of the year?

RE: Confusion Over Billing for L3020

We have frequently heard/"known" that Medicare does not cover L3020 UNLESS it is attached to a brace (this would support the KX modifier). NONE of our L3020s are attached to a brace, so we have the patient sign the ABN, and unless the secondary covers it, patients must pay upfront. Ultimately, the patient still wants us to bill NHIC, which we do with the GY modifier. We get a denial and then USUALLY the secondary carrier won't pay because the primary carrier didn't pay. But we knew that and have the patients' money already upfront.
 
Now the problem----we switched from in-house billing to a billing service, and they billed a whole bunch of the L3020s and used the KX modifier, and of course we got paid. I printed the documentation showing that the device must be attached to a brace, but the billing service says that they bill for all of their podiatrists this way and have never had a problem. Now I am REALLY panicking because everything I have always heard/read was that in order for L3020 to be covered by NHIC, it must be attached to a brace (we meet all other requirements), so I have lost total faith with this new service. OR we have been billing patients for a long time when we actually could have gotten reimbursed by NHIC instead---more panic! Please advise.

We must stop telling the public that shoes cause hallux valgus. In a recent post, a prominent podiatrist publicly stated that shoes can cause hallux valgus and can prevent the deformity from getting worse. I am not trying to insult anyone, but this is nonsense. If this were true, why would there be so many unilateral hallux valgus deformities? The patient wore the same shoes on both feet. If this were true, which shoes caused the deformity as I am sure the individual wore many styles and types of shoes over a lifetime. If this were true, then taping the deformity in the opposite direction would reduce it, and we all know that cannot happen.
 
Hallux valgus is an inherited biomechanical condition. External environment has no effect on the development of the condition, but I do agree that it can exacerbate the symptoms. I do agree with the statement that the only way to "cure" the condition is surgery. Shoes and orthotics can reduce symptoms but not change the structure of the foot.
